A preconceived assumption of certain attributes, characteristics, and behaviors of members of a certain group is referred as a stereotype (Falkner, 2018). People construct stereotypes form direct personal experience or from other people like the media. For example, Asians are good in math or women are bad drivers. The personal feeling or attitude toward a person or group based on the stereotype is referred to as having a bias (Falkner, 2018). Healthcare workers implicit bias by unconsciously displaying attitude that directly affects patient care based on stereotypes (Falkner, 2018). For example, a homeless patient is presumed to be more medication-seeking compared to people with a job. Although healthcare workers may want to think they are not biased, the occurrences very common (Falkner, 2018). Cultural competence is an imperative skill for nurses and other healthcare professionals. Cultural competency allows nurses to be effective in establishing rapport with patients with proper nursing assessments and implementing interventions designed to meet the patient’s needs (Flowers, 2004). There are models that nurses can assess to provide culturally competent care. For example, the Campinha Bacote Cultural Framework Model include: cultural awareness, knowledge, skill, encounters, and desire, the process of cultural awareness (Flower, 2004). Cultural awareness involves an in-depth exploration of one’s own cultural and professional background as learning about another person’s culture does not guarantee unbiased actions. Cultural knowledge involves the process of seeking information of different ethnic groups such as through journal articles or seminars. Cultural skill involves a nurse collecting relevant cultural data and accurately performing a culturally specific physical assessment such as space and communication. Cultural encounter involves the process of nurses directly engaging in cross-cultural interactions with patients from diverse backgrounds. And last, cultural desire involves the motivation of nurses to become culturally aware and seeking cultural encounters (Flowers, 2004). Hawaii is a very culturally diverse state with a rich cultural history of immigrants. Being an immigrant myself, I love learning about other cultures, especially ethnic foods. It’s amazing but the foods are very similar to one another, yet the taste is very different. For example, one of our Filipino resident’s intake was decreasing. She typically consumed less than 25% of all meals. As soon as we changed her diet to what she was familiar with, her intake changed to almost 100% with all meals. .

Community health nurses must be culturally compliant to provide the most adequate and highest level of quality care. Understanding certain feelings and recognizing these is the first step for the nurse to put aside stereotypes and bias. Most of the time, they are learned behaviors prior to nursing. Stereotyping in nursing is a preconceived assumption regarding a certain group of people; this, in turn, leads to various personal feelings built upon that stereotype resulting in being bias. In health care, these feelings can lead to implicit bias feelings we unconsciously display towards patients and can impact patient care (Falkner, 2018). It is challenging for nurses not to be biased against one group or the other due to the fact that medically and scientifically there are certain groups or populations that certain condition/diseases are more prevalent than others, but "jumping the gun" per say could result in false diagnosis or inadequate treatments (Puddifoot, 2019). Community nurses must take into account the scientific and medical data related to each and every individual.
Campinha-Bacote and Munoz (2001) proposed a five-component model for developing cultural competence: Cultural awareness, Cultural knowledge, Cultural skill, Cultural encounters, and Cultural desire. Every healthcare discipline values communication and must know their patients ethnocultural beliefs. Healthcare providers are more effective in caring for patients when they understand ethnocultural diversity. The model is a circle with three rims, the outlying rim represents global society, a second rim representing community, a third rim representing family, and the inner rim representing the person (Purnell, 2005). The interior of the circle is split into 12 parts representing cultural domains and their concepts. The 12 cultural domains construct the framework of the model. The Purnell model was developed for multiple purposes. Cultural sensitivity refers to the awareness as well as the sensitivity to culture and other practices. Cultural sensitivity can encompass examining different cultures and how they should be accurately approached in the health care. It also includes how to communicate according to within the health care setting. Cultural sensitivity is important area because it can impact the way people work in the health care facilities. It also encompasses valuing differences so as harassment and discrimination, either intended or not, do not happen. Culture is a fundamental and complex concept with consist of broad aspects of people in the health care setting (Campinha-Bacote, 2003). It includes the concepts of sexual orientation, gender, faith, age, disability, race, ethnicity, profession and socioeconomic status. Cultural sensitivity stems from this understanding in that; it is must encompass interpersonal skill as well as the knowledge that will allow the providers of health care, appreciate, understand, and work with individuals from different cultures other than theirs. It also consists of acceptance and awareness of the people’s cultural differences, knowledge, self-awareness of the cultures of the patient and adapting to the skills. Many cultural groups, including lesbian and gay people, individuals with disabilities, lower socio-economic groups and ethnic minorities, for example, African-Americans (Campinha-Bacote, 2003). Cultural sensitivity is considered one of the reasons peoples do not access quality health care services because people are not aware as well as understand the effects attached to one’s culture and how it may be perceived by others. An individual's culture should not appear to be dominant in the place of work.

Treatment setting, coping styles, social supports, stigma attached to substance use disorders, even whether an individual seeks help--all are influenced by a client's culture. Culture needs to be understood as a broad concept that refers to a shared set of beliefs, norms, and values among any group of people, whether based on ethnicity or on a shared affiliation and identity.

2. What It Means To Be a Culturally Competent Clinician
It is agreed widely in the health care field that an individual's culture is a critical factor to be considered in treatment. The Surgeon General's report, Mental Health: Culture, Race, and Ethnicity, states, "Substantive data from consumer and family self-reports, ethnic match, and ethnic-specific services outcome studies suggest that tailoring services to the specific needs of these [ethnic] groups will improve utilization and outcomes” (U.S. Department of Health and Human Services 2001, p. 36). The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) (American Psychiatric Association 1994) calls on clinicians to understand how their relationship with the client is affected by cultural differences and sets up a framework for reviewing the effects of culture on each client.
Because verbal communication and the therapeutic alliance are distinguishing features of treatment for both substance use and mental disorders, the issue of culture is significant for treatment in both fields. The therapeutic alliance should be informed by the clinician's understanding of the client's cultural identity, social supports, self-esteem, and reluctance about treatment resulting from social stigma. A common theme in culturally competent care is that the treatment provider--not the person seeking treatment--is responsible for ensuring that treatment is effective for diverse clients.

At the practicum site, Coral Gables Nursing and Rehabilitation, the effect of cultural incompetence in reference to the delivering poor quality care to a culturally diverse patient population is evident. With this in mind, the focus of this literature review is to provide insight on the trends of cultural incompetence, explore theories used to examine cultural incompetence, gaps in the pre-existing literature and solutions to cultural incompetence. This will help to contextual cultural incompetence and find lasting solutions for eradicating cultural incompetence and prioritizing cultural competence.

Cultural incompetence in nursing finds its roots in the nursing education and training. According to Bednarz, Schim, & Doorenbos (2010), as the general population records increased diversity, so do the nursing classroom where the minorities are enrolling in nursing education at a higher rate. This increases the need to focus on diversity in nursing education to nurture cultural competent nursing professionals. However, cultural incompetence among the teaching staff in terms of the inability to counter diversity barriers make it difficult to teach a diverse classroom and impart students with cultural competence. These barriers emerge from values and common attitudes held by nursing education and culture such as avoiding unwanted discrimination and the Golden Rule, which is “do unto others as you would have them do unto you” (para. 9). As Hassouneh (2013) indicate, the effect of such barriers, is “unconscious incompetence” as well intentioned faculties are unable to recognize realities, including the fact that each student is unique and deserves unique treatment, thus generating more barriers towards instilling students with cultural competence. The nursing education and training lacks uniformity in accommodating the needs of diverse students. As Nurses, we should learn the different traditional rules of cultures and their religion as well. Health care workers must have respect along with no judgment of what race or cultures display as their traditions. It also depends on where you live as some cities or towns or more diverse than others. For example, in New York, it is very diverse. There are very large amount of different translators to assist in any language barriers. Cultural competence is an important component of excellence in health care delivery and can contribute to the elimination of racial and ethnic health disparities (Minority Nurse, 2018). We can demonstrate cultural competency in nursing practice through several ways. One of them is using a interpreter if required. Secondly, there are cultural competency certificates they can obtain. In beginning the interview, a consideration to remember is that eye contact is considered rude to Hmong People and that tone of voice and body language are very important; taking too loudly, placing too much emphasis on words, or talking excessively with hands and arm movements can result in noncompliance (Carteret, 2012). As this patient is young and assumed to be mainstream with Western culture, she will likely be understanding and forgiving of eye contact, tone, and body language but interactions with older family members will require care.
Gender of the nurse might play a role in some assessments, it is important to ask if a male nurse has permission to touch the abdomen or auscultate the lungs, heart, or abdomen. Questions pertaining to sex should be private and held with a nurse who is the same gender as the patient, it is of note that questions or examinations regarding sexual health can be misinterpreted as judgment of promiscuity, resulting in refusal, so sex must be addressed with much explanation and rationale without judgment (Carteret, 2012). As infection can be related to sex or sexual contact, this should be addressed with this patient.
The patient’s language preference for the interview is also important. The patient is a young adult and in college, however, her preference might be Hmong, or the language typically spoken at home. Another consideration is, does the patient want anyone else present for her interview/assessment? Hmong People have a family structure that is patriarchal, meaning, the father generally very involved in decision making and can, ultimately have the final say on a topic or treatment; the mother is caregiver and may wish to be present to help take care of the patient. Hmong Elders also play a large role in decision making, with a Grandfather that might want to talk directly with the doctor and make decisions over the wishes of the patient or patient’s father (Carteret, 2012).
Hmong Culture has roots in animism, which is the belief that objects, places, animals, people, etc. all have spirits and bodies that maintain a natural balance (Duffy, J., Harmon, R., Ranard, D.A., Thao, B., & Yang, K. (2004). The fever in this patient could be related to an imbalance in her spirit, an inhabitation by another sprit that is making her ill, disapproval of recent behavior by dead ancestors, or a curse (Carteret, 2012). The family might elect to have a religious healer, or Shaman visit to perform holistic medicine on the patient, some of this medicine might cause burns or pinch marks with coining or skin pinching being common practices for illness (Khuu, Yee, & Zhou, 2017). This chapter presents the Purnell Model for Cultural
Competence, its organizing framework, and the assump-
tions upon which the model is based. In addition, American
cultural values, practices, and beliefs are presented to assist
non–native American health-care providers to understand
American ways. The American references are meant to
describe, not prescribe or predict, behaviors and practices.
Although the authors recognize that Canada and Mexico are
part of North America, American, as used in this chapter,
refers to the dominant middle-class values of citizens of the
mainland United States. Owing to space limitations, this
chapter deals not with the objective culture—arts, literature,
humanities, and so on—but rather with the subjective cul-
ture. Many Americans are not aware of the subjective culture
because they identify differences as individual personality
traits and disregard political and social origins of culture.
Many view culture as something that belongs only to for-
eigners or disadvantaged groups. However, when Americans
travel abroad, their host country inhabitants many times
stereotypically identify them as Americans because of their
values, beliefs, attitudes, behaviors, speech patterns, and
mannerisms. Some feel that Americans are “fun lovers” and
that, for some Americans, violence is a way of life. However,
“the right to bear arms” is guaranteed by the Constitution.
Most likely, the United States is not any more violent than,
or even as violent as, many other societies, but American
media coverage may be better than other countries, thereby
giving the impression that the United States is more violent
than it actually is. Accordingly, these stereotypes are not
always accurate or desirable.
Western academic and health-care organizations
stress structure, systematization, and formalization when
studying complex phenomena such as culture and eth-
nicity. Given the complexity of individuals, the Purnell
Model for Cultural Competence provides a comprehen-
sive, systematic, and concise framework for learning and
understanding culture. The empirical framework of the
model can assist health-care providers, managers, and
administrators in all health disciplines to provide holis-
tic, culturally competent therapeutic interventions;
health promotion and wellness; illness, disease, and
injury prevention; health maintenance and restoration;
and health teaching across educational and practice
settings.

Leininger (2014) defines culture as a set of values, beliefs, and traditions that are led by a specific group of people and handed down from generation to generation. Culture is learned, shared, values transmitted, a way of life, and practices of a particular group that guides day-to-day thinking, decisions, and actions.
Technological advances have transformed our social world into what Marshall McLuhan termed a global village. In other words, we need to think of the entire world when we talk about our social environment. Computers, satellites, and communication technology have brought the world closer together and made cross-cultural encounters an everyday occurrence.
Cultural competence is the ability to provide holistically effective care to patients who come from different cultures. In nursing, there is a requirement to be sensitive to communication, both verbally and non-verbally. It is an evolving process that depends on self-awareness, knowledge, attitude, and development and application of skills. Nurses should never make assumptions about their patient’s beliefs; for example, stating that all Jehovah’s Witnesses will refuse blood transfusions is a generalization.
Being culturally insensitive can lead to stereotyping, discrimination, racism, and prejudice. Some cultures do not call elders by their first names. Nurses should always ask how the patient wants to be addressed. Some cultures would not allow a male nurse to examine a female patient so the team should be aware of this cultural issue. People may have different beliefs about medical care and treatment in the United States, and the nurse needs to offer respect and gain knowledge of the patient’s situation.
In the healthcare systems of today, healthcare providers are diverse. Nurses have much to offer from their own culture. Culture is universal and dynamic. It is taught to other members of the health care team as a matter of sharing. Distinctions between culture, ethnicity, race, and religion may be confusing. Think of it this way, you are a member of the subculture of the student, you are also a member of an ethnic group, (e.g. Portuguese-American from the Azores), a racial group (white, African American), and a religion (Roman Catholic, Jewish), each with its own set of beliefs and values. So, your culture is a blend of all of those characteristics (Treas & Wilkinson, 2014).

It is agreed widely in the health care field that an individual's culture is a critical factor to be considered in treatment. The Surgeon General's report, Mental Health: Culture, Race, and Ethnicity, states, "Substantive data from consumer and family self-reports, ethnic match, and ethnic-specific services outcome studies suggest that tailoring services to the specific needs of these [ethnic] groups will improve utilization and outcomes” (U.S. Department of Health and Human Services 2001, p. 36). The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) (American Psychiatric Association 1994) calls on clinicians to understand how their relationship with the client is affected by cultural differences and sets up a framework for reviewing the effects of culture on each client.
Because verbal communication and the therapeutic alliance are distinguishing features of treatment for both substance use and mental disorders, the issue of culture is significant for treatment in both fields. The therapeutic alliance should be informed by the clinician's understanding of the client's cultural identity, social supports, self-esteem, and reluctance about treatment resulting from social stigma. A common theme in culturally competent care is that the treatment provider--not the person seeking treatment--is responsible for ensuring that treatment is effective for diverse clients.

Language barriers can be an obstacle when it comes to cultural care for a patient. A translator can be used to help understand the patient. Many cultures have traditional rules when it comes to who is superior in their family. For example, some parts of the Islamic culture, the husband is in charge of speaking and decides for the wife who is a patient. The wife cannot be alone in a room with another male nurse. As Nurses, we should learn the different traditional rules of cultures and their religion as well. Health care workers must have respect along with no judgment of what race or cultures display as their traditions. It also depends on where you live as some cities or towns or more diverse than others. For example, in New York, it is very diverse. There are very large amount of different translators to assist in any language barriers. Cultural competence is an important component of excellence in health care delivery and can contribute to the elimination of racial and ethnic health disparities (Minority Nurse, 2018). We can demonstrate cultural competency in nursing practice through several ways. One of them is using a interpreter if required. Secondly, there are cultural competency certificates they can obtain. Thirdly, staff can be trained to improve the competency culturally. Fourthly, we can research information about a culture along with asking the patient and family members. In beginning the interview, a consideration to remember is that eye contact is considered rude to Hmong People and that tone of voice and body language are very important; taking too loudly, placing too much emphasis on words, or talking excessively with hands and arm movements can result in noncompliance (Carteret, 2012). As this patient is young and assumed to be mainstream with Western culture, she will likely be understanding and forgiving of eye contact, tone, and body language but interactions with older family members will require care.
Gender of the nurse might play a role in some assessments, it is important to ask if a male nurse has permission to touch the abdomen or auscultate the lungs, heart, or abdomen. Questions pertaining to sex should be private and held with a nurse who is the same gender as the patient, it is of note that questions or examinations regarding sexual health can be misinterpreted as judgment of promiscuity, resulting in refusal, so sex must be addressed with much explanation and rationale without judgment (Carteret, 2012). As infection can be related to sex or sexual contact, this should be addressed with this patient.
The patient’s language preference for the interview is also important. The patient is a young adult and in college, however, her preference might be Hmong, or the language typically spoken at home. Another consideration is, does the patient want anyone else present for her interview/assessment? Hmong People have a family structure that is patriarchal, meaning, the father generally very involved in decision making and can, ultimately have the final say on a topic or treatment; the mother is caregiver and may wish to be present to help take care of the patient. Hmong Elders also play a large role in decision making, with a Grandfather that might want to talk directly with the doctor and make decisions over the wishes of the patient or patient’s father (Carteret, 2012).
Hmong Culture has roots in animism, which is the belief that objects, places, animals, people, etc. all have spirits and bodies that maintain a natural balance (Duffy, J., Harmon, R., Ranard, D.A., Thao, B., & Yang, K. (2004). The fever in this patient could be related to an imbalance in her spirit, an inhabitation by another sprit that is making her ill, disapproval of recent behavior by dead ancestors, or a curse (Carteret, 2012). The family might elect to have a religious healer, or Shaman visit to perform holistic medicine on the patient, some of this medicine might cause burns or pinch marks with coining or skin pinching being common practices for illness (Khuu, Yee, & Zhou, 2017). An understanding of Western medicine might not be present, the patient or family may ask for dosages of antibiotics for infection or acetaminophen of fever to be increase or decrease based on how they feel; it is important to explain that medications are dosed on scientifi.

LARRY D. PURNELL
This chapter presents the Purnell Model for Cultural
Competence, its organizing framework, and the assump-
tions upon which the model is based. In addition, American
cultural values, practices, and beliefs are presented to assist
non–native American health-care providers to understand
American ways. The American references are meant to
describe, not prescribe or predict, behaviors and practices.
Although the authors recognize that Canada and Mexico are
part of North America, American, as used in this chapter,
refers to the dominant middle-class values of citizens of the
mainland United States. Owing to space limitations, this
chapter deals not with the objective culture—arts, literature,
humanities, and so on—but rather with the subjective cul-
ture. Many Americans are not aware of the subjective culture
because they identify differences as individual personality
traits and disregard political and social origins of culture.
Many view culture as something that belongs only to for-
eigners or disadvantaged groups. However, when Americans
travel abroad, their host country inhabitants many times
stereotypically identify them as Americans because of their
values, beliefs, attitudes, behaviors, speech patterns, and
mannerisms. Some feel that Americans are “fun lovers” and
that, for some Americans, violence is a way of life. However,
“the right to bear arms” is guaranteed by the Constitution.
Most likely, the United States is not any more violent than,
or even as violent as, many other societies, but American
media coverage may be better than other countries, thereby
giving the impression that the United States is more violent
than it actually is. Accordingly, these stereotypes are not
always accurate or desirable.
Western academic and health-care organizations
stress structure, systematization, and formalization when
studying complex phenomena such as culture and eth-
nicity. Given the complexity of individuals, the Purnell
Model for Cultural Competence provides a comprehen-
sive, systematic, and concise framework for learning and
understanding culture. The empirical framework of the
model can assist health-care providers, managers, and
administrators in all health disciplines to provide holis-
tic, culturally competent therapeutic interventions;
health promotion and wellness; illness, disease, and
injury prevention; health maintenance and restoration;
and health teaching across educational and practice
settings.
The purposes of this model are to
1. Provide a framework for all health-care providers
to learn concepts and characteristics of culture.
2. Define circumstances that affect a person’s cul-
tural worldview in the context of historical per-
spectives.
3. Provide a model that links the most central rela-
tionships of culture.
4. Interrelate characteristics of culture to promote
congruence and to facilitate the delivery of con-
sciously sensitive and competent h.

Background:
Leininger (2014) defines culture as a set of values, beliefs, and traditions that are led by a specific group of people and handed down from generation to generation. Culture is learned, shared, values transmitted, a way of life, and practices of a particular group that guides day-to-day thinking, decisions, and actions.
Technological advances have transformed our social world into what Marshall McLuhan termed a global village. In other words, we need to think of the entire world when we talk about our social environment. Computers, satellites, and communication technology have brought the world closer together and made cross-cultural encounters an everyday occurrence.
Cultural competence is the ability to provide holistically effective care to patients who come from different cultures. In nursing, there is a requirement to be sensitive to communication, both verbally and non-verbally. It is an evolving process that depends on self-awareness, knowledge, attitude, and development and application of skills. Nurses should never make assumptions about their patient’s beliefs; for example, stating that all Jehovah’s Witnesses will refuse blood transfusions is a generalization.
Being culturally insensitive can lead to stereotyping, discrimination, racism, and prejudice. Some cultures do not call elders by their first names. Nurses should always ask how the patient wants to be addressed. Some cultures would not allow a male nurse to examine a female patient so the team should be aware of this cultural issue. People may have different beliefs about medical care and treatment in the United States, and the nurse needs to offer respect and gain knowledge of the patient’s situation.
In the healthcare systems of today, healthcare providers are diverse. Nurses have much to offer from their own culture. Culture is universal and dynamic. It is taught to other members of the health care team as a matter of sharing. Distinctions between culture, ethnicity, race, and religion may be confusing. Think of it this way, you are a member of the subculture of the student, you are also a member of an ethnic group, (e.g. Portuguese-American from the Azores), a racial group (white, African American), and a religion (Roman Catholic, Jewish), each with its own set of beliefs and values. So, your culture is a blend of all of those characteristics (Treas & Wilkinson, 2014).

Some people have an extraordinary ability to inspire others and bring forth loyalty. A person who has such a personality is said to have charisma. The German sociologist Max Weber explains in his Theory of Social and Economic Organization: “The term ‘charisma’ applies to a certain quality that causes one to be set apart from ordinary people and to be treated as endowed with superhuman, or at least exceptional, powers or qualities. In this sense, charisma is a gift or power of leadership.”21
In 1976 R. J. House published a theory of charismatic leadership that has received a great deal of attention by researchers. He traces the influence of the charismatic leader to a combination of personal characteristics and types of behavior. The characteristics of charismatic leaders include being dominant, ambitious, and self-confident, as well as having a strong sense of purpose.
Charismatic leaders also demonstrate specific types of behaviors: (1) They are role models for the beliefs and values they want their followers to adopt. For example, Gandhi advocated nonviolence and was a role model of civil disobedience. (2) They demonstrate ability that elicits the respect of followers. Leaders in art, science, religion, business, government, and social service influence followers through their personal competence. (3) They have ideological goals with moral overtones. Martin Luther and Martin Luther King both employed this type of charismatic behavior. (4) They communicate high expectations for their followers and show confidence in their ability to meet those expectations. Military history is replete with examples of charismatic war leaders. (5) Charismatic leaders ignite the motives of their followers to take action. Motives and tasks fall broadly into three areas—power, achievement, and affiliation.22
The psychologist David McClelland describes the nature of charismatic leadership:
We set out to find exactly, by experiment, what kinds of thoughts the members of an audience had when exposed to a charismatic leader. They were apparently strengthened and uplifted by the experience; they felt more powerful, rather than less powerful or submissive. This suggests that the traditional way of explaining the influence of leaders has not been entirely correct. The leader does not cause followers to submit and go along by intimidation and force. In fact, the leader is influential by strengthening and inspiring the audience. The personality of the leader arouses confidence in followers, and the followers feel better able to accomplish whatever goals they share with the leader.23

Though some of the shortfall owes to the severity of
the last downturn, the metro area has consistently
underperformed its Florida peers over the last de-
cade. The labor market is also softer than previously
believed—the Quarterly Census of Employment
and Wages indicates that survey-based estimates
overcounted employment in 2019. Private services
are underperforming, including leisure/hospitality,
but education/healthcare is expanding with vigor.
At 4.5%, the jobless rate is nearing its cycle low,
and labor market tightness has sent hourly earnings
sharply higher, with average pay closer to the Flor-
ida and U.S. averages than at any time since 2013.
Retirees. A large and expanding retiree popu-
lation will help right the ship and secure HOM’s
spot in the top quintile of areas in job growth
through 2023. Residents age 65 or older account
for a well above-average one-quarter of HOM’s
population, as retirees are drawn to the warm cli-
mate, low taxes, and high housing affordability
relative to other senior havens in the state.

A preconceived assumption of certain attributes, characteristics, and behaviors of members of a certain group is referred as a stereotype (Falkner, 2018). People construct stereotypes form direct personal experience or from other people like the media. For example, Asians are good in math or women are bad drivers. The personal feeling or attitude toward a person or group based on the stereotype is referred to as having a bias (Falkner, 2018). Healthcare workers implicit bias by unconsciously displaying attitude that directly affects patient care based on stereotypes (Falkner, 2018). For example, a homeless patient is presumed to be more medication-seeking compared to people with a job. Although healthcare workers may want to think they are not biased, the occurrences very common (Falkner, 2018). Cultural competence is an imperative skill for nurses and other healthcare professionals. Cultural competency allows nurses to be effective in establishing rapport with patients with proper nursing assessments and implementing interventions designed to meet the patient’s needs (Flowers, 2004). There are models that nurses can assess to provide culturally competent care. For example, the Campinha Bacote Cultural Framework Model include: cultural awareness, knowledge, skill, encounters, and desire, the process of cultural awareness (Flower, 2004). Cultural awareness involves an in-depth exploration of one’s own cultural and professional background as learning about another person’s culture does not guarantee unbiased actions. Cultural knowledge
- 2. involves the process of seeking information of different ethnic groups such as through journal articles or seminars. Cultural skill involves a nurse collecting relevant cultural data and accurately performing a culturally specific physical assessment such as space and communication. Cultural encounter involves the process of nurses directly engaging in cross-cultural interactions with patients from diverse backgrounds. And last, cultural desire involves the motivation of nurses to become culturally aware and seeking cultural encounters (Flowers, 2004). Hawaii is a very culturally diverse state with a rich cultural history of immigrants. Being an immigrant myself, I love learning about other cultures, especially ethnic foods. It’s amazing but the foods are very similar to one another, yet the taste is very different. For example, one of our Filipino resident’s intake was decreasing. She typically consumed less than 25% of all meals. As soon as we changed her diet to what she was familiar with, her intake changed to almost 100% with all meals.
