comes. The linchpin to that is ensuring that clinicians
regularly follovi best practices and adhere to evidence-
based protocols.
"If this [transformation] is about value and value equals qual-
ity divided by cost, then it makes sense that you look at the evi-
dence," says Joseph Pepe, M.D., CEO of Catholic Medical Center,
Manchester, N.H.
Pepe, who served as CMC's chief medical officer for 12 years
before moving into the chief executive role in 2012, acknowledges
that one of the biggest stumbling blocks to instituting evidence-
based practice more broadly is the fear that it is "cookbook medi-
cine." That's a passé notion, he says. Evidence-based care is not
only about following results from the most recent clinical studies,
but blending that with a patient's values and desires, as well as
relying on a physician's judgment.
"Physicians have gotten a bad rap," says Jean Slutsky, direc-
tor of the Center for Outcomes and Evidence at the Agency for
Healthcare Research and Quality, when talking about the percep-
tion that doctors routinely reject the move toward evidence-based
care. "Physicians are lifelong learners. The very nature of what
they do is about learning."
A 2008 AHRQ handbook on implementing evidence-based
care supports the notion that this is not a completely rigid process.
It defines evidence-based care as "the use of current best evidence

The advent of the electronic health record (EHR) and other large electronic datasets has revolutionized efficient access to comprehensive data across large numbers of patients and the concomitant capacity to detect subtle patterns in these data even with missing or less than optimal data quality. This chapter introduces two approaches to knowledge building from clinical data: (1) knowledge discovery and data mining (KDDM) and (2) practice-based evidence (PBE). The use of machine learning methods in retrospective analysis of routinely collected clinical data characterizes KDDM. KDDM enables us to efficiently and effectively analyze large amounts of data and develop clinical knowledge models for decision support. PBE integrates health information technology (health IT) products with cohort identification, prospective data collection, and extensive front-line clinician and patient input for comparative effectiveness research. PBE can uncover best practices and combinations of treatments for specific types of patients while achieving many of the presumed advantages of randomized controlled trials (RCTs).

Cancer ranks as a leading cause of death and an important barrier to increasing life expectancy in every country of the world.1 According to estimates from the World Health Organization (WHO) in 2019, cancer is the first or second leading cause of death before the age of 70 years in 112 of 183 countries and ranks third or fourth in a further 23 countries . Cancer's rising prominence as a leading cause of death partly reflects marked declines in mortality rates of stroke and coronary heart disease, relative to cancer, in many countries.
Between 30–50% of all cancer cases are preventable. Prevention offers the most cost-effective long-term strategy for the control of cancer. WHO works with Member States to strengthen national policies and programmes to raise awareness and, reduce exposure to cancer risk factors, and also ensure that people are provided with the information and support they need to adopt healthy lifestyles.
To strengthen national efforts to address the burden of cancer and other noncommunicable diseases (NCDs), the WHO Global Action Plan for the Prevention and Control of NCDs 2013–2020 provides a road map to reduce premature mortality from NCDs by 2025 through targeting many of the risk factors below:

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

Rationale for EBCC ■ In a complex field and rapidly changing as oncology, clinical practice guidelines (CPG) are an important tool in patient care. ■ Despite around millions of published papers from clinical trials; much of it presenting conflicting results, sometimes derived from poor-quality research methodology. ■ Also, CPGs don't replace individual clinical expertise, and they can’t be used in the absence of the patients’ specific information. Factors such as comorbidities and individual preferences need to be considered. ■ The ability to find and evaluate all the relevant studies, and draw robust conclusions from the totality of the evidence, takes skill and practice, and this is called “Evidence Based Cancer care”
