2. •EEG mempunyai manfaat untuk mengevaluasi pasien
dengan penurunan kesadaran.
•EEG dapat menunjukkan penyebab penurunan
kesadaran :
1. Disfungsi fisiologis otak difus (ensefalopati
metabolik, hipoksia, sepsis, intoksikasi obat
atau penyakit degeneratif)
2. Lesi fokal otak (lesi massa supratentorial,
massa subtentorial, lesi destruktif)
3. Epilepsi tanpa gerakan kejang
•Keterbatasan EEG : kurang spesifik

ENCEFALOPATI METABOLIK
Gambaran EEG :
Perlambatan irama latar belakang difus dari alpha theta
delta
Tingkat perlambatan selaras dengan tingkat perubahan
kesadaran dan menunjukkan keparahannya.
Semburan aktifitas delta intermiten secara general : dewasa
FIRDA
Tidak patognomonik, tidak spesifik untuk satu etiologi
tertentu

4. Encefalopati Hepar
Gambaran EEG :
Spike-and-slow wave complexes/Triphasic wave
(TWs)
• Morfologi gelombang 3 fase, mengelompok
• Amplitudo sedang – tinggi (100-300uV)
• Frekuensi 1,5-2,5 Hz
• Sering dominan di frontal & bisynkronous
Perlambatan gelombang latar belakang
Spike positif 14 & 6 Hz

5. Encefalopati Hepar
Gelombang Trifasik :
Penyebab : hepar, azotemia, hipoksia,
hiperosmolaritas, hipoglikemia, demensia Alzheimer.
Sulit dibedakan TWs karena encefalopati dengan
epileptiform (GNCSE).
Spike positif 14 & 6 Hz :
Ditemukan pd anak & remaja yg mengantuk/ tidur
ringan.
50% ditemukan pd Reye’s syndroma yang koma,
disertai aktifitas delta difus.

6. Encefalopati Hepar
Beda TWs pada encefalopati dg epileptiform , pada
encefalopati didapatkan :
Kompleks 1 gel durasi pendek & frek lebih tinggi drpd
GNCSE.
Tdp gel.paku tambahan sblm 1 gel.
Gel.latar belakang kurang lambat.
Fase 2 nya maksimal, minimal 50% dr semua gel.,
umumnya dr anterior ke posterior.
Meningkat setelah stimulasi.

7. Encefalopati Renal
Penyakit ginjal menunjukkan kelainan yg mirip dg
encefalopati metabolik lainnya.
Kejang dan epileptiform umum terjadi pada
encefalopati uremik.
Pada pasien dg sindrom ketidakseimbangan dialisis,
terjadi perburukan klinis dan gambaran EEG stlh
dialisis.

8. Encefalopati Renal
Gambaran EEG :
Mirip dg encefalopati metabolik
lainnya
Perlambatan progresif dari
gelombang latar belakang.
Aktifitas lambat dengan semburan
superimpose.
Kadang dg stimulasi cahaya, tdp
respon fotoparoksismal atau
fotomyogenik.

HIPOKSIA
Hipoksia mhasilkan berbagai pola EEG abnormal :
Letupan pola supresi.
Spike waves, sharp waves, atau TWs berulang pada interval
sekitar 1 detik dengan adanya supresi antar interval, atau letupan
pola supresi.
Pola periodik general.
Supresi general intermiten yang singkat, tanpa berhubungan
dengan adanya letupan.
Pola alpha / theta coma (ATC).
Pola spindle coma.

HIPOKSIA
Letupan pola supresi :
•Dikaitkan dg prognosis yg sangat buruk.
•Dapat tjd gerakan oro-facial-lingual spontan menyesatkan
penentuan tk kesadaran.
•Pola periodik general stlh gagal kardiorespirasi memp prognosis buruk.
•Tidak berhub. dg perubahan klinis, tp dg penurunan tk kesadaran.
•Supresi general intermiten yg singkat juga dikaitkn dg prognosis yg
jelek.
•Aktifitas delta dominan, supresi intermiten, letupan supresi, dan ECI
bernilai tinggi dlm menentukan prognosis yg buruk. 11
14. KOMA KARENA
HIPOKSIA
Pola alpha coma :
oDapat ditemukan setelah hipoksia akibat gagal kardiorespirasi
dan intoksikasi obat.
oFrekuensi kisaran alpha, tersebar luas, memp amplitudo
terbesar di anterior, tdk bereaksi dg membuka & menutup mata.
oTerdapat varian ATC komplit dan inkomplit
o88% pasien dg alpha coma menjadi fatal/vegetatif state
persisten.
o Juga didapatkan pada lesi batang otak.

INTOKSIKASI OBAT
•Menghasilkan bermacam2 pola EEG abnormal.
•Sebagian besar obat anestesi menghasilkan pola EEG yang serupa.
Gambaran umum EEG pd intoksikasi obat:
•Aktifitas cepat (beta) yg menyeluruh pd pasien koma curiga
•intoksikasi obat (barbiturat, benzodiazepin).
•Episode supresi intermiten.
•Letupan pola supresi.
•ECI.

INTOKSIKASI OBAT
Penyalahgunaan narkoba :
gelombang general, bisynchronous sharp complexes, periodik dan sering
berupa bentuk trifasik.
alpha coma dan spindle coma .
Obat-obat anestesi :
Ex. Propofol, midazolam.
peningkatan aktivitas beta dengan menghilangnya irama alpha.
Seiring dengan peningkatan dosis, frekuensi aktivitas EEG menurun,
sedangkan amplitudonya meningkat.
Letupan pola supresi terjadi pada dosis tinggi.
Alpha coma, spindle coma.
lebih lanjut, dapat mengakibatkan ECI

Drug Withdrawal (barbiturat) :
abnormalitas epileptiform spontan.
mungkin ada respon fotomyogenic atau fotoparoxysmal.
Drug Withdrawal (alkohol) :
EEG sering normal .
mungkin terjadi aktifitas cepat dengan amplitudo rendah yang
berlebihan.
respon fotomyogenic sangat jarang .
respon fotoparoxysmal tidak akan tjd pd withdrawal alkohol
yg tdk diobati atau setelah terapi kejang yg berhub dg alkohol19

Drug Withdrawal VPA :
menginduksi ensefalopati hyperammonemia.
gambaran EEG tidak spesifik.
mungkin ada perlambatan general atau kadang-kadang ada
gelombang trifasik.
jarang mengancam kehidupan.

21. KOMA AKIBAT INFEKSI SSP
Herpes Simpleks Encefalitis
Gambaran EEG :
PLED lateralisasi berulang stp 1-2 dtk.
BIPLED yang tidak sinkron.
perlambatan fokal temporal.
PLED terjadi dlm berbagai kelainan, paling sering infark atau tumor.
Dalam review 586 kasus PLED, etiologinya yaitu kelainan
serebrovaskular (35%), lesi massa (26%), infeksi (6%),
hipoksia (2%), dan lainnya (22%) .
PLED juga ditemukan pd pasien dengan kejang kronis atau lesi lama 21
24. ENCEFALOPATI ENDOKRIN
Encefalopati Hashimoto
Berhub dg tiroiditis Hashimoto, sindroma autoimun.
Menyebabkn perubahan tk kesadaran, kejang, myoklonus,
halusinasi.
Respon baik thd obat imunosupresif (SREAT).
Peningkatan kadar Ab anti tiroid peroksidase &/
Ab antithyroglobulin serum

25. ENCEFALOPATI ENDOKRIN
Encefalopati Hashimoto
Gambaran EEG :
Tidak spesifik, tp 90% biasanya timbul pd Encefalopati
Hashimoto
Perlambatan general sedang hingga berat.
TWs, kelainan epileptiform, respon fotomyogenic dan
fotoparoxysmal juga bisa ditemukan.
Peningkatan cepat pada aktifitas latar belakang
setelah pengobatan SREAT dengan steroid

26. KOMA AKIBAT LESI STRUKTURAL
Supratentorial
EEG biasanya abnormal :
Seringkali terdapat aktifitas delta polimorfic (PDA) yang fokal
kontinu pada sisi yang terkena.
Peningkatan aktifitas delta dan theta fokal.
Pada ggn struktural yang lebih dalam, biasanya muncul FIRDA.
Penekanan aktifitas fokal, penurunan frekuensi pada sisi yang
terkena, dan kelainan epileptiform fokal, seperti PLED.
Tidak spesifik untuk sebuah etiologi tertentu.

27. KOMA AKIBAT LESI STRUKTURAL
Subtentorial
EEG kurang bernilai untuk evaluasi dan prognosis pasien koma
dengan lesi subtentorial
Gambaran EEG normal pada pasien yang benar-benar koma
(bukan koma psikogenik), merupakan indikasi dari lesi batang
otak.
Adanya pola alpha coma pada pasien dengan lesi batang otak.

28. RINGKASAN
EEG dapat sangat membantu dalam evaluasi pasien
dengan perubahan tingkat kesadaran.
Karena pola-pola ini tidak spesifik untuk etiology
tertentu, EEG dapat membantu jika penyebabnya diketahui.
Jika ensefalopati etiologinya tidak diketahui, maka
rekaman berturut-turut diperlukan untuk prognosis.

32. Classification: burst-suppression. Note that this is a 15-second segment, to show
the periods of suppression (4-5 s) separated by the bursts. Suppression periods are
characterized by activity less than 10 mV.

33. Classification: electrocerebral inactivity. The recording demonstrates no cerebral activity
greater than 2 mV. Given the high sensitivity (2 mV/mm), a combination of ECG and 60-Hz 33
artifact often is present, as observed here.
34. Alpha coma. Unlike a normal alpha rhythm, the alpha activity observed here is not posterior
dominant, is continuous, and is nonreactive. If the entire record is present and the patient is

known to be comatose, this qualifies as alpha coma.
35. Beta coma. Prominent fast (beta) activity is noted at 15-22 Hz. To qualify as "excessive fast"
activity, the pattern has to be the predominant frequency and excessive in amount (ie, nearly
continuous and unreactive) and amplitude, ie, greater than the typical 30 microvolts of
the normal beta activity. Note that this pattern could be seen in an awake patient, so that35
the term "beta coma" is reserved for patients known to be comatose.
36. Spindle coma. Note the prominent spindlelike activity at 13-16 Hz. Typically, spindlelike activity
associated with coma is even more continuous than shown here, and unreactive. The term 36
"spindle coma" is reserved for patients known to be comatose.
37. Triphasic waves. Note the near continuous pattern of periodic triphasic waveforms, with a large
frontal positivity (downgoing) preceded and followed by smaller negative deflections. The wave
marked near the middle of the sample illustrates the classic anterior-posterior lag. This pattern is
typically unreactive. Note that a triphasic morphology is necessary but not sufficient to classify

a pattern as triphasic waves.
38. Encephalopathic EEG patterns in uremia. This 76-year-old woman has a history of dementia and
uremia. She has been stuporous for one week, and her BUN is 88. The patient is difficult to arouse,
but she is able to answer simple "yes/no" questions and follow a few simple commands. The EEG
shows continuous theta and delta activity. Auditory stimulation results in a reduction in voltage and 38
a decrease in amount of delta.
39. Subacute sclerosing panencephalitis. This is a 7-year-old boy who is comatose and having myoclonic
jerks. The onset of the illness began 14 months ago with deterioration of intellectual function and he has
become progressively unresponsive. This EEG shows stereotyped high-voltage (300-400 mV) bursts of 39
activity every 4-6 seconds.
40. creutzfeldt-Jakob disease. This is a 56-year-old female professor who became more withdrawn and
Increasingly forgetful. Over the next 4 months she deteriorated rapidly and became mute, bedridden,
and unable to eat with myoclonic jerks of her arms. The EEG done 6 months into her illness shows
a pseudoperiodic, well-organized 1- to 2-Hz biphasic and triphasic waves and very little other activity.
In this clinical setting, this EEG is virtually pathognomonic of Creutzfeldt-Jakob disease and is regarded as

a manifestation of severe gray matter disease involving the cortex and deep nuclei.
41. According to guidelines of the American Clinical Neurophysiology Society, the following
are minimum technical standards for EEG recording in suspected brain death: 15
•A minimum of 8 scalp electrodes
•Impedances between 100 and 10,000 ohms
•Integrity of entire recording system tested by touching each electrode individually to
obtain appropriately located artifact potential
•Interelectrode distances of at least 10 cm
•Sensitivity of at least 2 µV for 30 minutes of the recording, with appropriate
calibrations documented
•High-frequency filter (HFF) not set below 30 Hz and low-frequency filter (LFF) not set
above 1 Hz
•Additional monitoring techniques used as necessary to eliminate or prove waveforms
are artifactual
•No EEG reactivity to strong and thorough tactile, auditory, or visual stimulation
•Recording performed by a qualified technologist working under the direction of
a qualified electroencephalographer
•If ECI in doubt, EEG repeated after an interval (suggested 6 h) 41
