Sacral Neuromodulation (SN) is a therapy consisting of electrical stimulation of sacral nerves at S3 level delivered by a cylindrical electrode connected to a full-implantable neurostimulation device. SN in adult patients for urinary tract dysfunctions other than neurogenic modulates cortical and subcortical structures involved in urination, beside modulating structures with an important role in awareness of the rate of urination and filling of the bladder. A possible explanation for SN’s therapeutic effect is due to the activation and inhibition of supraspinal brain areas by the spinal cord, which normalizes urinating functions. A failed IVF cycle can be because of poor egg quality, sperm quality or uterine lining. It is assumed that all the stimulation egg pick up, laboratory procedures and embryo transfers have been done meticulously in previous attempts. We offer certain modifications in an IVF cycles for optimizing outcome in couples suffering from failed IVF attempts- Optimized stimulation protocol: The short antagonist protocol offers the best results in terms of selection of the best oocytes (eggs) in most cases. Selection of Sperm: In many cases, Intra Cytoplasmic Sperm Injection (ICSI) is offered as it has been suggested that it may improve fertilization rates and hence, overall pregnancy outcome. Our embryologist takes special care to select the best sperms for doing ICSI. Hysteroscopy: The hysteroscope aids us in picking up uterine abnormalities which are sometimes missed at routine ultrasound e.g., small polyps. It is also useful in washing and cleaning the uterus which sometimes may help in improving the outcomes. Endometrial scratching is also done at the same setting to improve the uterine receptivity. Intravenous Immunoglobulin (IYIg): IVIg seems to directly affect NK cell level and activity, by reducing their absolute numbers and increasing the expression of inhibitory receptors CD94 which potentially can improve pregnancy outcome. Vitamins and Antioxidants: DHEA, L Arginine, Zinc, selenium etc. are given to women and men as indicated to improve the egg and sperm quality. Atosiban: This is a uterine relaxant which is given during the embryo transfer. It helps in relaxing the uterus and therefore, improving the endometrial receptivity. Laser Hatching: Laser hatching of the embryos is performed on the day of embryo transfer to ensure that the shell of the embryo hatches easily. This allows the embryo to implant better. This is mainly suitable for embryos with thick shell, advanced age group and frozen embryos. ERA: ERA presumably detects the phase of the endometrium in which the embryo best implant. However, there is controversy regarding the actual benefit of this in improving the live birth rate. PGS: PGS is a way of detecting abnormal embryos thus may help in improving the pregnancy rates. However, each case must be individualized. Day of Transfer: Not all women will be benefitted by Blastocyst (Day 5 ) transfer as many seem to believe by studying the internet. The day of transfer should be individualized for each patient. Meticulous Transfer Technique: Embryo transfer is the final and one of the most crucial step of IVF. All embryo transfers at AFGC are performed by Dr Kaberi Banerjee who has taken special training in embryo transfer from UK.

Abstract: Prematurity used to be a major cause of infant deaths. The premature babies need improved medical and nursing techniques by highly competence nursing team. Material and Methods: This descriptive hospital based study was conducted at Soba university hospital, Khartoum state in the period from January to March 2014. The study aimed at assessing the knowledge and practices of pediatric nurses in neonatal intensive care unit concerning nursing management of preterm babies. The sample size compromised of 50 nurses that constituted the total coverage of study population during the period of the study. Data were collected using structured interview questionnaire and observation check list designed f or the study. The data was analyzed using statistical package for social sciences (SPSS). Results:The results obtained that the majority of nurses were knowledgeable about the characteristics of preterm babies, causes of prematurity, immediate nursing care of preterm, signs of hypothermia were adequate (100%, 92%, 100%,100% respectively). Half of them (50%) identify the breathing pattern of preterm baby. The nurses clinical performance were inadequate where 70% of them recorded pulse rate only when recorded the baby pulse.100% did not wear mask, 80% find a difficulty on selecting appropriate vein for sampling . Also 48% of nurses gave feeding incorrect and 60% of them did not aspirate gastric contents before feeding. Conclusion: The study concluded that the majority of pediatric nurses had adequate knowledge about prematurity, but they were lacking in their clinical skills to manage the preterm baby. So the study recommended continuous training programs for the nurses to refresh their knowledge and practices towards management of preterm babies to ideal standards.Keywords: Pediatric Nurses’ Knowledge, Practices Regarding Nursing Management, Premature Babies. Mothers who perceive breastfeeding to be healthier, easier and more convenient breastfeed longer than those who perceive that breastfeeding is restrictive, inconvenient and uncomfortable. This study focused on the breastfeeding practices of postnatal mothers with regard to exclusivity, frequency and duration. It was a cross-sectional research design covering the three levels of health care institutions in the South-East Zone of Nigeria. Convenient sampling method was used to select 299 postnatal mothers who visited infant welfare clinics along with their infants. Three research questions and one null hypothesis guided the study. The instruments used for data collection were questionnaire on patterns of breastfeeding by postnatal mothers (QPBF) and checklist on health status of infants with varied breastfeeding patterns (CHSIVBP). Frequency distribution and percentages were used to answer the research questions while chi-square test was used in testing the null hypothesis at 0.05 level of significance. The result showed that most of the postnatal mothers practiced EBF for a short period, majority breastfed their infants on demand day and night, and majority also reported that their infants suckle the breast for more than 20minutes. Also breastfeeding patterns of the postnatal mothers was found to differ significantly across the three levels of health care institutions. Childbearing mothers need to be motivated on the need to practice EBF for six month postpartum.

In conclusion, it seems feasible and safe to start total enteral feeding in stable VLBW infants born with birth weight greater than 1000–1200g. This approach can reduce the need of intravenous fluid or PN and its adverse consequences as well as decrease the length of hospital stay for these infants. A larger randomized trial is needed to ensure ETEF is not associated with increased risk of NEC in stable preterm infants with birth weight greater than 1000g and gestational age greater than 28 weeks. Furthermore, there is a need to explore whether ETEF regimen is feasible in extremely low birth weight infants.

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol that is rapidly distributed in the body and brain. Ethanol alters many neurochemical systems and has rewarding and addictive properties. It is the oldest recreational drug and likely contributes to more morbidity, mortality, and public health costs than all illicit drugs combined. The 5th edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) integrates alcohol abuse and alcohol dependence into a single disorder called alcohol use disorder (AUD), with mild, moderate, and severe subclassifications (American Psychiatric Association, 2013). In the DSM-5, all types of substance abuse and dependence have been combined into a single substance use disorder (SUD) on a continuum from mild to severe. A diagnosis of AUD requires that at least two of the 11 DSM-5 behaviors be present within a 12-month period (mild AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria). The four main behavioral effects of AUD are impaired control over drinking, negative social consequences, risky use, and altered physiological effects (tolerance, withdrawal). This chapter presents an overview of the prevalence and harmful consequences of AUD in the U.S., the systemic nature of the disease, neurocircuitry and stages of AUD, comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and pharmacotherapies for AUD.
