Agreement Contract
                           For Those Working In
                       The World Health Survey Project

It is on   / / 2009

An agreement has been reached between the Ministry of Health             First Party

And Mr/Mrs/Miss:…………………………Nationality …………………Second Party



                                  On the Following:-


First: The second party accepts to work for the first party on a temporary basis in
the Ministry with a monthly allowance of KD 200 paid by the special fund for the
World Health Organization Survey. The allowance is paid at the end of each month
- each three months of working as field researcher or other work assigned by the
first party / /2009


Second: Period of the contract …………The contract shall be valid on / /2009
And expires on / / 2009 unless it is terminated before that date for reasons given
by the first party.


Third: This contract maybe renewed upon the agreement of the two parties on a
period and a salary to be agreed upon. The contract in this case shall be deemed
extended for matters agreed upon.


Fourth: The second party shall, in addition to the conditions stated above, bear in
mind the following:
A - Keep the secrecy of the data collected during the health survey, and after that.
B - Good treatment of all the families even in the case of refusing to participate in
the survey.
C - Implement the instructions of the field supervisor and abide by them.
D - Sign an undertaking to absolve The Ministry for liability concerning traffic
accidents and violations that are committed by them or against reported by the
authorities concerned.
E- The second party will complete, at least 20 questionnaires, per month. The first
party will deduct from the second party the amount equivalent to the number of
incomplete forms.

Fifth: The second party shall be paid a transport allowance of KD 50 and mobile
phone card KD 5 during the field work.


Sixth: If the second party breaches his/her work duties, the first party may
terminate the contract without prior notice.


Seventh: The second party has no right to claim any allowance when the contract is
terminated for any reason except for the allowance agreed upon in the first article.


Eighth: The first party may terminate this contract after notifying the second party
with prior notice within a period not less than 15 days.


Ninth: This contract is written in three copies one of them is given the second party.

Fourth: The second party shall, in addition to the conditions stated above, bear in mind the following: A - Keep the secrecy of the data collected during the health survey, and after that. B - Good treatment of all the families even in the case of refusing to participate in the survey. C - Implement the instructions of the field supervisor and abide by them. D - Sign an undertaking to absolve The Ministry for liability concerning traffic accidents and violations that are committed by them or against reported by the authorities concerned. E- The second party will complete, at least 20 questionnaires, per month. The first party will deduct from the second party the amount equivalent to the number of incomplete forms. 1
  • 2. Fifth: The second party shall be paid a transport allowance of KD 50 and mobile phone card KD 5 during the field work. Sixth: If the second party breaches his/her work duties, the first party may terminate the contract without prior notice. Seventh: The second party has no right to claim any allowance when the contract is terminated for any reason except for the allowance agreed upon in the first article. Eighth: The first party may terminate this contract after notifying the second party with prior notice within a period not less than 15 days.
