First i ask myself what does collect bargaining mean? as i was doing my reading for this week and some research i found that it means on page 251 the process whereby workers organize under the representation of a union in order to share a degree of power with management to determine selected aspects of the conditions of employment. Then i ask myself how does it effect me as a upcoming nurse or a nurse in the healthcare field? it effects me in many ways i can use being apart of a union to get my concerns addressing and maybe answered such as pay, hours, better benefits, more sick days and paid vacations. Collective bargaining can help you build that relationship and respect between you and your employer and also contribute to stable and productive labor relations. I ask how it effects the patient, i believe they are the most important part of this process if it wasn't for the patients we wouldn't have a union. it affects the patients in many ways such as getting the adequate time/ care from the physicians , less medical errors. One thing that i read was having a positive work place makes it better for the patient and the staff.

In my personal opinion, these are great tools for the nurses. Having a Union to represent you in a disagreement that your employer may not understand is of great value. When a group of nurses come together to resolve a common issue and have a union representing them for a collecting bargain is an organized way of hospitals diffusing the fight over power. Sometimes when you go to a nursing supervisor or a manager they are too busy with their own agendas that a recommendation by a nurse may be overlooked. Or having nurses worked short staffed may be working for the nursing manager but not for the actual nurses working the floor.
Having a Union has helped the Nursing profession a great deal because nurses are able to strike and do as they see fit in order to have their employers see their passion without being reprimanded or fired for voicing their opinions. Collective bargaining is the organized way of coming to an agreement. It will not really impact the patient because the Union provides a 10 day letter stating when or where the strike will take place so that the hospital can make prior arrangements to have staff for the patients therefore keeping patients safe at all times

Creating a Culture of Evidence-Based Practice
Evidence-based practice (EBP) in its most simplistic form is using the evidence, whether from clinical experiences or patient preferences, to make decisions that affect patient care positively (Polit & Beck, 2017). Evidence-based practice is essential for determining changes in practice that are needed to protect and provide safe care for patients. Nurses are the front-line of the healthcare system and are able to recognize and change policies and procedures. Therefore, nurses are responsible for sharing with their peers and co-workers the information obtained from their evidence-based research.
In order to make evidence-based changes, a dissemination plan needs to be in place. In our facility, our evidence-based practice nurse committee is responsible for teaching the staff on changes in practice. Once they have decided on the changes they present the information to the Emergency Department leadership. From there the changes are reported to the nursing staff through department meetings, bulletin boards, and online learning modules. This is based on the ACE Star Model of Knowledge Transformation which seeks to take research findings and use them to impact patient outcomes by using evidence-based care (Polit & Beck, 2017).
“Often in the dissemination phase, there are considerable barriers that exist. These barriers consist of prejudice toward findings, lack of approval from leadership, nurses attitudes, and the resources needed to make changes. Moore & Tierney (2019) found,
“an overarching theme of disconnection between research and evidence and the participants’ perceptions of contemporary nursing practice was underpinned by three themes:
1) We should be using it… but we’re not.
2) Employees suggested that research involvement was something left after graduation and no longer part of their day-to-day roles.
3) Research is other people’s business (p. 90).
In another report, it was suggested that evidence-based practice is challenging for nurses because of the pressures of a patient satisfaction culture and time constraints when caring for patients (Henderson & Fletcher, 2015). These barriers can only be overcome if nursing leadership has the courage to address them and help nurses see the positive benefits of evidence-based practice.
A culture of change is vital to making a significant improvement in the lives of patients. At this time nursing researchers are limited by a non-existent research culture leaving them nurses with the responsibility to develop that culture (Berthelsen & Holge-Hazelton, 2018). Creating an awareness of the research that is taking place by their peers removes the barriers of feeling not competent to participate. As nursing leadership, our role is to build a culture that creates curiosity and critical reflection ab.

Research Questions
PICOT Statement
The paper will put into focus the aspect associated with the shortage of nursing staff in the healthcare system of the United States of America. It is evident that the shortage of nursing professionals in the country has resulted in an escalation of LTAC that is Long Term Acute Care patients who are of forty years and above. It is evident that the shortage has resulted in such patients staying in the health facilities for a prolonged period of time of about five months. Staying in a health facility for that prolonged period of time has not had a positive effect on the health of the patients and instead, it has resulted in their health decrementing to low levels

Population Problem
The shortage of nurses in the USA has had drastic effects on the healthcare system. Additionally, the ratio of nursing professionals to that of the patients is not equivalent as per the set legal, policy and management standards needed to ensure that the quality of healthcare disseminated to the patients is sufficient. The shortage has been contributed by the increase in demand for better healthcare, a decline in the number of experienced nurses’ professionals and also a decline in the number of registered nurses. The high rate of nurses’ turnover has also led to a decline in the number of nurses. Research indicates that in every three nurses one drops out of the profession after working for an estimated period of two years (Burns, & Grove, 2011). Additionally, it is also evident that one out of five nurses drop out of the profession in cases where the nurses work under stressful environments. Forecasts have indicated that the problem may escalate in years to come especially due to the fact that the demand for better healthcare will not be met by the supply of nursing professionals. The problem has affected a number of healthcare facilities that have seen them being closed due to a decline in nature and the quality of the services being offered. This has led to a decline in the ratio of nurses to the patients. The problem has also led to overworking of nurses especially in facilities that focus mainly on chronic illnesses (Burns, & Grove, 2011
).
Intervention
In the case, an evidence-based approach can be used in finding an intervention. The research will put into consideration six hospitals and the data will be collected based on a random sampling of the respondents. The respondents will be chosen from the nurses' population in the highlighted facilities and will be used in trying to come up with solutions for the problem (Marć, Bartosiewicz, Burzyńska, Chmiel, & Januszewicz, 2019
).

First i ask myself what does collect bargaining mean? as i was doing my reading for this week and some research i found that it means on page 251 the process whereby workers organize under the representation of a union in order to share a degree of power with management to determine selected aspects of the conditions of employment. Then i ask myself how does it effect me as a upcoming nurse or a nurse in the healthcare field? it effects me in many ways i can use being apart of a union to get my concerns addressing and maybe answered such as pay, hours, better benefits, more sick days and paid vacations. Collective bargaining can help you build that relationship and respect between you and your employer and also contribute to stable and productive labor relations. I ask how it effects the patient, i believe they are the most important part of this process if it wasn't for the patients we wouldn't have a union. it affects the patients in many ways such as getting the adequate time/ care from the physicians , less medical errors. One thing that i read was having a positive work place makes it better for the patient and the staff.

In my personal opinion, these are great tools for the nurses. Having a Union to represent you in a disagreement that your employer may not understand is of great value. When a group of nurses come together to resolve a common issue and have a union representing them for a collecting bargain is an organized way of hospitals diffusing the fight over power. Sometimes when you go to a nursing supervisor or a manager they are too busy with their own agendas that a recommendation by a nurse may be overlooked. Or having nurses worked short staffed may be working for the nursing manager but not for the actual nurses working the floor.
Having a Union has helped the Nursing profession a great deal because nurses are able to strike and do as they see fit in order to have their employers see their passion without being reprimanded or fired for voicing their opinions. Collective bargaining is the organized way of coming to an agreement. It will not really impact the patient because the Union provides a 10 day letter stating when or where the strike will take place so that the hospital can make prior arrangements to have staff for the patients therefore keeping patients safe at all times

Creating a Culture of Evidence-Based Practice
Evidence-based practice (EBP) in its most simplistic form is using the evidence, whether from clinical experiences or patient preferences, to make decisions that affect patient care positively (Polit & Beck, 2017). Evidence-based practice is essential for determining changes in practice that are needed to protect and provide safe care for patients. Nurses are the front-line of the healthcare system and are able to recognize and change policies and procedures. Therefore, nurses are responsible for sharing with their peers and co-workers the information obtained from their evidence-based research.
In order to make evidence-based changes, a dissemination plan needs to be in place. In our facility, our evidence-based practice nurse committee is responsible for teaching the staff on changes in practice. Once they have decided on the changes they present the information to the Emergency Department leadership. From there the changes are reported to the nursing staff through department meetings, bulletin boards, and online learning modules. This is based on the ACE Star Model of Knowledge Transformation which seeks to take research findings and use them to impact patient outcomes by using evidence-based care (Polit & Beck, 2017).
“Often in the dissemination phase, there are considerable barriers that exist. These barriers consist of prejudice toward findings, lack of approval from leadership, nurses attitudes, and the resources needed to make changes. Moore & Tierney (2019) found,
“an overarching theme of disconnection between research and evidence and the participants’ perceptions of contemporary nursing practice was underpinned by three themes:
1) We should be using it… but we’re not.
2) Employees suggested that research involvement was something left after graduation and no longer part of their day-to-day roles.
3) Research is other people’s business (p. 90).
In another report, it was suggested that evidence-based practice is challenging for nurses because of the pressures of a patient satisfaction culture and time constraints when caring for patients (Henderson & Fletcher, 2015). These barriers can only be overcome if nursing leadership has the courage to address them and help nurses see the positive benefits of evidence-based practice.
A culture of change is vital to making a significant improvement in the lives of patients. At this time nursing researchers are limited by a non-existent research culture leaving them nurses with the responsibility to develop that culture (Berthelsen & Holge-Hazelton, 2018). Creating an awareness of the research that is taking place by their peers removes the barriers of feeling not competent to participate. As nursing leadership, our role is to build a culture that creates curiosity and critical reflection ab.

Research Questions
PICOT Statement
The paper will put into focus the aspect associated with the shortage of nursing staff in the healthcare system of the United States of America. It is evident that the shortage of nursing professionals in the country has resulted in an escalation of LTAC that is Long Term Acute Care patients who are of forty years and above. It is evident that the shortage has resulted in such patients staying in the health facilities for a prolonged period of time of about five months. Staying in a health facility for that prolonged period of time has not had a positive effect on the health of the patients and instead, it has resulted in their health decrementing to low levels
.
Population Problem
The shortage of nurses in the USA has had drastic effects on the healthcare system. Additionally, the ratio of nursing professionals to that of the patients is not equivalent as per the set legal, policy and management standards needed to ensure that the quality of healthcare disseminated to the patients is sufficient. The shortage has been contributed by the increase in demand for better healthcare, a decline in the number of experienced nurses’ professionals and also a decline in the number of registered nurses. The high rate of nurses’ turnover has also led to a decline in the number of nurses. Research indicates that in every three nurses one drops out of the profession after working for an estimated period of two years (Burns, & Grove, 2011). Additionally, it is also evident that one out of five nurses drop out of the profession in cases where the nurses work under stressful environments. Forecasts have indicated that the problem may escalate in years to come especially due to the fact that the demand for better healthcare will not be met by the supply of nursing professionals. The problem has affected a number of healthcare facilities that have seen them being closed due to a decline in nature and the quality of the services being offered. This has led to a decline in the ratio of nurses to the patients. The problem has also led to overworking of nurses especially in facilities that focus mainly on chronic illnesses (Burns, & Grove, 2011
).
Intervention
In the case, an evidence-based approach can be used in finding an intervention. The research will put into consideration six hospitals and the data will be collected based on a random sampling of the respondents. The respondents will be chosen from the nurses' population in the highlighted facilities and will be used in trying to come up with solutions for the problem (Marć, Bartosiewicz, Burzyńska, Chmiel, & Januszewicz, 2019
).
