ZERO HARM | SAFE FROM START
I chose to look the other way
I could have saved a life that day,
But I chose to look the other way.
It wasn't that I didn't care,
I had the time, and I was there.
But I didn't want to seem a fool,
Or argue over a safety rule.
I could have saved a life that day,
But I chose to look the other way.
Now every time I see his wife,
I'll know I should have saved his life.
That guilt is something I must bear,
But it isn't something you need to share.
I knew he'd done the job before,
If I called it wrong, he might get sore.
The chances didn't seem that bad,
I've done the same, he knew I had.
So I shook my head and walked on by,
He knew the risks as well as I.
He took a chance, I closed an eye,
And with that act I let him die.
If you see a risk that others take,
That puts their health or life at stake,
The question asked, or the thing you say,
Could help them live another day.
If you see a risk and walk away,
Then hope you never have to say,
I could have saved a life that day,
But I chose to look the other way.

Lock Out Tag Out
17
LOTO is the safety procedure & standard used in industry to ensure
that dangerous machines are properly shut off & not able to start
up again prior to the completion of maintenance or repair work.
Step 1: Preparation
Step 2: Shut Down
Step 3: Isolation
Step 4: Lockout/Tagout
Step 5: Stored Energy Check
Step 6: Isolation Verification
A lockout/tag out program will help to prevent contact with a
hazard while performing tasks that require the removal, by-passing,
or deactivation of safe guarding devices, unintended release of
hazardous energy (stored energy), start-up or motion of machinery,
equipment, or processes.
One person One Lock - Every person who will work in the machine
must put there individual lock.
Who is allowed to remove a lock out/tag out? - Each lockout or
tagout device must be removed from the energy-isolating device by
the employee who applied the device [OSHA 29 CFR
1910.147(e)(3)].

distance, so that he can’t understand that he is being observed. Let the task complete.
Stage II- Call the person after he finishes the task. Start discussing positive things first,
which he performed correctly as per SOP/COP.
Stage III- Discuss the mistakes/ procedure not followed by him as per SOP/COP in
performance of the job.
Stage IV -The concerned person shall normally resist and will not be willing to accept his
mistakes, as normal human behavior. He will try to defend his procedure. Convince him with
logic and the consequences. This is your managerial competence.
Stage V - Call his co-workers and other people in the vicinity and explain your observation
of the particular task. Discuss the correct processes performed by him and then discuss the
processes not followed by him as per COP & SOP. Make them agree with your observation
