• HEALTH
1. Percentage of pregnant women registered for ANC within the first trimester
Action To Be Taken - Screening for eligible couples for pregnancy test kits to be
distributed by ASHAS, monthly distribution, awareness to be given by Ashas and
MO’s.
2 .Percentage of institutional deliveries against total reported deliveries
Action To Be Taken -Educational Videos, awareness camps Door to door visitation
by ASHAS and training them for proper enrolment in the app. Meeting with
traditional birth assistants for collaborating in spreading awareness along with the
MO, BPM ,Ashas
• 3.Percentage of person screened for Hypertension against targeted population
• Action To Be Taken - House to House screening by Ashas HB (haemoglobin) tests are
to be provided to Ashas for NCD,awareness to be given by MO’s and ASHAS, Training to
be given to ashas for NCD testing kits

4. Percentage of FPOs formed in the block against total sanctioned:
Action to Be Taken: Mapping for crop cultivation of Block is in progress
in order to Choose the right crop to produce for marketing for
formation of FPOs, meeting with theProducer Groups to be held for
discussion on FPOs and it’s benefits for the agricultural economy.
EDUCATION
5. Transitioning rate percentage of both boys and girls from primary to
secondary :
Action To Be Taken:
• Sports will be organized to target the students in participation in
school activities to make their learning atmosphere more lively for
decreasing the dropout rates and improving transitioning rates and
more school activities will be organized to target this .

1- Percentage of operational Anganwadis Centres with
functional toilets.

1. Awareness Programs/Health Camps:
• Organizing awareness programs and health camps at each panchayat level.
Conducting institutional deliveries, pre-delivery checkups, vaccinations, and
providing information on nutrition and departmental schemes. Screening
for diseases like diabetes , hypertension, and anemia will also be conducted.
2. ANC Checkup/Vaccination Camp:
• Organizing ANC (Antenatal Care) checkup and vaccination camps.

• Organizing special awareness camps to raise awareness about tuberculosis.
