12 Working With Families: The Case of Carol and Joseph Carol is a 23-year-old, heterosexual, Caucasian female and the mother of a 1-year-old baby girl. She is currently unemployed, having previously worked for a house cleaning company. The baby is healthy and developmentally on target, and she and the parents appear to be well bonded with one another. Carol lives in a rented house with her husband, Joseph. Joseph is a 27-year-old, heterosexual, Hispanic male. He was recently arrested at their home for a drug deal, which he asserts was a setup. Both parents were charged with child endangerment because weapons were found in the child’s crib and drugs were found in the home. The parents assert that the child never sleeps in the crib but in their bed. As a result of the parents’ arrest, social services was notified, and the child was temporarily placed in a kinship care arrangement with the maternal grandmother, who resides nearby. As a result of Joseph’s arrest, he was fired from the cleaning company where he worked, and the family is now experiencing financial difficulties. After initial contact was made with the parents, a number of concerns were noted and the family was recommended for additional case management. Carol’s mother indicated that she had concerns about Carol’s drinking habits and stated that Carol’s father and grandfather were alcoholics. She and the father separated when Carol was a baby, and Carol has had only limited contact with him. There appears to be significant tension between the grandmother and Carol and Joseph. I advised Carol that she could not have any unsupervised contact with her child until she completed intensive inpatient substance abuse treatment. I made arrangements for her placement, but after a week, she was discharged for noncompliance with the rules. She was then referred to an intensive outpatient program and began therapy there. Initially her attendance was erratic because she had lost her license as a result of the DUI. Eventually, however, she became engaged in the program and began to address her issues. Dissociative Diane Diane is a thirty-six year old white female. Currently she is divorced with two children and is employed as a daycare worker. During the session the client seemed to be intelligent with excellent personal and spiritual insight into her situation. Her non-verbals indicated some anxiety (crossed legs, face twitches) and depression (teariness). Diane is the older of two children. She was adopted as an infant after her mother gave her because she was the byproduct of a very violent rape. She describes a childhood filled with intense physical, verbal and sexual abuse by several members of her family. Beginning at age two Diane has memories of her paternal Grandfather sexually abusing her and using her in conjunction with satanic rituals. The rituals involved physical abuse as well as threats with a knife, blood and an Italian horn being laid on Diane’s bare chest. To the best of her knowledge this abuse continued until Diane was five. During this time her paternal Grandmother, who was a spiritist, said prayers to protect Diane and would often cursed other family members by putting menstrual blood in the family’s ‘Sunday sauce’. Diane’s uncle simultaneously abused her during this time. His abuse was also sexual in nature, but was also very physically abusive. Specific memories include being thrown naked into a pile of laundry and then molested, being thrown on a bed, being molested while watched by her uncle’s friends and being slapped across the face when not being willing to perform oral sex. Diane’s maternal grandparents did not participate in any abuse, but witnessed both physical and sexual abuse and often told Diane that she had brought the actions on herself. This pattern was duplicated by Diane’s adoptive parents who would vacillate from calling her their ‘little princess’ to choking, punching and sometimes torturing Diane, particularly in her teen years. At the age of twenty-eight Diane married her current husband. The couple was together six years until their divorce five years ago. Matt also sexually abused Diane before their marriage, causing her to hemorrhage and be taken to the hospital, at which time she was also treated for multiple bruises. Matt also came from a background of sexual abuse by his father, uncle and and priest. He was forced to commit several homosexual acts in a church context as well as childhood pornography. Several years later his father was arrested as a pedophile. During their marriage Diane experiences several bouts of verbal, physical and sexual abuse and now experiences intense PTSD symptoms when he comes to pick up the children. He is described as "constantly running around" and presenting with difficulties listening and following instructions. Human Service Practice in the Criminal Justice Setting Scenario Cindy Hart is 23 years of age. She was born on 6/11/86, the fourth child born to Betty Hart (born 6/1/66) and Barney Hart (born 4/11/55). Her father, Barney Hart, was an alcoholic who drank daily and recently passed away from cirrhosis of the liver. Mr. Hart was extremely violent with his wife and his children.
