• Grant Re-cap: Discuss logic model
• School Level Re-cap: Review STEPS to SUCCESS tool and our
collaborative results.

• Individual reflection.
• Team discussion

• Felt tips (fine/ medium point)
• Markers / colored pencils
• Scotch tape and/or glue sticks

• Dodecahedron template on card stock
Activities:
• Using insights from the reflection exercise above think of 6
successes that you and/or your team has had since the beginning of
the program. Write these on the dodecahedron. Illustrate if you
would like.
• Using insights from the reflection exercise above, think of 6 goals
that you and/or your team would like to accomplish between now
and this summer. Write these on the dodecahedron. Illustrate if you
would like.
• Cut out the dodecahedron on the solid lines.
• Fold on the dotted lines.
• Assemble with Scotch tape and/or glue sticks.
• Display someplace where it will be a reminder to you or your
successes and goals.
