The document discusses hand hygiene and efforts to reduce healthcare-associated infections. While hand hygiene compliance has improved due to numerous interventions, evidence shows infections have not significantly decreased. Other factors like overcrowding, understaffing, and contamination of the hospital environment may negate the benefits of hand hygiene. Strict enforcement of hand hygiene policies alone may not be effective and a balanced approach is needed that addresses systemic issues in healthcare systems.
Effectiveness of Low-Level Lasers in the Management of Recurrent Aphthous Sto...DrHeena tiwari
This study assessed the effectiveness of low-level lasers in treating recurrent aphthous stomatitis. 30 subjects with minor, major, or herpetiform aphthae were treated with lasers and their pain was evaluated using VAS scores. For all groups, pain was significantly less after 24 hours and further reduced at 72 hours compared to pre-treatment levels. At 1 week, pain scores were minimal or zero. The results demonstrated that low-level lasers provide effective pain relief and accelerated healing for recurrent aphthous stomatitis lesions.

2) Results showed that staples significantly reduced surgical site infections compared to sutures, especially in incisions 5-10cm and clean wounds. Staples also significantly reduced postoperative pain levels.
3) Staples yielded better cosmetic results, with superior scar appearance rated by patients and observers, especially in incisions under 5cm and over 10cm.
- The document discusses evidence from systematic reviews and meta-analyses on the use of advanced wound dressings and antimicrobial dressings for chronic wounds such as diabetic foot ulcers, pressure ulcers, and venous leg ulcers.
- The evidence is generally of low quality, and there is insufficient evidence to conclude that any advanced or antimicrobial dressing is more effective for wound healing compared to other dressings.
- Dressing selection should be based on individual clinical assessment and cost-effectiveness considerations. Silver dressings should only be used when there are signs of infection.
Cutimed® Epiona is a new 3D MatrixTM technology that was shown to fast-track healing in a clinical study of 31 patients with chronic wounds. After 3 weeks, the mean reduction in wound area was almost 40% and 14 wounds were reduced by 50% or more. Complete wound healing occurred in 3 wounds within 3 weeks. The study demonstrated Cutimed® Epiona's ability to enhance tissue generation and promote healing in stalled wounds through its unique porous structure and scaffold that mimics the extracellular matrix.

This document discusses a proposed solution to utilize design thinking and IoT technology to prevent pressure ulcers in healthcare. The system would continuously monitor patients' positions and health parameters like temperature, pressure, mobility and activity using sensors. It would alert caregivers when patients need to be repositioned according to hospital guidelines. The IoT technology allows for centralized monitoring and recording of patient data. In initial tests, the system successfully tracked patient positions and identified common poses. The proposed solution could significantly improve compliance with turning guidelines and help reduce hospital-acquired pressure ulcers.

This article proposes a new classification system for wound dressings called the Amit Jain classification. It divides dressings into 3 classes based on complexity: Class 1 are simple dressings like hydrogels and foams; Class 2 are more complex like collagen-based and growth factor dressings; Class 3 are the most complicated like skin substitutes. The classification aims to help practitioners easily understand dressing types and choose appropriate ones based on wound characteristics and cost. Over 500 dressings exist but only a small portion are commonly used, so this classification simplifies the options into categories to aid clinical decision making.
This study aims to systematically compare and
contrast the two most commonly used techniques of
tonsillectomy- Cold tonsillectomy and cobilation tonsillectomy.
Three different age group of patients were examined and
operated. The total number of patients was 104. There were 52
patients each of cobilation and cold tonsillectomy. The
specifications being following: 35 patients of age between 3 to 7
years, 6 patients of age 7-12 years and 11 patients of age greater
than 12 years.
The result of this study showed that there is no significant and
noticeable difference between the two procedures of operating
tonsils. However slight differences in the post operative pain and
primary and secondary bleeding was seen. The operation time
was considerably lower in patients of younger age.
A 32-year-old male patient presented with a large wart on his second finger and numerous warts on his right foot. Warts are a common skin infection caused by HPV that can persist for many years. Diagnosis of warts is usually not difficult to make clinically or histologically. Treatment options for warts include cryotherapy, which the author uses to treat digital warts, CO2 laser for foot warts, salicylic acid, photodynamic therapy, lasers, and other topical treatments. While many treatments are available, evidence for their efficacy is sometimes limited.

This document evaluates the use of a Hydrofiber foam dressing on a large, necrotic foot wound in a paraplegic male with diabetes. The wound showed impaired healing over 4 years. After debridement and application of the Hydrofiber foam dressing, the wound rapidly reduced in size and healed within 4 months, restoring the patient's confidence and reducing dressing changes from every other day to twice weekly within 2 weeks. The Hydrofiber foam dressing was found to provide a moist wound environment, promote granulation, and was cost-effective by reducing clinician time spent on dressing changes compared to other dressings.

This document summarizes a case study comparing the cost benefits of using a microfiber debridement pad versus conventional treatment methods for healing a venous leg ulcer. The conventional treatment took approximately 9 months and 28 nurse visits at a cost of 773 euros. In contrast, using the microfiber pad required only 4 nurse visits over 5 months, with the patient performing most debridement, resulting in a total cost of 230 euros and faster healing. The conclusion is that the microfiber pad promotes easier wound cleaning, faster healing, and significant cost savings compared to conventional methods.

A 62-year-old man with schizophrenia, diabetes, and venous leg ulcers was treated over 11 months. He had two large sloughy ulcers on his right leg. Treatment involved debriding the ulcers and skin with a monofilament product, dressing the wounds with superabsorbent and collagen dressings, and using compression therapy. The debridement improved the patient's participation and trust in treatment. Over time the ulcers granulated and reduced in size, though a new ulcer later developed. The patient-centered care including debridement and wound preparation led to largely successful treatment of the ulcers.

This case study describes an 80-year-old woman with atypical venous ulcers that were resistant to various treatments including electrostimulation, negative pressure therapy, and skin grafting. Application of a TLC-NOSF dressing along with venous compression stimulated healing, with the ulcers fully healed within 6 weeks. The dressing promoted granulation tissue growth and re-epithelialization over successive applications where other treatments had failed. This case suggests that TLC-NOSF dressings may effectively treat difficult-to-heal venous ulcers when combined with compression therapy.

The document summarizes the experience of two clinicians in treating traumatic wounds with a TLC-NOSF dressing. It describes two case studies where the dressing helped heal wounds that had previously failed to respond to conventional treatments. In the first case, a 76-year-old woman's leg wound healed completely within 9 weeks with the dressing. In the second case, a 23-year-old woman's wound under a skin graft epithelialized fully within 8 weeks with the dressing. The conclusion is that the TLC-NOSF dressing provides an effective first-line treatment alternative for chronic traumatic wounds.

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

INTRODUCTION
Chronic wounds are characterized by
their great prevalence, their high costs
and the incapacitation for the activities
of the daily life of our patients.
With this new modernization in
dressings we contributed not only to
the humidity degree necessary to
debridement to granulate and to
epitalitation a wound but that
simultaneously the dressing is able to
control its excess of liquids and to give
at every moment whatever it needs.
Relevant data significantly important to
have presents/displays it in our new
performances in treatment of hurt
since we diminished the cures, the
material, and simultaneously the costs
and our time of dedication.
4. AIM
4
Our objective is to demonstrate the
effectiveness with cadexomer
iodine® in treatment of wound in
patients with venous ulcers of degree
III and signs of infection without use
dressings silver.
To demonstrate the need of a
change in the treatment of the
wounds that offers a global approach
to us of the same ones, with the
purpose of obtaining a fast and
effective healing.
5. MATERIAL AND METHODS
Descriptive study in 15 patients of a primary health service with venous
ulcers of degree III and signs of critical colonization and infection with the
cadexomer iodine®.
We considered, regarding the skin, painful wound, surrounding area of the
wound, exudation, colonization, necrosis, and the need to have a use full
solution. The control of the humidity, colonization and reduction of pain were
the most important goals.
Previous deciding not to use silver dressings, we reviewed guidelines SIGN
(Scottish Intercollegiate Guidelines Network), NICE (National Institute for
health and clinical Excellence) Cocrhane Library.
Finally decided to use SIGN to reflect the most recent evidence on
chronic venous leg ulceration.

Case:
65 year-old male
Locally infected superficial
Treatment before: Silver, and
different dressings every 48-72h.
Ulcer duration: 2 weeks
8. CASE 2
8
Case:
75 year-old female
Locally infected superficial
Treatment before: Silver, and
different dressings every 48-72h.
Ulcer duration: 1 year in both legs
9. RESULTS
9
A Cochrane systematic review reviewed three RCTs comparing
silver dressings with hydrocellular foam, alginate or best practice.
Two studies showed no difference between products and one
reported statistically faster healing rates but did not report complete
healing. All three studies were small and of short duration and could
not be combined for a meta-analysis.49
An RCT (n=213) comparing, silver dressings and non-adherent
dressings (in addition to standard compression), found no significant
differences in median time to complete healing or healing rates at
three, six and 12 months.
A Silver dressings are not recommended in the routine
treatment of patients with venous leg ulcers
10. RESULTS
After application for 2 weeks with wound dressing we observed a
good evolution of skin surrounding the wound, a reduction of
exudate, the removal of necrotic tissue, the reducing the size of
the ulcer, an atraumatic treatment and the supply of an adequate
humidity to the wound frequently to absorb according to the
necessities of the wound simultaneously.
After the application to 15 patients during 10 days, we observed
the reorganization of the edges of the wounds skin, exudate
control in each patient without macerating the new skin, the
reduction of the dressing changes to 2 times per week and the
easy procedure in the application by the professional as for the
patient at the time of making the cure.
Being an atraumatic treatment to provide the wound with the right
humidity frequently.

● SIGN 2012(SCOTHIST INTERCOLLEGIATE GUIDELINES NETWORK)
We support this new technology in dressings, which
unites the characteristics of reducing costs, and
improving at the same time healing rates and the wound
treatment itself. There is no doubt that this new wound
dressing, CADEXOMER IODINE®, meets this requirements.
11
12. CONCLUSIONS
After this first experience with the use of these dressings
we saw a reduction of the dressing change, an excellent
exudate control with a lower risk of macerations.
The wound dressings were well tolerated and the easy
applicability and painless removability are much valued
by both treating persons and patients.
Evidence of variation in both healing rates and
recurrence rates of venous leg ulcers highlights the
need for an updated evidence based guideline to
support practice.
