- The aim of this programme is to enable more older people, particularly those with dementia, to live and to die with dignity at home (as most would prefer) or in residential care settings
- Slides provide an overview of the seaprate projects developing within the Changing Minds Programme and much of the work will really built and enhance the overall effectiveness of the programme Clarify that this Workstream aims to develop tols and resources for healthcare staff in all healthcare setting who aprovide end of life care for people with dementia.
- History - development grants were open to all people but ofr past 2 years half the amount was specially allocated to promote DPC Criteria – All discsiples and sectors but need to have a link with DPC Last 2 years 6 grants have been awarded.
