12. Vitiligo
Vitiligo is a common skin condition characterized by loss of pigment cells called melanocytes, resulting in chalky white patches on the skin. It affects around 0.5-2% of the population worldwide and has no known cause, though it may be related to an autoimmune response or genetic factors. The patches of depigmentation are well-defined, often appear on areas exposed to the sun like the face and hands, and may worsen with sun exposure. Treatment options include PUVA therapy which uses psoralen drugs and ultraviolet light to stimulate repigmentation, topical corticosteroids to reduce inflammation, and camouflage creams to hide patches.
Phototherapy
Three out of five children have jaundice to some degree. In approximately 18% of these babies, the condition is severe and requires treatment. Severe jaundice, when left untreated or ineffectively treated, can lead to severe brain damage, a condition called kernicterus, or death. This can be averted simply by reducing the serum bilirubin through phototherapy.

Phototherapy uses natural or artificial light to treat jaundice in newborns by helping the liver process bilirubin. The three main types of newborn jaundice are physiological, pathologic, and breastfeeding jaundice. Phototherapy works by converting bilirubin into water-soluble forms that can be excreted from the body. It is a safe and effective way to lower bilirubin levels and prevent more serious complications from extreme jaundice. Nurses caring for infants under phototherapy must monitor their temperature, feeding, weight, urine output and bilirubin levels closely.
Disorders of pigmentation

INTRODUCTION Phototherapy is a type of medical treatment that involves exposure to fluorescent light bulbs or other sources of light like halogen lights, sunlight, and light emitting diodes (LEDs) to treat certain medical conditions. Phototherapy devices are equipment used in administering phototherapy.
- 3. Bilirubin, a product of the decomposition of red blood cells, is a yellowish substance which in abnormally high concentration in the blood produces a condition, called hyperbilirubinemia, which is responsible for jaundice. Incidence of jaundice in the newborn infant is 60 to 80 per cent, and is even higher in premature infants.
- 4. INDICATIONS FOR USE phototherapy is a choice of treatment In some dermatological conditions such as Vitiligo Psoriasis Common acne and Eczema,
- 5. VITILOGO Vitilogo is a condition in which the skin loses its pigment cells (melanocytes). This can result in discolored patches in different areas of the body, including the skin, hair and mucous membranes fig 1: vitilogo present on the fingers
- 6. ECZEMA Eczema is a kind of dermatitis which can be seen over a large proportion of the skin. This condition is also treated with narrow-band UVB phototherapy
- 7. JAUNDICE Phototherapy has been used in the treatment of neonatal hyperbilirubinaemia. It causes unconjugated bilirubin to be mobilized from the skin by structural isomerization to a water soluble form (lumirubin) that can be excreted in the urine. Phototherapy decreases the level of unconjugated bilirubin in order to prevent acute bilirubin encephalopathy and hearing loss. Lamps emitting light between the wavelengths of 400 - 500 nm are specifically used for administering phototherapy as bilirubin absorbs this wavelength of light. The light is visible blue light and contains no ultraviolet light.
- 8. PHOTOTHERAPY TECHNIQUE Perform hand wash. Place baby naked in cradle or incubator. Fix eye shades & genital area. Keep baby at least 45 cm from lights,if using closer monitor temperature of baby. Start phototherapy. Frequent extra breast feeding every 2 hourly.
