• Self directed home care available to Medicaid
consumers
• Eligibility determined by the local department of social
services (LDSS)
• LDSS determines level of assistance
• Open to anybody
3. WHO IS ELIGIBLE
• Must receive Medicaid in
New York State and live in
Cortland County
• Must medically require
assistance in home care
functions
• Self directing
• Be willing to instruct service
providers

6. CONSUMER RESPONSIBILITIES
• Interview, hire & terminate
• Train their aides
• Manage supervision of their aides
• Follow the care plan
• Verify time sheets
• Set schedules
