Background: Cervical screening through conventional cervical cytology is most commonly used throughout the world. The Cervical cancer is the second most common cancer worldwide and in developing countries, the leading cause of death. It is one of the most preventable and curable of all cancers.
Objective: To Study the role of Pap smear in detecting neoplastic and non-neoplastic lesions of cervix and to determine the occurrence of various lesions in remote area of Bagalkot.
Methods: This is prospective study of 240 women with age group 20 to 60 years was carried out from May 2015 to June 2016 cytology section of pathology department, S. Nijalingappa Medical College & H.S.K Hospital & Research Centre, Bagalkot, India. Pap smears were prepared, fixed, stained and carefully examined.
Results: In this study, Reactive cellular changes associated with inflammation was the most common with 182 cases (75.8%) followed by Low-grade squamous intraepithelial lesions (SIL) with 11 cases (4.5%), then atypical squamous epithelial cells of undetermined significance 8 cases (3.3%), High-grade squamous intraepithelial lesions with 5 cases (2.1%), Atrophy with 3 cases (1.3%) and Atypical Glandular Cell in 3 cases (1.3%). The average age of women for all the epithelial abnormalities was 40 years.
Conclusion: This study will increase awareness of the Pap test and cervical cancer, thereby paving a way for the prevention of cervical cancer.

Chapter 20: Cervical Cancer Screening and Diagnosis
Diagnostic testing is important in early detection of cancer (Hubert & VanMeter, 2018).
Hubert & VanMeter (2018), state that the examination of tumor cells is the only definitive
method to diagnose cancer cells. Other tests should be used in conjunction with cytologic tests
and to monitor treatment and follow up after a diagnosis is made (Hubert & VanMeter, 2018).
Cytologic testing, are used to screen individuals as high risk, to make a diagnosis, and to follow a
clinical plan of care or monitor (Hubert & VanMeter, 2018). To make a diagnosis, clinicians use
histologic and cytologic examinations to gather cells that are sent to a lab for further examination
(Hubert & VanMeter, 2018). In some cases, biopsies are retrieved and in other cases exfoliative
cytology is used to make a dependable confirmation of malignancy (Hubert & VanMeter, 2018).
For all tests, good technique and preservation of the sample is key in obtaining an accurate
evaluation (Hubert & VanMeter, 2018). The Papanicolaou (Pap) test is the standard of care for
screening, evaluating and diagnosing cervical cell changes that can lead to cervical cancer
(Hubert & VanMeter, 2018).
Cervical dysplasia, or atypical glandular cells on cervical cytology, are categorized based
on the degree of cellular change (Goodman & Huh, 2020). The three main categories of cervical
intraepithelial lesions (CIN) include: CIN-1 or lesser abnormalities, CIN 2 and CIN 3 (Goodman
& Huh, 2020). CIN-1 or lesser abnormalities includes atypical cells of undetermined significance
(AS-CUS) and low grade squamous intraepithelial lesions (LSIL) with HPV 16 or 18 infection,
or persistent HPV infection (Goodman & Huh, 2020). Low grade CIN has a low potential to
progress to malignancy (cancer) but high grade CIN has a high potential to develop into cancer
(Goodman & Huh, 2020). The Pap test has significantly contributed to the decrease in invasive
cervical cancer incidence however every year there are 12, 000 new cases of invasive cervical

cancer are diagnosed in the U.S (Zhao, et al., 2014). Zhao, et al. (2014) explored Factors

effort to highlight the importance of accurate interpretation of screening tests.

Cervical Cancer Care Seeking Behaviour Among Community Women, Jos-North, Plat...AI Publications
Most people are infected with HPV shortly after the onset of sexual activity. The screening rate has not reached the WHO's target. This study seeks to assess community women's care-seeking behavior toward cervical cancer screening participation. A cross-sectional design was employed, community women who met the criteria for inclusion were given a self-developed questionnaire conveniently. Results show that the majority (51.9%) of women were between the ages of 21-29, majority (83.0%) had only one sexual partner, majority (75.0%) were extremely poor with very low yearly income of less than 284,700.00 Naira, and 19.3% experienced gynecological symptoms. Majority (78.5%) have the fear of being diagnosed with cancer, and 69.6% fear exposing their genital area. Women agreed that HPV is the causative organism of cervical cancer and husband do not allow someone to touch their wives’ private parts. They moderately agreed that a woman must obtain consent from her husband before going for screening, that exposing their private part is culturally inappropriate. Only 14.1% of those surveyed had ever undergone a cervical cancer screening. Among the respondents who had screened, 57.9% had bad experience. There is a significant difference between age group and screening where majority who had screened were older women within the age group 30-65 years (X2=8.402; P-value=0.005) and having gynecological symptoms has positive influence on screening participation (X2=7.422; P-value=0.012). The majority (92.6%) believed that husband involvement and the caregiver's friendly attitude are among other facilitating conditions to screening. In conclusion, there was low screening participation among community women. Women's knowledge of cervical cancer and screening did not translate into participating in screening. Low socioeconomic status has a significant impact on screening, screening rates were higher among older women than younger ones, and experiencing gynecological symptoms has a positive impact on screening. It was therefore recommended that interventions aimed at enhancing care seeking behavior based on women's needs be implemented.

BACKGROUND Invasive Cervical Cancer ICC has been identified as the second most common cause of morbidity and mortality compared to other cancers among women in Cameroon. Cervical cancer can be treated e ectively if diagnosed early. Less than half the number of participants presented with good practice.The correlation between participants’ knowledge, attitude and practice showed that there was a significant association which therefore provides sufficient evidence to reject the null hypothesis. The result obtained in this study indicates how useful it will be to establish health education programs to increase women’s awareness and knowledge about cervical cancer. RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

Abstract:
Back ground: Cancer cervix is a leading cause of mortality and morbidity among women. Many studies have
proved that via and vili has sensitivity comparable to papsmear not only in a resource poor setting but in a well
equipped centre also. Present study compares the effectiveness of via-vili done in two centers with low resource
setting.
AIM:To compare the efficacy of our via-vili test with other centers test.
Methodology: During 1 year, 5285 women have undergone via-vili screening test at our hospital. Only 430
were positive. Same period 778 via-vili positive cases were referred from surrounding PHCs for further
management. The two groups were confirmed Colposcopy and biopsy. Results were compared between two
groups. Data were analyzed and interpreted by ‘Z’ test of proportions at 5% significance.
Results: TKMCH, out of 430, Colposcopy positive was 262 (60.9%) and among Colposcopy positive 35 (13.4%)
were biopsy positive. Among 778 referred positive cases Colposcopy positive was 318(40.8%) and biopsy
positive was 28 (8.8%). The confirmation of via-vili by Colposcopy was statistically significant (Z=6.802 and
P<0.001). But confirmation of Colposcopy positive by biopsy was not statistically significant (Z=1.745 and
P>0.05).
Discussion: The confirmatory Colposcopy positive 60.9% of our hospital was significantly greater than the
referred confirmatory 40.9%. It shows that our via-vili test was stringent and effective than the referred. The
biopsy confirmations (13.4% and 8.8%) were not statistically significant since it was performed at our hospital.
Conclusion: The statistically significant difference in via-vili outcome between rural and urban population
needs further detailed study. The via-vili screening test may be improved at primary health centers still more to
minimize the expenditures of Govt. as well as individuals.

I. Introduction
Cancer Cervix is the leading cause of mortality and morbidity in women.
Second most common cancer among women world wide. Nearly 5 lac new cases per year, ¾ of the world
burden occur in developing countries.Most common cancer among women in developing countries. Most
common cause of death among middle aged women.
Prevention and screening of cervical cancer is of paramount importance.
If detected early and treated optimally, would result in a very high cure rate.
Various studies have proved that via-vili is effective in identifying pre cancerous lesions and even more
effective, when clubbed with colposcopy.
This study compares the effectiveness of VIA and VILI(visual inspection after acetic acid and visual
inspection after lugol’s iodine) as a screening tool for CIN between Tertiary and Primary health care levels.
II. Materials and Methods
During a period of one year from june 2013 to May 2014, 5285 women between the age group of 30 –
60yrs were screened by via-vili by trained staff nurses in medical college hospital.
430 Via/Vili Positive cases were subjected to colposcopy by trained doctors.778 via vili positive
referral cases from periphery were also subjected to same procedure. Pregnant women, puerperal women, those
undergone hysterectomy, H/o Cervical cancer were excluded.

262 cases were colpo positive (60.9%) of which 35 cases were positive for CIN and cancer (13.4%).

Confirmatory colpoabnormal cases of 60.9% among via vili positive cases in the tertiary hospital
was significantly higher than in referred cases (40.9%) from PHC.

2. Criteria for test positivity
3. Inter observer variation
4. Coexisting infections, inflammation, metaplasia.
Via-vili screening test may be improved in primary health centres still more to minimize the expenditure to
govt. as well as to individuals.
