A value proposition exists at the intersection of two innovations - IM clinical practice and a crowdfunding research platform – at this nexus arises the potential solutions to address the U.S. healthcare Triple AIM: better care, better outcomes, lower costs. Crowdfunding is a “socially mediated phenomenon” based on intrinsic trust individuals place on shared connections and a willingness to fund projects or causes based on passion alone. What gets funded is what strikes a chord with funders. The passage of the 2012 JOBS Act has resulted in a flood of crowdfunding platforms that have raised over $5.1 billion. Crowdfunding is being utilized for medical research. Health philanthropists and impact investors are collaborating in new ways to make a bigger impact and build capacity, however they want to see buy-in before taking on risk. Crowdfunding mechanisms provide an automatic feedback loop for concepts and give indication of risk prior to investor venture. At the other side of this nexus a crippling healthcare system has produced lower outcomes/higher costs fast approaching 20% of GDP. Evidence suggests that IM practices is effective and could ease the pain in our healthcare system. Barriers exist for clinical practice of IM, based on lack of sufficient evidence and funded research. NCCIH’s portion of the total NIH budget remains at less than 1% and PCORI is not focused on comparative studies of conventional interventions with IM in seeking better outcomes. Creating a crowdfunding research platform is an intriguing proposition that could engage mindful investment in research methodology for IM clinical practices. At this nexus – a crowdfunding IM/CAM research platform - all those who are passionate about a healthcare system that promotes health creation and treats the whole person through holistic approaches that optimize healing, could be funders such as philanthropists, impact investors, patients, and practitioners alike.

This study will conduct case studies and a cross-case analysis of six successful health system strengthening interventions supported by USAID. The objectives are to understand how the interventions were implemented, identify factors that facilitated or constrained success, determine common factors across cases, and provide recommendations for future interventions. The study will involve selecting cases, conducting case studies using documents and interviews, analyzing common themes across cases, and developing policy recommendations. A technical advisory group will provide input to ensure the study addresses USAID priorities and evidence needs.

Health promotion can be defined as giving people the knowledge and tools to help gain control and improve their health. Health promotion can be used to notice behavior trends, but can be used as a tool to educate, support and provide tools for communities to live a healthier lifestyle. Education is a very powerful tool and providing outreach programs or tools to populations who don't have access to health information can be very beneficial in ensuring a healthier lifestyle for those patients. Community health and health promotion can work together in providing health promotion to a population or community. Health policies are used in helping a community achieve a health related goal. The CDC has a step by step process on how to ensure a health policy, problem analysis, policy analysis, strategy development, policy enactment, and policy implementation. (CDC Policy Process, 2015) The CDC works together with the World Health Organization by creating policy prevention programs. The CDC also creates a team of healthcare workers when a crisis is going on and disperses them throughout the world to help the situation at hand. For example, when the Ebola epidemic started in 2014 both organizations worked together to find out where the source of the disease was coming from and they also worked together ensuring no additional exposure was obtained in other countries by travel restrictions. They worked together in handling this epidemic outbreak and containing it in a short period of time. The CDC helps to address Millennium Development Goals by allowing research to be done in prevention and treatment of diseases like HIV/AIDs and Malaria. They also partner up with different companies in finding new medications for treatment methods for diseases. The Millennium Development Goal I was given was Global Partnership for Development. Many people in low and middle income countries face high pharmaceutical costs of medications. Therefore many people go without treatment even low priced generic medications may be priced substantially high for low income in developing countries. The purpose of the developing a global partnership for development is to partner up with pharmaceutical companies to provide affordable medications and make them accessible to these developing countries. (WHO, 2018)

Epidemiology and Population Health
Reflect on your nursing practice for a moment. If you could wipe out one illness, what would it be? How would that impact not just an individual patient, but your entire patient population? What would be the long-term benefits of eliminating that one illness?
The eradication of smallpox by 1979 provides an excellent example of this scenario. This eradication came about as a result of global collaborative efforts involving many countries and organizations, as well as the application of epidemiologic methods. In spite of high initial financial costs, it is estimated that millions of dollars continue to be saved around the world each year as a result of the eradication of this disease.
The eradication of smallpox illustrates the rich history of epidemiology and demonstrates the cost/benefits and implications of improving health at the population level. The application of epidemiologic methods and principles to other critical population health issues continues to play an essential role in improving health and health outcomes.
For this Discussion, you will identify a current population health problem, and you will examine how, and if, the problem is being addressed through the application of epidemiologic principles. You will also discuss the cost-effectiveness of dealing with the problem at the population level.
To prepare:
Review the Learning Resources, focusing on the smallpox epidemic of the 1960s and 1970s and how health organizations applied principles of epidemiology to eradicate this disease. (See attached file)
In light of this example, consider the cost effectiveness of addressing smallpox at the population level.
Using the Learning Resources, research a current population health problem (local or global). Select one on which to focus for this Discussion.
Think about how principles of epidemiology are being applied—or could be applied—to address the problem.
What lessons from the use of epidemiology in the eradication of smallpox might be applicable to this selected problem? What are the financial benefits of addressing this issue at the population level as opposed to the individual level?

The main myth that drew my attention is that managed care is that HMOs threaten the quality of care. HMOs equal (bad) for-profit medicine. This research shall report on the validity or invalidity of this myth. From my point of view, the myth is invalid as it will be supported by the points highlighted below:
1) The goal of HMOs is to ensure that there is enough health care is available to society at the most affordable means.
2) HMOs exist as insurance set up for the cultural aspects of the people. This is due to the presence of the domineering medicinal culture in this land.
The topics in this writing shall include:
1) Fact elaboration-this will describe the fact about the myth presented here. It is factual to say that 0.89 of members of HMOs in Massachusetts are non-profit mind oriented. Most of the members here are among the excellent members of the country (Luft, 1980).
2) The impact of the myth to the public. It takes the bold citizens to refute this myth, and probably, the teenagers are in the light of successfully rebutting it. Under this topic, I shall include two subtopics which include:

2b) The effect of the myth on the seniors. Society is made to believe in the worse managed health care.

Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
