identification and stratification, risk assessment, care planning, coordination of
services, and care monitoring of complex and chronically ill members. The solution
is designed to support compliance with NCQA’s Model of Care, providing
Interdisciplinary Care teams (ICTs) with rules-based workflows and proactive
automation for optimal care planning. Through guided workflows, the solution
provides the ICT with the tools to efficiently and effectively collaborate and
coordinate the necessary care, services, and community resources to meet the
comprehensive medical, behavioral health and social needs of the SNP member.
At the center of InfoMC’s Model of Care solution is the integrated Care Plan. Using
real-time data, the system generates pre-populated Care Plans based on the individual member’s physical
and behavioral health conditions, psychosocial factors, and social needs. The integrated Model of Care
workflows guide the ICT through the necessary steps to engage the member, establish and empower the care
team, and ensure coordination of quality-driven care and services throughout the care planning process.
InfoMC’s Care Team portal, Incedo CareConnect,
provides online and mobile access for all ICT members
who share responsibilities for SNP members. Incedo
CareConnect promotes real-time ICT communication,
collaboration and coordination of care while
engaging both the member and caregiver throughout
the process. Incedo CareConnect provides the ICT,
including member and caregiver, access to real-time
data, care plans, assessments and measures,
messaging, task management, scheduling, and care
team alerts.
