Imagine if writing a progress note was really as simple as the note featured on the title slide. This physician progress note style filled the charts of a 120 bed nursing facility in rural Texas. Walking down the facility hallways I observed many unstable conditions and behaviors. Staff were numb to the yelling. When I questioned about the yelling "we don't medicate behaviors" was the most popular response. I needed to be medicated after a few hours of being there. Collecting data had never been this difficult. Stable and clinical documentation now had a whole new meaning.
This month I am instructing Nurse Practitioner students on Medicare and Long-Term Care Coding and Documentation. Revising this lecture reminds me of all of the clinician types I have met during my long-term care travels.
1. Amazing Clinicians
◾ They know state and federal regulations and practice perfectly.
◾ They don't cross your path very often.
◾ When you meet them you must stop, listen and absorb all their knowledge.
2. So-So Clinicians
◾ They don't know that there are regulations on state and federal levels.
◾ They are pretty common and are doing enough with meaningful practices.
◾ When you meet them you must stop, talk and they absorb your knowledge.
3. Corner Cutters
◾ They are not doing enough and practice out of compliance.

In the mid 1970s, a nursing educator in Idaho had contact, through a student, with a female client who had chronic myelogenous leukemia. This form of leukemia can often be managed for years with little or no chemotherapy. The woman had done well for about twelve years and ascribed her good condition to health foods and a strict nutritional regime. However, her condition had turned worse several weeks before and her physician had advised her that she needed chemotherapy if she were to have any chance at survival. The physician had also advised her of the potential side effects of the therapy including hair loss, nausea, fever, and immune system suppression.
The woman consented to the therapy and signed the appropriate forms, but later, she began to have second thoughts. The nursing educator and student had given the patient one dose of the therapy when the woman began to cry and express her reservations about the therapy. She questioned the nurse about alternative treatments to the use of chemotherapy. The patient related that she had accepted the therapy because her son had advised her that this was the best treatment. She related that she had not asked about alternate forms of treatment as the physician had indicated that chemotherapy was the only treatment indicated. The nurse did not discuss the patient's concerns with the physician, and later that evening, she talked to the patient about alternate therapies. In the discussion, rather nontraditional and controversial therapies were covered including reflexology and the use of laetrile. During the talk, the nurse made it very clear that the treatments under discussion were not sanctioned by the medical community.
The patient's feelings toward alternate therapies were strengthened by the evening's conversation; however, she continued with chemotherapy. The treatments, however, did not bring remission to her crisis and she died two weeks later. Upon hearing about the conversation between the off duty nurse educator and his patient, the physician brought charges against the nurse for unprofessional conduct and interfering with the patient-physician relationship. (In re Tuma, 1977).

I conducted a health assessment on a 70 year’s old male patient from California who was
suffering from hypertension. The patient is currently taking diuretics medication which cause
increase urination. The medication also causes his sodium and fluids levels to drop thus lowering
patient’s blood pressure so it’s imperative to balance and monitor vital sign. The patient stated
that he was moderately satisfied with the care received in the facility. The patient is a smoker and
stated his concern of experiencing vision issues and heart failure, which prompted immediate
care for this patient.
Before being admitted to the hospital, patient stated that he was experiencing chest pain
which was not ending nor was it increasing in exertion. He did not experience nausea, vomiting,
dizziness or unconsciousness. He took some pain killers with no relief but managed to sleep in
the morning. After beginning his daily activities, the pain increased and was then rushed to the
hospital. He underwent some test that clarified that he was suffering from hypertension. When I
reviewed patient past medical records, it showed that he has condition at early age and minor
surgery. I reviewed the patient's cardiovascular, eyes, nose, throat, respiratory system, and his
immune system. Patient immunization was also put into consideration when I reviewed his past
medical record as this will be helpful planning for his care.
Thirty years ago, the patient developed a diffuse rash after he was injected with penicillin
which he was allergic to, but it was not known. Some of his past health operations includes:
chest pains in 1990 and minor surgery on his left arm. He has a medical case of pneumonia at an
early age, which did not take long to heal. The patient was raised by his stepmother since his
mother passed away 72 years ago from heart failure. His father passed on 68 years ago after
committing suicide in his house. His wife died 10 years ago after struggling with tuberculosis for
