It is essential to note that we actually live in a diverse nation and overcoming the shortcomings associated with cultural preferences and beliefs is a big obstacle for healthcare professionals today. In the article “Health Disparity and Cultural Conflict” cultural diversity is singled out to be more than one race. It is important for healthcare workers to know that addressing cultural diversity is far more than just understanding the values, practices, customs, and beliefs (Hughes, 2011). Apart from national origin and racial classification, there are various kinds of cultural diversity and these include language, age, religious affiliation, and geographical location.
Healthcare providers are expected to diligently accommodate the varying needs of the individuals they meet in their service provision. The needs actually range from the different cultural backgrounds to the learning styles and preferences as well as the mixed opinions that define their health and well-being (Lochman, 2012). The issue of language barrier is a big problem to many healthcare providers.
Nurses are normally exposed to various beliefs and cultures as they work in their healthcare fields. Apart from diversity, it is important

The early treatment process however needs colon resection. The treatment process is discussed with the patient and risk also explained, particularly blood loss. In the process of surgery, the patient loses a lot of blood and the level of haemoglobin reduces below the normal range. This required immediate blood transfusion. However, the culture of the patient did not accept blood transfusion. The transfusion process had to be delayed since the patient refused. Such kind of cultural affiliations can affect the manner which care delivery is done and providers must be aware of them and teach the patients. Teaching should actually be a multidisciplinary team work which includes social work, dietician, and therapy department. Challenges of practicing in a multicultural city
There are so many challenges of practicing in a multicultural city. Communication barrier is one of the challenges. When the practitioner and the patient cannot communicate efficiently, therefore the nurse will not be able to deliver effectively (Murcia & Lopez, 2016). Secondly, a nurse may experience culture shock in a multicultural city. This happens when nurses are confronted with very new cultural beliefs and values and then they try to compare with their own beliefs and everything looks so new. Thirdly, another challenge could be rejection by the city residents. When there is a new nurse with different cultural beliefs and values, chances are the residents might reject the nurse’s services and prefer to be served by only home nurses. This is because people feel comfortable receiving care from people who understand their cultural beliefs and values.

Patients who have complex health needs require not only medical. But also social services and support from a variety of caregivers and providers. Facility managers who are part of care coordination could assist patient in receiving optimal care by addressing the challenges in coordinating care for these patients, and offer programmatic changes and policies that help deliver the best services to all patients.
Facility managers can come up with strategic plans based on prior data and make necessary changes based on preexisting conditions. “Patient- centered, comprehensive, coordinated, and accessible care that continuously improved through a systems-based approach to quality and safety” (AHRQ, 2012) are what’s needed to achieve the highest quality care possible in any health care facility.
Patient centered care can’t be achieved with providers only. It requires team work and collaboration among all stakeholders. To improve the quality and safety of patients, health care facility managers can work hand and hand with the coordinated team to provide a system based approach by drawing on decision-support tools, taking into account patient experience, and using population health management approach. Patient preference and needs on what aspects of care to be improved.

As a facility manager, and part of the care coordination team, I would look into models of care that would assist our situation. With the Affordable Care Act in place, there are accountable care organizations (ACOs), which provide models of care (“Promise,” 2013). There are many different definitions and perspectives on care coordination, but all lead to the goal of meeting patient needs and providing adequate healthcare (“Care,” 2014).
Care coordination is essential because each patient can interact with a variety of professionals each visit. For example, for a routine physical appointment, the patient could meet with the scheduling staff, medical assistants, nurses, doctors, pharmacists, and the billing staff. If each one of these member fails to coordinate as a whole, the patient could be harmed or neglected. As a care coordinator, I would be responsible for discussing an individualized care plan with each patient and ensuring that they understand their responsibilities. All barriers should be identified, such as financial, social (language), psychological, and anything that would effect the patient from following their correct plan of care and interacting with the staff (“Promise,” 2013). Another key point is to ensure the medical staff has reviewed the patient’s medical records and ensure that everyone is on the same page. These are just a few examples, because each case is different and each patient will have different needs. .

For out-patients, I will educate them during their waiting time. As a nurse I will take every opportunity I will come across during the patients’ visit to the hospital and throughout their admission in the hospital to educate them about their health care. I will provide patients with instructions to follow on self-care and how to maintain certain problems. Some of the self-care instructions include;
· How to follow the steps of self-care
· How to know early signs of certain illnesses
· How to go about emergency problems
· Who to contact in case of problems
The intended audience
I will educate people of all populations in my education program regardless of their age, culture, illness, ethnicity, and gender. General education will be provided to all patients on how to take care of themselves when they leave the hospital. This important because sometimes patients go home, neglect themselves, resume their unhealthy practices, and forget to manage their medical conditions. For patients suffering from diabetes, I will educate and provide them with instructions on how to inject themselves with insulin. For new mothers, they will learn how to take care of their new born babies and how to bath the infants. Community/public health nursing is nursing care that is population-focused and occurs in non-hospital settings. 1. Theories and Models
This project will be guided by both occupational based model Person-Environment- Person-Participation known as PEOP and other related knowledge theory bases related to immigration studies. All of these theories are explained as following. The detailed explanations about these theories are eliminated and the main features of them that are align with this project are illustrated.
1.1. Person-Environment-Occupation-Performance (PEOP) Model
The Person-Environment-Occupation-Performance (PEOP) Model was generated in 1985 and published in 1991. Development of this model was a response to need for more occupational based models over the paradigm shift. (Christiansen & Baum, 1991, 1997; Christiansen et al., 2005). It is envisioned that although there are some similarities with other models, yet different from other models in terms of its focus on occupational performance and participation, as well as using a top-down approach (Christiansen et al., 2005). The PEOP model is defined as a system model that looks at the function in the systems as a whole and considers the interaction over its components. Occupational performance has been received close attention in the PEOP model and contains three main components: (1) characteristics of the person (including physiological, psychological, motor, sensory/perceptual, cognitive, or spiritual), (2) characteristics of the environment (including cultural, social support, social determinants, and social capital, physical and natural environments, health education and public policy, assistive technology), and (3) characteristics of the activity, task, or role. The most recent edition is developed to provide therapists to identify the client’s enablers and barriers to occupational performance, and might be employed both individual and organizational/community based (Christiansen et al., 2005). PEOP pays close attention to the importance of occupational competency in order to attain occupational participation. Occupational participation in PEOP is wider compared to occupational performance as it embeds the ability to engage in preferred lifestyle choices to participate .

The Agency for Healthcare Research and Quality (2015) suggests the burden of these tasks significantly impact how patients manage their chronic conditions, and that patients often find it difficult to complete all these tasks in order to manage their condition effectively. Personally, I see examples of this every day at the hospital: Patients are not confident in their self-management ability and are therefore unable to demonstrate skill or awareness in regards to their condition. Despite receiving adequate medical attention from outstanding multidisciplinary teams, patients continue to show little interest in self-management, ultimately resulting in an overwhelming number of older adults who lack the confidence to manage one or more chronic conditions (Bodenheimer, 2005). Healthcare providers are being forced to seek new and innovative ways to connect with patients and reinforce educational material in order to give patients the confidence and skill to manage their care. I believe this objective to be most important because self-management is clinically proven to result in better outcomes. It is proven that support for patients and caregivers improve confidence in managing conditions. Recently, my hospital has added to its emphasis on education and follow up... Simply providing information to patients is not enough to build confidence, skill, nor the knowledge to manage their health. 1. Theories and Models
This project will be guided by both occupational based model Person-Environment- Person-Participation known as PEOP and other related knowledge theory bases related to immigration studies. All of these theories are explained as following. The detailed explanations about these theories are eliminated and the main features of them that are align with this project are illustrated.
1.1. Person-Environment-Occupation-Performance (PEOP) Model
The Person-Environment-Occupation-Performance (PEOP) Model was generated in 1985 and published in 1991. Development of this model was a response to need for more occupational based models over the paradigm shift. (Christiansen & Baum, 1991, 1997; Christiansen et al., 2005). It is envisioned that although there are some similarities with other models, yet different from other models in terms of its focus on occupational performance and participation, as well as using a top-down approach (Christiansen et al., 2005). The PEOP model is defined as a system model that looks at the function in the systems as a whole and considers the interaction over its components. Occupational performance has been received close attention in the PEOP model and contains three main components: (1) characteristics of the person (including physiological, psychological, motor, sensory/perceptual, cognitive, or spiritual), (2) characteristics of the environment (including cultural, social support, social determinants, and social capital, physical and natural environments, health education and public policy, assistive technology), and (3) characteristics of the activity, task, or role. The most recent edition is developed to provide therapists to identify the client’s enablers and barriers to occupational performance, and might be employed both individual and organizational/community based (Christiansen et al., 2005). PEOP pays close attention to the importance of occupational competency in order to attain occupational participation. Occupational participation in PEOP is wider compared to occupational performance as it embeds the ability to engage in preferred lifestyle choices to participate .

The Agency for Healthcare Research and Quality (2015) suggests the burden of these tasks significantly impact how patients manage their chronic conditions, and that patients often find it difficult to complete all these tasks in order to manage their condition effectively. Personally, I see examples of this every day at the hospital: Patients are not confident in their self-management ability and are therefore unable to demonstrate skill or awareness in regards to their condition. Despite receiving adequate medical attention from outstanding multidisciplinary teams, patients continue to show little interest in self-management, ultimately resulting in an overwhelming number of older adults who lack the confidence to manage one or more chronic conditions (Bodenheimer, 2005). Healthcare providers are being forced to seek new and innovative ways to connect with patients and reinforce educational material in order to give patients the confidence and skill to manage their care. I believe this objective to be most important because self-management is clinically proven to result in better outcomes. It is proven that support for patients and caregivers improve confidence in managing conditions. Recently, my hospital has added to its emphasis on education and follow up... Simply providing information to patients is not enough to build confidence, skill, nor the knowledge to manage their health. professionals today. In the article “Health Disparity and
Cultural Conflict” cultural diversity is singled out to be more
than one race. It is important for healthcare workers to know
that addressing cultural diversity is far more than just
understanding the values, practices, customs, and beliefs
(Hughes, 2011). Apart from national origin and racial
classification, there are various kinds of cultural diversity and
these include language, age, religious affiliation, and
geographical location.
Healthcare providers are expected to diligently accommodate
the varying needs of the individuals they meet in their service
provision. The needs actually range from the different cultural
backgrounds to the learning styles and preferences as well as
the mixed opinions that define their health and well-being
(Lochman, 2012). The issue of language barrier is a big problem
to many healthcare providers.
Nurses are normally exposed to various beliefs and cultures as
they work in their healthcare fields. Apart from diversity, it is
important
to consider the level of education plus the style of learning.
This will enable the nurse to formulate the best teaching method
relevant for the patient. Patients with various cultural
differences can sometimes not be willing to take part in
education or take in the teachings. In order to initiate any
2. formal kind of teaching, trust must be developed (Campinha,
2003). Nurses should assess the values and ideas related to the
illness and health of the patients, treatment provided,
medication being taken, and the expectations to ensure effective
teaching or learning.
Healthcare professional should pay very close attention to the
cultural needs of their patients including expression and
language. For example, a Chinese patient who has been
disregarding signs unusual signs and symptoms of rectal
bleeding for a long time. He has come to the hospital and
diagnosed with colon cancer. The early treatment process
however needs colon resection. The treatment process is
discussed with the patient and risk also explained, particularly
blood loss. In the process of surgery, the patient loses a lot of
blood and the level of haemoglobin reduces below the normal
range. This required immediate blood transfusion. However, the
culture of the patient did not accept blood transfusion. The
transfusion process had to be delayed since the patient refused.
Such kind of cultural affiliations can affect the manner which
care delivery is done and providers must be aware of them and
teach the patients. Teaching should actually be a
multidisciplinary team work which includes social work,
dietician, and therapy department. Nurses should also be aware
of their own values and beliefs before interacting with the
various different cultures and accommodate them effectively.
Healthcare workers can overcome the shortcomings of cultural
differences by accommodating the needs of every patient
(Lachman, 2012).The issue of language barrier would require an
interpreter service to ensure that the patient has a solid
understanding of the diagnosis, the treatment process, the signs
and symptoms, and education that follow up the treatment
process. Religious beliefs are another big obstacle that
healthcare providers face in addition to cultural preferences.
Healthcare providers should be very responsive in their abilities
to develop a flexible learning environment for the various
diverse encounters in their career.
