Diagnosis of prolapse at the earliest will help to reduce the complications of prolapse as well as continue child bearing function of the young woman.
Introduction: Preeclampsia is a medical emergency implicated in maternal and foetal morbidity and mortality. Research directed at finding the aetiologies of pre-eclampsia is inconclusive because most studies are either cross-sectional or semi -longitudinal in design with very limited potentials to determine precisely the age of pregnancy to suspect the development of preeclampsia with an absolute biomarker.

An ectopic pregnancy is a pregnancy that develops outside a woman's uterus. This happens when the fertilized egg from the ovary does not reach or implant itself normally in the uterus. Instead, the egg develops somewhere else in the abdomen. The products of this conception are abnormal and cannot develop into fetuses. Urine pregnancy test is often done by women itself once amenorrhea is present about 2 weeks of expected menstrual period, however, pregnancy blood test such Beta – human Chorionic Gonadotropin (BhCG) and ultrasound examination are the best tool of diagnosis. The most common place that ectopic pregnancy occurs is in one of the fallopian tubes, a so-called tubal pregnancy. These are the tubes that transport the egg from the ovary to the uterus. Ectopic pregnancies also can be found on the outside of the uterus, on the ovaries, or attached to the bowel. Most serious complication of an ectopic pregnancy is intra-abdominal hemorrhage. In the case of a tubal pregnancy, for example, as the products of conception continue to grow in the fallopian tube, the tube expands and eventually ruptures. This can be very dangerous because a large artery runs on the outside of each Fallopian tube. If the artery ruptures, the woman can bleed severely. Ectopic pregnancy is usually found in the first 5-10 weeks of pregnancy and is the leading cause of pregnancy-related deaths in the first trimester of pregnancy in the USA. In Albania, we face difficulties not only in application of high technology of ultrasound machine in public health sector but unfortunately we are unable to perform BhCG in public health sector laboratories, such making not only challenge and even delay but an expensive process of diagnosis of this medical problem. In conclusion, since ectopic pregnancy is an abnormal pregnancy, and comes with high risk of serious complication, early diagnosis of pregnancy location and its management is crucial in preventing medical complication.

Neonatal and Obstetric Risk Assessment (NORA) Pregnancy Cohort Study in Singa...Premier Publishers
The Neonatal and Obstetric Risk Assessment (NORA) pregnancy cohort study was set up to assess clinical, biochemical and biophysical markers for risk assessment and prediction of the outcomes early in pregnancy. A total of 3271 patients who were in KK Women’s and Children’s Hospital between September 2010 and October 2014 were screened and 1013 patients consented to participate in the study. Women were followed at 18 to 22 weeks, 28 to 32 weeks and 34 weeks and above, till their postnatal discharge from the hospital. Finally, 926 patients remained for studying the outcome. In NORA study, we established locally derived and gestational age-specific reference intervals for the five thyroid hormone parameters. Higher serum progesterone levels at 28–32 weeks of pregnancy were observed in women who had preterm deliveries compared with women with term deliveries in the cohort. We also found that extracellular vesicle (EV) biomarkers enhanced the predictive robustness of an existing pre-eclampsia (PE) biomarker sufficiently to justify PE screening in a low-risk general obstetric population. We plan to further conduct a range of serial assessments from the biosamples which will provide a comprehensive and valuable information of the dynamics of maternal conditions and fetal development during pregnancy.

Introduction: Though there are many studies on the effects of anesthesia methods used for cesarean section on the newborn,
research on this topic still continues. In our prospective observational study, we investigated the effects of different anesthesia techniques used in routine cesarean deliveries on early neonatal outcomes in our hospital. This prospective, observational, randomized study included a total of 222 ASA II risk group pregnant women undergoing elective cesarean section at term (38-41 weeks’ gestation) without fetal distress. The women were randomized into three groups. In the general anesthesia with propofol group (Group P, n = 74), anesthesia was induced with 2 mg∙kg-1 propofol and 0.6-0.9 mg∙kg-1
rocuronium. In the general anesthesia with thiopental sodium group (Group T, n = 74), anesthesia was induced with 5 mg∙kg-1 thiopental sodium and 0.6-0.9 mg∙kg-1 rocuronium. Women in the spinal anesthesia group (Group SA, n = 74) were administered 0.5% (10 mg) hypertonic bupivacaine and 10 mcg fentanyl.

Cardiovascular Events in Overweight and Obese Patients
With Cardiovascular Risk Factors
A Randomized Clinical Trial

IMPORTANCE Few cardiovascular outcomes trials have been conducted for obesity
treatments. Withdrawal of 2 marketed drugs has resulted in controversy about the
cardiovascular safety of obesity agents.
OBJECTIVE To determine whether the combination of naltrexone and bupropion increases
major adverse cardiovascular events (MACE, defined as cardiovascular death, nonfatal stroke,
or nonfatal myocardial infarction) compared with placebo in overweight and obese patients.
DESIGN, SETTING, AND PARTICIPANTS Randomized, multicenter, placebo-controlled,
double-blind noninferiority trial enrolling 8910 overweight or obese patients at increased
cardiovascular risk from June 13, 2012, to January 21, 2013, at 266 US centers. After public
release of confidential interim data by the sponsor, the academic leadership of the study
recommended termination of the trial and the sponsor agreed.
INTERVENTIONS An Internet-based weight management program was provided to all
participants. Participants were randomized to receive placebo (n=4454) or naltrexone,
32 mg/d, and bupropion, 360 mg/d (n=4456).
MAIN OUTCOMES AND MEASURES Time from randomization to first confirmed occurrence of a
MACE. The primary analysis planned to assess a noninferiority hazard ratio (HR) of 1.4 after
378 expected events, with a confidential interim analysis after approximately 87 events (25%
interim analysis) to assess a noninferiority HR of 2.0 for consideration of regulatory approval.
RESULTS Among the 8910 participants randomized, mean age was 61.0 years (SD, 7.3 years),
54.5% were female, 32.1% had a history of cardiovascular disease, and 85.2% had diabetes,
with a median body mass index of 36.6 (interquartile range, 33.1-40.9). For the 25% interim
analysis, MACE occurred in 59 placebo-treated patients (1.3%) and 35 naltrexone-bupropion–
treated patients (0.8%; HR, 0.59; 95% CI, 0.39-0.90). After 50% of planned events, MACE
occurred in 102 patients (2.3%) in the placebo group and 90 patients (2.0%) in the
naltrexone-bupropion group (HR, 0.88; adjusted 99.7% CI, 0.57-1.34). Adverse effects were
more common in the naltrexone-bupropion group, including gastrointestinal events in 14.2%
vs 1.9% (P < .001) and central nervous system symptoms in 5.1% vs 1.2% (P < .001).
CONCLUSIONS AND RELEVANCE Among overweight or obese patients at increased
cardiovascular risk, based on the interim analyses performed after 25% and 50% of planned
events, the upper limit of the 95% CI of the HR for MACE for naltrexone-bupropion
treatment, compared with placebo, did not exceed 2.0. However, because of the

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.
