• There are many instances for error at shift report 
• Studies have shown that there is much unneeded 
information during shift change and much critical 
information that is missing 
• Time, miscommunication and stress all contribute to errors 
in communication

I. 70% of all sentinel events are the result of poor communication 
II. 50% of nurses feel that the biggest concern during shift report 
is missing information 
III. 42% of nurses feel that distractions cause miscommunications 
during report 
IV. Clearly it is a recognized problem within the nursing population

1. Lack of confidentiality 
2. May take more time for report 
3. Nurses in a routine and don’t want to change 
4. Nurses uncomfortable doing report in front of 
patients

 The Communication process at bed-side meets the needs of both 
patienets & nurse by providing an opportunity to improve patient 
safety 
 The Joint Commission has identified communication failures during 
shift reports as a leading cause of Sentinel events 
 Bedside reporting reassures patients that the nursing staff works as a 
Team & patient gets to witness a safe , professional transfer 
responsibilities
