The Prevalence Of Pressure Ulcers During Home Settings...
Methodology
This DNP project will be a retrospective chart review on the prevalence of pressure ulcers in elderly
in home settings receiving services from a community–based health care provider located in New
Jersey with a total patient population of over 3000 in eight counties. Prevalence will be defined as
the total number of patients who currently have pressure ulcers or have been treated for pressure
ulcers and the risk associated with the development of pressure ulcer for the past 12 months. A
retrospective chart review has been chosen for this project because it is often used to evaluate
associations between models of care and the clinical outcomes of interest (Hess, 2004). It is also an
inexpensive way to research accessible data for any condition where there is latency in available
data and most importantly to generate hypothesis for future studies (Gearing, Mian, Barber &
Ickowicz, 2006). Setting: The reviewer will extract 100 convenience sample charts from the
electronic database of the participating primary practice to determine the prevalence of pressure
ulcer. The practice is a community–based primary health care provider that services the elderly in
their homes. The majorities of these elderly are cognitively impaired, have mobility problems, and
can no longer leave their homes and are in need of medical care. The geographical locations of the
patients are Passaic, Essex, Bergen, Union, Middlesex, Morris, Monmouth, and Ocean counties in
New Jersey.

A Research Study On Autistic Spectrum Disorder
Autistic spectrum disorder, also known as ASD can be defined as a group of complex developmental
disorders characterized by impairment in verbal and nonverbal communication, behavioral
challenges, impaired cognitive abilities, and impaired social interactions. Nurse practitioners
providing primary health care to children should be familiar with ASD for early identification,
evaluation, and management as this is significantly important to the well–being of children and their
families. The purpose of this paper is to raise awareness.............
The history of ASD dates back to the year 1908 when the word autism was first used to describe a
subset of withdrawn schizophrenic patients. The word autism was then used again in 1943 when
American child psychiatrist Leo Kanner, M.D., published a paper called "Autistic Disturbance of
Affective Contact" describing eleven socially isolated children who displayed the same desire for
sameness and aloneness describing their condition as early infantile autism (Autism Speaks, 2013).
A year later German scientist named Hans Asperger reported on boys who were highly intelligent,
but had obsessive interests and trouble with social interactions. He described these boys as having a
milder form of autism, now known as Asperger's syndrome. During the years 1950's to 1960's
autism was classified as schizophrenia in the International Statistical Classification of Diseases and
Related Health Problems and the term "refrigerator mothers" was used to describe

Nursing Terminology System Vs. The International...
Nursing Terminology System Comparison Nursing terminology could be described as the formal
and informal communication that occurs in a nurses daily activity. According to McGonigle &
Mastrian (2009) nursing terminology allows nurses to communicate nursing data, information and
knowledge specific to nursing. In addition, standardized nursing terminology refers to a system.
This means they have undergone approval by a specific authority. The American Nurses Association
(ANA) is one authority with a standardized nursing practice and information infrastructure
committee, which recognizes twelve standardized terminologies, both nursing specific and
interdisciplinary (p. 98). The Clinical Care Classification system and the International ... Show more

The CCC system's major developer is Dr. Virginia K. Saba. Dr. Saba has pioneered the integration
of computer technology and nursing. The ICNP was developed by the International Council of
Nurses (ICN) to cover diagnoses, nursing actions and nursing outcomes as it relates to nursing
phenomenon (p. 99). The CCC website (2012) describes this system of nursing terminology as a
"unique coding structure for assessing, documenting, and classifying patient care by nurses and
other clinical professionals in any health care setting" (www.sabacare.com). The CCC system serves
nursing as well as other health care providers such as physical, occupational and speech therapists
and medical social workers. The ICN website (2015) reports ICNP was created to improve
worldwide healthcare as an integral part of the global information infrastructure to influence health
care practice and policy. This database is intended for use by nurses, who can describe and report in
detail, nursing assessments and interventions, as opposed to being a system for other clinical
healthcare professionals. The ICNP is reported reliable to support care and effective decision
making and useful for nursing education, research and health policy (www.icn.ch/what–we–
do/about–icnpr/). The CCC website (2012) states there are a total of 176 nursing diagnoses and 804
concepts and core interventions to make up the nursing interventions and actions. There are a total
of 21 care components representing functional,

Evaluation Of Counseling As Assessment And Diagnosis
Whiston in section three of her book provided an in–depth analysis of applications and issues of
assessments in counseling as assessment and diagnosis are vital concepts in the counseling process.
It therefore becomes imperative for differing counselors to understand formal diagnostic procedures
and the fact that "diagnosis is the prevalence of individuals with significant psychological
difficulties" (p. 296). Diagnostic systems are used to provide professionals with a general language
governing diagnostic terms to eliminate individuals' use of personal definitions. The most widely
used diagnostic systems within the mental health service are; the Diagnostic and Statistical Manual
of Mental Disorders (DSM–IV– TR–TR) and the International Classification of Diseases (ICD–10–
CM). The use of diagnostic systems is also considered "helpful in conceptualizing cases" (p. 296).
Effective use of a diagnostic system requires a "thorough appraisal of a client's problems, physical
condition and symptoms, and environmental influences" (p. 297). A diagnosis is usually derived
from client information obtained from either informal or formal assessment tools or simply an
amalgamation of both. The DSM–IV–TR diagnostic system is multiaxial and captures the
information needed for treatment planning on five axes labeled axis I, axis II, axis III, axis IV and
axis V. Axis I looks at clinical disorders and other clinical conditions that may be a focus of
attention. Axis II captures personality

Abstract
Nowadays Diabetes Mellitus (DM) is becoming a major health problem in all over the world. If a
person having prolonged group of disease with high level of blood glucose then it is Diabetes
Mellitus. It will cause severe other health issues like cardiovascular diseases, disease related to
vision, Kidney, blood vessels and nerves. There are three types of diabetes. They are Diabetes
Mellitus Type –I(T1DM) , Diabetes Mellitus Type –II(T2DM) and Gestational Diabetes. The people
who is diagnosed by diabetes in Saudi Arabia mostly has Diabetes Mellitus Type –II (T2DM). Saudi
Arabia ranks 7th position in all over the world to have the highest number of people with diabetes in
the year 2000 and 2030. International Diabetes Federations says that there were 3.4 million cases of
diabetes in Saudi Arabia in 2015. A Research says that the prevalence of Diabetes mellitus in Jazan

Research Paper Evaluation of the research articles This work is using two original research papers
and their findings to analyse the validity of the practice of SR (seclusion and restraint) in psychiatric
inpatient facilities. The research papers chosen shed light on this questionable practice by analysing
multiple patient characteristics and the events leading to SR. The other chosen research paper
evaluates the inpatients perception of SR. The first research was found in the 'Journal of the
American Psychiatric Nurses Association' titled 'Characteristics of patients with histories of multiple
seclusion and restraint events during a single psychiatric hospitalisation' and aimed by means of a

For example inpatients have many times vocalised and/or shown by their response to SR, that they
believe this strategy is merely used to exert power over them (Mehan et al. 2004). (Taylor et al.
2012) quoted in their discussion 'The results of the present study suggest that patients' opinions were
not included in treatment planning. This might show that paternalistic decision–making still exists in
psychiatric hospitals.' Both research papers selected allow for transferability since their findings in
mental health facilities located in the USA and Finland are the same as the observations gained in
Tasmania, ranging from age, sex, aggression criteria and inpatient perceptions of SR (Colorado State
University 2011). If the findings can be generalised could be answered with 'yes' and 'maybe', 'yes'
because the application of the research results can be applied beyond those examined, since the
criteria are always the same, people with mental illnesses, SR in mental health facilities and
aggression or violence shown, 'maybe' because similar research studies conducted in countries with
a lower threshold for valuing human life and therefore higher thresholds of fair treatment
expectancy by inpatients would change the research criteria and results (Collis and Hussey 2003, p.

Advantages And Disadvantages Of Health Insurance
Germany is the first to establish the national social health insurance system in the world since 1883
[136]. The health insurance in Germany is mandatory for all the population [137], and as a universal
multi–payer system, it is split in two parts. One is the statutory health insurance (SHI), roughly 85%
of the population who are with a free choice of the comprehensive health insurance plan provided by
each of more than 200 competing SHI companies [138]. Of these, the medical services are
reimbursed by SHI companies, and the framework and contents of quality assurance for these
services is specified centrally by the Federal Joint Committee which is the highest decision–making
body of the joint self–government of physicians, dentists, hospitals and health insurance ... Show

Indeed, German health claims data aims to cost calculation and reimbursement rather than
documentation of disease. That may result in various distortions concerning false–positive and
false–negative diagnoses, and consequently, studies on validity and plausibility are required.
Although an internal validation procedure can be developed to deal with false–positive diagnoses,
false–negative diagnoses were unable to be identified [150]. Moreover, claims data may have
limitations such as missing data on implants and operation techniques, the subjectiveness of patient
evaluation or deficits in documentation [149]. However, SHI data avoid selection and recall bias by
large and provide sound, detailed information on patient–level health care service utilization and
direct medical expenditures in a real world setting. This enables ample cross–sectional and
longitudinal analyses on care structures and expenditures components in many kinds of subgroups.
What is more, it provides data needed to evaluate population–based interventions [151].

WHO Nomenclature Regulations, adopted in 1967, stipulated that Member States use the most
current ICD revision for mortality and morbidity statistics. The ICD has been revised and published

The codes use alphanumeric characters in all positions, not just the first position as in ICD–9. In
addition, there are 68,000 existing codes in the ICD–10, as opposed to the 13,000 in ICD–9.
Therefore, the codes have been greatly expanded and this expansion allows a significant increase in
the specificity of the reporting in the ICD–10, allowing more information to be conveyed in a code.
For instance if you had a burn on your right arm in did not specify which arm had the burn in the old
ICD–9, now in ICD–10 it specifies which arm has the burn. The terminology has been modernized
and has been made consistent throughout the code set. There are codes that are a combination of
diagnoses and symptoms, so that fewer codes need to be reported to fully describe a condition. The
reason they changed it is because the practice of medicine has changed considerably in the last 25
years, and the ICD needed to reflect those change. There have been many new diseases discovered,
new type of medical devices and new treatment and the

Eating Disorders: A Case Study
Eating disorders and associated obesity are major public health issues. In addition to its adverse
health problems, behavior related to overeating may have significant psychological and neurological
underlying conditions (Moreno & Tandon, 2011). Research indicates that there may be an addiction
component associated in the development of eating disorders, as there are similarities between
substance dependence and overeating (Uher & Rutter, 2012). Their shared neurobiology and
treatment, has led to new conversations and suggestions for a new category for food addiction in the
new DSM–V, as well as be included with substance disorders under a broader category for addiction
disorders (Moreno & Tandon, 2011; Uher & Rutter, 2012). The current classification systems of
mental disorders in the Furth Edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM–VI), Fifth Edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM–V)
and the Tenth Edition of the International Classification of Diseases (ICD–10) are limited and

Eloy Quesada As of October 1, 2015, all healthcare providers will be required to stop using ICD–9
and start using ICD–10. Switching over to the more in depth system of ICD–10 will require
adjustments from large healthcare providers, like hospitals, as well as small practices run by doctors
in order to successfully adapt. Some will have to adjust more than others. While hospitals are bigger
and more complex, they will have an easier time adjusting to ICD–10 than small practices, which
are operated by a single physician or a group of physicians.

a standard diagnostic tool. ICD has many uses throughout the health industry. For the most part ...

The reason for the need of all these extra details is because not only does it increase accuracy in
medical records but it also allows doctors to administer better patient care. Using ICD–10 for
reimbursement purposes will also be a lot more efficient and cost–effective. With ICD–9's broad
classification system some procedures, associated with certain codes, would be unnecessary for
specific patients. However, with ICD–10 because the codes are so specific, procedures associated
with those codes will all be necessary for further diagnoses and no procedure will go to waste. As
necessary as ICD–10 is, smaller practices that only have a few physicians on staff will have more of
a difficult time integrating the system than larger organizations. The reason being that larger
healthcare organizations tend to have coders. Coders are individuals who hardly ever have contact
with patients but they are important in the sense that they are the ones that input the ICD codes into
patients' medical records. Without coders, physicians would have to be the ones to input the ICD
codes into the patients' medical record. Luckily physicians at hospitals and other large health
organizations do not need to do this. Unfortunately, physicians at smaller organizations do not have
coders at their disposal and they are the ones that enter the ICD codes.
How does this pose a problem for the physician or

Infantile Autism Research Paper
The word autism comes from the Greek word "autos," meaning "self;" although the term itself has
been used for about 100 years, it was initially used to describe one set of symptoms of
schizophrenia. Eugen Bleuler, a Swiss psychiatrist, was the first person to utilize the term around
1911. In the 1940s, researchers in the United States began to use the term to describe children with
social and emotional troubles. Leo Kanner, an American child psychiatrist, published a paper in
1943 describing 11 children who were greatly intellectual but displayed "a powerful desire for

that autism was caused by mothers not loving their children enough. This theory has since been

medications like LSD, electric shock, and behavioral modification relying on pain and punishment.
By this time, autism had been categorized below schizophrenia in the International Statistical

there is no relation between the two conditions. In 1977, research on twins found that autism is
significantly influenced by genetics and biological differences in brain development. Autism was
officially distinguished from childhood schizophrenia and added to the DSM in 1980, and in 1987, it
was given a more expansive description and a list of diagnostic criteria. A UCLA psychologist, Ivar

Analysis Of The Mental Health Of Ruby Wax
Being able to find a first–person account, which provides explicit details on the mental health of
Ruby Wax has been unequivocally challenging. However, after much research, numerous resources
have been sought, which include her autobiography, How Do You Want Me? (2002), Ted talks and
more recently, her appearance on BBC One Who Do You Think You Are? (2017), which contain
accounts of her mental health issues and her recovery.
How Do You Want Me? (2002) is an autobiographical memoir by Ruby Wax, which details a
brutally honest and touching account of her personal life, including her upbringing, fame, marriage,
children and some details of her breakdown and recovery. Not only is the book an honest account, it
is funny and irreverent.
Ruby Wax is well–known for being a comedian, actress, author and recently a campaigner for

called Edward and her mother was called Berthe. Her parents escaped from Austria in 1938, due to
impending Nazi threat. They set up roots in Evanston, Illinois. Her father set up his own business,
and became a wealthy sausage maker, and her mother was an accountant. Ruby is married to Ed
Bye, who is a British television producer and director. They have three children, a boy and two girls.

In 1937 Charles Bradley used stimulants to treat children who were supposed to be suffering from
this disorder and it was because of this work that in 1956 Ritalin as a drug was introduced to treat
the hyperactivity disorder. In early 1960 the disorder was named as "Minimal Brain Dysfunction"
and still stimulants were used to treat the disorder. In the latter stages of 1960 the disorder became
known as "Hyperkinetic Disorder of Childhood". In 1970s more symptoms were joined with this
disorder which showed how an individual would behave when he had ADHD. It was in 1980s that
the name Adult Deficiency Disorder evolved and this was included in the DSM III given by the
American Psychiatric Association. In 1987 the name was changed to Adult Deficiency Hyperactivity
Disorder and new medications which were not stimulants came into the market in the year of 2003

Depression : The Causes Of Depression As A Medical Depression
Depression is a medical illness that affects negatively the way an individual feels, thinks, and acts.
MDD occurs when a person goes through extreme sadness for an extended period. The symptoms of
MDD vary from mild to severe. A person feels sad and depressed, loses interest in activities, feels
worthless, and starts having suicidal thoughts. MDD affects a person's life as it impacts a person's
mood and behavior as well as sleep and appetite. People with MDD lose interest in activities,
develop poor concentration and loss of energy. These problems become embedded in an individual
and make it difficult for them to cope with everyday responsibilities. MDD is a common mental
disorder in the United States. Some individuals do not seek treatment, but those who seek treatment
get better through medications and psychotherapy. The precise cause of MDD has not been
identified, but several factors increase a person's vulnerability to the condition. These factors include
alcohol and drug abuse, stressful medical diagnosis such as cancer, and medications such as steroids.
Different schools of thought have developed their own theories to answer why people become
depressed. Psychological beliefs provide explanations for people's thinking and why they behave the
way they do. Freud and Aaron Beck developed theories that help explain the causes of depression.
Personality factors, history, and personal relationships are important factors that contribute to
depression. Cognitive theories became prominent to integrate mental events into the behavioral
framework. Cognitive behavioral theorists believe that depression is a result of irrational cognitions.
According to Aaron Beck, dysfunctional beliefs create negative thoughts that cause depressive
symptoms (Disner, Beevers, Haigh & Beck, 2011). He asserts that having more negative thoughts
increases the depression that a person will experience. Beck stated that a person has three
dysfunctional beliefs that predict the probability of occurrence of depression. They include the belief
that a person is defective, the belief that a person's experiences result in failure, and the belief that
the future is hopeless. If these beliefs are present in a person's mind, there is a high

Notwithstanding the common understanding that data mining applies to data analysis problems with
vast amounts of data and is solved by resorting to effective combination of analytical methods, the
term data mining is still unspecific, largely due to its process–based nature. For this reason a large
variety of methods are considered part of data mining, including computer science approaches, such
as multidimensional databases, machine learning, soft computing and data visualization, and
statistical–based methods, including hypothesis testing, clustering, classification, and regression.
Data mining technologies have proven to be useful and effective in different areas, including
marketing, customer relationship management, engineering, and biomedical research [20, 21]. This
research uses the applications of data mining technique on the collected data, particularly the
analysis of diabetes mellitus (DM) data. Health is wealth a saying in Tamil proverb. Jazan lies on the

Caffeine Research Paper
Caffeine is addictive. This is an undisputable fact. The American Psychiatric Association lists
caffeine withdrawal as a formal disorder in its Diagnostic and Statistical Manual of Mental
Disorders 5th ed. under the Section III, conditions for further study. The World Health Organization
names caffeine withdrawal as a withdrawal or dependence inducing psychostimulant in its
International and Statistical Classification of Diseases and Related Health Problems 10th ed., which

positive effect on the user's mood combined with a delayed negative effect. This makes one

The National Institute Of Mental Health
In 2013, the National Institute of Mental Health (NIMH) broadly launched the Research Domain
Criteria project (RDoC). The purpose of RDoC is to provide a working framework to advance
dimensional approaches of classifying psychopathology on the basis of observable behaviors and
neurobiological measures (National Institute of Mental Health, 2013). Reflecting on the last thirty
years, incredible progress has been made in understanding brain–behavior relationships in numerous
mental disorders. The continued interdisciplinary work of geneticists, cognitive and affective
neuroscientists, and behavioral researchers has not only expanded our knowledge of biological
causal and maintenance factors, but has also provided the field with a strong ... Show more content

Within each of these broad domains the project details more specific constructs and subconstructs.
For example, the construct effortful control is nested under the cognitive systems domain. Effortful
control is then further elaborated into subconstructs such as goal selection, updating, and
performance monitoring. Over the next few years, NIMH will be conducting a series of workshops
focused on further refining the definition of the broad domains of functioning and respective
dimensional constructs and subconstructs covered within each domain. Additionally, these workshop
groups are tasked with organizing and disseminating gold standard measures or indicators of
analyses for the previously mentioned areas of study (e.g., genes, neural circuitry, physiology).
Performance on these variables will then be used to classify research participants in future studies.
This approach would replace the current systems designated by the Diagnostic and Statistical
Manual of Mental Disorders (DSM–5; American Psychiatric Association, 2013) and the
International Statistical Classification of Diseases and Related Health Problems (ICD–10; WHO,
1992). In comparing the RDoC initiative to the DSM–5 and ICD–10 diagnostic approaches, RDoC
is noticeably unique in three principal ways. The RDoC project and respective framework are
envisioned as a dimensional system that spans in range from typical to atypical.

INTRODUCTION
Diabetes is one of the most common chronic, debilitating complex and intricate disease found in all
age groups ranging from infants to old age people in all populations. Insulin is a vital and an
essential component of our human body metabolism which is used for its proper growth.
Diabetes may be caused due to inadequate production of insulin or inappropriate utilization of the
produced insulin. In any of the situation, plenty of glucose is available in the blood stream which
human body could not utilize it properly [1], resulting in the diabetes. Broadly, diabetes is
categorized into two categories: Type 1 and Type
2. Due to insulin deficiency, Type 1 diabetes is caused, also known as juvenile diabetes. The only
solution to this type of diabetes is to inject the required amount of insulin as supplement in the
patient's body. Whereas: Type 2 diabetes is more commonly known as Adult–onset diabetes. It is the
most common type of diabetes, which usually develops at the age of 40 and older.
According to Diabetes Atlas, it is estimated that about 194 million people worldwide are suffering
from diabetes and it is estimated that this will increase to 333 million or 6.3% by 2025 [2].
Type 2 diabetes is spreading like an epidemic and constitutes about 85% to 95% of all diabetes in
developed countries and even higher in developing countries [3]. Diabetes is taking shape of a
serious global problem

We also need to have our systems upgraded with the correct software. Staff must be aware that the
old and new ICD will have to co–exist while the system is reengineered. We must review all of our
processes, systems, and reports to find out where icd–9 codes are presently being used. That will
make sure that no one is left out once the upgrade takes effect. The transition to ICD–10 gives us an
opportunity to focus on our processes. This may also help to ease the transition to ICD–10. The
change to the new system will affect many staff members in our clinic. Physicians should find it
easier to diagnose patients since this coding set is more specific than the previous one. For instance
when previously diagnosing asthma you had 2 choices, intrinsic and extrinsic. With this new set of
codes the provider can choose from mild intermittent, mild persistent, and moderate persistent just
to name a few. There is a broader range for the providers to select which will make it easier on them.
Since there will be more documentation, providers will have to ensure they keep up so as not to get
behind. The coders will have to learn 5 times as many sets of codes. They will also have to be re–
certified to use ICD10 (AHIMA 03). We

"In December 2011, a 14–year–old girl in Hagerstown, Maryland, went to the mall with friends,

friends were again at the mall, and she drank another 24–ounce can. She went into cardiac arrest that

underlying heart condition'...'' (S4, P1) Energy Drinks: A Misunderstood Industry provides
information about the energy drink companies and what measures they take to protect the common
people. No laws should be put in place in regards to the amount of caffeine in energy drinks for two
main reasons; one, energy drinks are labeled to protect the public, and two, energy drinks can be

"The energy drink manufacturer has a label that clearly states it does not recommend its product to
'children, pregnant women, or those sensitive to caffeine.'"(S1, P1) Therefore, energy drink
companies do label their products with the correct warnings, but people choose to ignore the dangers
and blindly take a risk instead of reading package. Some may believe that energy drinks don't have
proper warning labels that list the amount and effects of the caffeine that we consume in an energy
drink. "...Some of these energy drinks have more than 500 milligrams of caffeine."(S6, P4)
However, the amount of caffeine within an energy drink is regulated. "The FDA...regulates all our
food and beverages, and they set a limit on how much caffeine can go into a beverage." (S2, P4)
Thus, it is impossible to have for energy drinks to have too much caffeine in one recommended
serving. Instead, it is the public that is overdosing and consuming too much caffeine, hence causing

The International Classification of Diseases (ICD), first initiated in Paris, is the official world
classification used by health practitioners across the world to record the diagnoses of all patients
seen in psychiatric care where official statistics are collected (Tyrer, 2014). The Diagnostic and
Statistical Manual of Mental Disorders (DSM), published by the American Psychiatric Association
(APA), is the official classification in the USA used mainly by psychiatrists for clinical diagnosis
(Tyrer, 2014). The main differences between these classification systems include: the ICD focusing
on primary care and low– and middle–income countries, while the DCM focuses on secondary
psychiatric care in high–income countries; the ICD reducing the number of diagnoses with the focus
being on clinical utility, while the DSM increases the number of diagnoses with each revision; and
the ICD does not depend on operational criteria but instead provides diagnostic descriptions and
guidance, while the DSM uses a polythetic system for most conditions which depends on
operational criteria (Tyrer, 2014). These differences result in the latest revisions of these
classification systems, the proposed ICD–11 and the DSM–5, diagnosing mental disorders like
PTSD differently, with the ICD–11 criteria identifying less individuals than the DSM–5 (Hafstad et
al., 2017). Worldwide this raises a challenge for the identification of

Exposure to the object or situation brings about an immediate reaction, causing the person to endure
intense anxiety (nervousness) or to avoid the object or situation entirely. The distress associated with
the phobia and/or the need to avoid the object or situation can significantly interfere with the
person's ability to function. Adults with a specific phobia recognize that the fear is excessive or
unreasonable, yet are unable to overcome it.PSYCHOTIC DISORDERS:– These illnesses affect an
individual's mind and alter a person's ability to think clearly, make good judgments, communicate
effectively, understand reality, behave appropriately and respond emotionally. When symptoms are
severe, people with psychotic disorders often are unable to meet the ordinary demands of daily life.
People with this illness have changes in behaviour and symptoms, such as delusions (a feeling of
being watched or monitored in some way) and hallucinations (hearing, seeing or smelling things that
aren't there) that last a very long time.SUBSTANCE–RELATED DISORDERS:– According to the
DSM, substance related disorders are separated into two types: 1. Substance use/abuse disorders.
Substance use disorders include abuse and dependence of any substance. Substance dependence is
characterized by continued use of any substance even after the user has experienced serious
substance–related problems.

Failure To Abnormal Behavior
Behaviour is considered abnormal under this definition if it is deemed to be unacceptable in the
society. This approach comes with many limitations. Social norm could be a desirability and is
easily manipulated. Some societies find certain people undesirable, often the ones opposing
government or religion. In USA for example, in 1950s people with communist views were sent to
mental health institutions. Every society is changing with the time, change of believes and affected
by other factors. For example homosexuality was classed as abnormality by World Health
Organisation until 1980s, but nowadays it is normal in the Western Society, while there are still
numerous societies, where homosexuality is classed as abnormal. The norms vary between the
societies so significantly that a norm in them could be an opposite behaviour. ... Show more content

person is abnormal. This method is very useful as gives a clear checklist for assessment. At the same

potential psychological problems and possible need of professional help. Secondly, any behaviour
must be considered in the context, which is not reflected in the assessment. Finally, person with
mental disorder in some cases is able to function adequately in day to day life, while person affected
by factors, unrelated to the mental health could be failing. Deviation from Ideal Mental Health This

Affordable Care Act Case Study
Universal health care is not impossible to achieve, but takes lots of planning and strategy to make it
work. As President Obama campaigned, he strongly emphasized the importance of universal health
care. With a great team and planning, the Affordable Care Act, also know as Obama Care, was
passed. The ACA should be kept because it covers health disorders, businesses with employees have
to have insurance, and allows parents to add their children to their plan up to the age of 26. The
Affordable Care Act makes it possible for the US population to have access to low cost health
insurance. It also protects individuals from being dropped from coverage when they get, be denied
treatment, or get charged more for being sick. The main problem is that many people don't like the
mandate because taxes are heavy on people with high incomes and on the healthcare industry. Since
the system is private it can also be confusing and customers risk themselves paying more or less
money. The higher the price individuals pay the better the quality. Insurance establishments are

since depression is over diagnosed. He stated that 11% of the population of the Americans is on
antidepressants. Another Psychiatrist, Dr. Suzane Renaud was hoping for DSM–5 to include things
for borderline personality. She said "I would have expected the DSM–5 to use the multi–

research, but they kind of withdrew it, saying that it was too complicated for clinicians." Below

Mental Healthcare in America
Mental Health Care and the American Social System
American history is littered with tails of reform and revolution. Earlier on in America's young life,
revolution included war, struggle for basic human rights and dignity as well as radical tactics taken
by the public. As time went on Americans learned that revolution and reform could occur through
the government systems that our forefathers had put in place. The battle for human rights has all but
ended but the way in which Americans wage war is a different story altogether. Now American's
fight for better public healthcare, equal rights for homosexual individuals and couples, and stricter
standards for social welfare programs.

With her endless efforts, Dorothea Dix succeeded in helping create a congressional bill regarding
the government's role in mental health care. Unfortunately after the bill had been passed by both
houses, President Franklin Pierce vetoed the proposal. Even though Ms. Dix did not see the bill she
created become law, she did manage to create 32 hospitals for the mentally ill and approximately 15
schools for the mentally handicapped and blind (Marshall, 1937). Dorothea also helped establish
several training facilities for nurses and established libraries in numerous prisons across the nation.
Though Dorothea Dix was considered to be ahead of her time, the efforts she made and her
accomplishments were key in establishing a better mental health care system in America.
Another great advocate for the mentally ill in America was a man named Clifford Beers. This Yale
graduate and business man was subjected, firsthand, to the torturous treatment utilized by metal
health care facilities in America when he suffered a psychological breakdown in 1900. After years of
degradation, Mr. Beers made it his mission to expose the atrocities being committed through the

Healthcare industries around the globe caring for people at their most vulnerable are always
conscious of the risk associated with their industries. Hence, these life–sustainer enterprises begin

The World Health Organization defines Safety and Risk Management as " activities or measures

real or potential (patient) safety event"1
Risk management in the International Classification of Diseases (ICD) could be defined as ICD's

The Prevalence Of Pressure Ulcers During Home Settings...

  • 1. The Prevalence Of Pressure Ulcers During Home Settings... Methodology This DNP project will be a retrospective chart review on the prevalence of pressure ulcers in elderly in home settings receiving services from a community–based health care provider located in New Jersey with a total patient population of over 3000 in eight counties. Prevalence will be defined as the total number of patients who currently have pressure ulcers or have been treated for pressure ulcers and the risk associated with the development of pressure ulcer for the past 12 months. A retrospective chart review has been chosen for this project because it is often used to evaluate associations between models of care and the clinical outcomes of interest (Hess, 2004). It is also an inexpensive way to research accessible data for any condition where there is latency in available data and most importantly to generate hypothesis for future studies (Gearing, Mian, Barber & Ickowicz, 2006). Setting: The reviewer will extract 100 convenience sample charts from the electronic database of the participating primary practice to determine the prevalence of pressure ulcer. The practice is a community–based primary health care provider that services the elderly in their homes. The majorities of these elderly are cognitively impaired, have mobility problems, and can no longer leave their homes and are in need of medical care. The geographical locations of the patients are Passaic, Essex, Bergen, Union, Middlesex, Morris, Monmouth, and Ocean counties in New Jersey. ... Get more on HelpWriting.net ...

  • 3. A Research Study On Autistic Spectrum Disorder Autistic spectrum disorder, also known as ASD can be defined as a group of complex developmental disorders characterized by impairment in verbal and nonverbal communication, behavioral challenges, impaired cognitive abilities, and impaired social interactions. Nurse practitioners providing primary health care to children should be familiar with ASD for early identification, evaluation, and management as this is significantly important to the well–being of children and their families. The purpose of this paper is to raise awareness............. The history of ASD dates back to the year 1908 when the word autism was first used to describe a subset of withdrawn schizophrenic patients. The word autism was then used again in 1943 when American child psychiatrist Leo Kanner, M.D., published a paper called "Autistic Disturbance of Affective Contact" describing eleven socially isolated children who displayed the same desire for sameness and aloneness describing their condition as early infantile autism (Autism Speaks, 2013). A year later German scientist named Hans Asperger reported on boys who were highly intelligent, but had obsessive interests and trouble with social interactions. He described these boys as having a milder form of autism, now known as Asperger's syndrome. During the years 1950's to 1960's autism was classified as schizophrenia in the International Statistical Classification of Diseases and Related Health Problems and the term "refrigerator mothers" was used to describe ... • 5. Nursing Terminology System Vs. The International... Nursing Terminology System Comparison Nursing terminology could be described as the formal and informal communication that occurs in a nurses daily activity. According to McGonigle & Mastrian (2009) nursing terminology allows nurses to communicate nursing data, information and knowledge specific to nursing. In addition, standardized nursing terminology refers to a system. This means they have undergone approval by a specific authority. The American Nurses Association (ANA) is one authority with a standardized nursing practice and information infrastructure committee, which recognizes twelve standardized terminologies, both nursing specific and interdisciplinary (p. 98). Dr. Saba has pioneered the integration of computer technology and nursing. The ICNP was developed by the International Council of Nurses (ICN) to cover diagnoses, nursing actions and nursing outcomes as it relates to nursing phenomenon (p. 99). The CCC website (2012) describes this system of nursing terminology as a "unique coding structure for assessing, documenting, and classifying patient care by nurses and other clinical professionals in any health care setting" (www.sabacare.com). The CCC system serves nursing as well as other health care providers such as physical, occupational and speech therapists and medical social workers. The ICN website (2015) reports ICNP was created to improve worldwide healthcare as an integral part of the global information infrastructure to influence health care practice and policy. This database is intended for use by nurses, who can describe and report in detail, nursing assessments and interventions, as opposed to being a system for other clinical healthcare professionals. The ICNP is reported reliable to support care and effective decision making and useful for nursing education, research and health policy (www.icn.ch/what–we– do/about–icnpr/). • 7. Evaluation Of Counseling As Assessment And Diagnosis Whiston in section three of her book provided an in–depth analysis of applications and issues of assessments in counseling as assessment and diagnosis are vital concepts in the counseling process. It therefore becomes imperative for differing counselors to understand formal diagnostic procedures and the fact that "diagnosis is the prevalence of individuals with significant psychological difficulties" (p. 296). Diagnostic systems are used to provide professionals with a general language governing diagnostic terms to eliminate individuals' use of personal definitions. The most widely used diagnostic systems within the mental health service are; the Diagnostic and Statistical Manual of Mental Disorders (DSM–IV– TR–TR) and the International Classification of Diseases (ICD–10– CM). The use of diagnostic systems is also considered "helpful in conceptualizing cases" (p. 296). Effective use of a diagnostic system requires a "thorough appraisal of a client's problems, physical condition and symptoms, and environmental influences" (p. 297). A diagnosis is usually derived from client information obtained from either informal or formal assessment tools or simply an amalgamation of both. The DSM–IV–TR diagnostic system is multiaxial and captures the information needed for treatment planning on five axes labeled axis I, axis II, axis III, axis IV and axis V. Axis I looks at clinical disorders and other clinical conditions that may be a focus of attention. Axis II captures personality ... Get more on HelpWriting.net ...

  • 13. The practice of seclusion and restraint in psychiatric... Research Paper Evaluation of the research articles This work is using two original research papers and their findings to analyse the validity of the practice of SR (seclusion and restraint) in psychiatric inpatient facilities. The research papers chosen shed light on this questionable practice by analysing multiple patient characteristics and the events leading to SR. The other chosen research paper evaluates the inpatients perception of SR. The first research was found in the 'Journal of the American Psychiatric Nurses Association' titled 'Characteristics of patients with histories of multiple seclusion and restraint events during a single psychiatric hospitalisation' and aimed by means of a quantitative study to find solutions to ... Show more content on Helpwriting.net ... For example inpatients have many times vocalised and/or shown by their response to SR, that they believe this strategy is merely used to exert power over them (Mehan et al. 2004). (Taylor et al. 2012) quoted in their discussion 'The results of the present study suggest that patients' opinions were not included in treatment planning. This might show that paternalistic decision–making still exists in psychiatric hospitals.' Both research papers selected allow for transferability since their findings in mental health facilities located in the USA and Finland are the same as the observations gained in Tasmania, ranging from age, sex, aggression criteria and inpatient perceptions of SR (Colorado State University 2011). If the findings can be generalised could be answered with 'yes' and 'maybe', 'yes' because the application of the research results can be applied beyond those examined, since the criteria are always the same, people with mental illnesses, SR in mental health facilities and aggression or violence shown, 'maybe' because similar research studies conducted in countries with a lower threshold for valuing human life and therefore higher thresholds of fair treatment expectancy by inpatients would change the research criteria and results (Collis and Hussey 2003, p. 225). Essentially it can be implemented to make the process of filing a insurance claim much easier and hassle free so that you do not get a denied form back when you submit a claim on behalf of the patient. I came across this when I was searching about new technology that has been implemented in modern times as per chapter 2 section on developing new technology and how it has always had benefits and disadvantages within the field of healthcare. From what I have researched it would appear as though ICD–10 is no exception. I found an article talking about the pros and cons of this program and essentially what it comes ... • 17. Advantages And Disadvantages Of Health Insurance Germany is the first to establish the national social health insurance system in the world since 1883 [136]. The health insurance in Germany is mandatory for all the population [137], and as a universal multi–payer system, it is split in two parts. One is the statutory health insurance (SHI), roughly 85% of the population who are with a free choice of the comprehensive health insurance plan provided by each of more than 200 competing SHI companies [138]. Of these, the medical services are reimbursed by SHI companies, and the framework and contents of quality assurance for these services is specified centrally by the Federal Joint Committee which is the highest decision–making body of the joint self–government of physicians, dentists, hospitals and health insurance ... That may result in various distortions concerning false–positive and false–negative diagnoses, and consequently, studies on validity and plausibility are required. Although an internal validation procedure can be developed to deal with false–positive diagnoses, false–negative diagnoses were unable to be identified [150]. Moreover, claims data may have limitations such as missing data on implants and operation techniques, the subjectiveness of patient evaluation or deficits in documentation [149]. However, SHI data avoid selection and recall bias by large and provide sound, detailed information on patient–level health care service utilization and direct medical expenditures in a real world setting. This enables ample cross–sectional and longitudinal analyses on care structures and expenditures components in many kinds of subgroups. What is more, it provides data needed to evaluate population–based interventions [151]. ... Get more on HelpWriting.net ...

Therefore, the codes have been greatly expanded and this expansion allows a significant increase in the specificity of the reporting in the ICD–10, allowing more information to be conveyed in a code. For instance if you had a burn on your right arm in did not specify which arm had the burn in the old ICD–9, now in ICD–10 it specifies which arm has the burn. The terminology has been modernized and has been made consistent throughout the code set. There are codes that are a combination of diagnoses and symptoms, so that fewer codes need to be reported to fully describe a condition. The reason they changed it is because the practice of medicine has changed considerably in the last 25 years, and the ICD needed to reflect those change. There have been many new diseases discovered, new type of medical devices and new treatment and the ... Get more on HelpWriting.net ...

In addition to its adverse health problems, behavior related to overeating may have significant psychological and neurological underlying conditions (Moreno & Tandon, 2011). Research indicates that there may be an addiction component associated in the development of eating disorders, as there are similarities between substance dependence and overeating (Uher & Rutter, 2012). Their shared neurobiology and treatment, has led to new conversations and suggestions for a new category for food addiction in the new DSM–V, as well as be included with substance disorders under a broader category for addiction disorders (Moreno & Tandon, 2011; Uher & Rutter, 2012). The current classification systems of mental disorders in the Furth Edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM–VI), Fifth Edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM–V) and the Tenth Edition of the International Classification of Diseases (ICD–10) are limited and challenging due to the complexity of symptoms that may occur in on one specific disorder (Hebebrand et al., 2014; Robbins et al., 2012). • 25. Application Of Icd 9 And Start Using Icd 10 Eloy Quesada As of October 1, 2015, all healthcare providers will be required to stop using ICD–9 and start using ICD–10. Switching over to the more in depth system of ICD–10 will require adjustments from large healthcare providers, like hospitals, as well as small practices run by doctors in order to successfully adapt. Some will have to adjust more than others. While hospitals are bigger and more complex, they will have an easier time adjusting to ICD–10 than small practices, which are operated by a single physician or a group of physicians. ICD stands for International Classification of Diseases and is used by countries all over the world as a standard diagnostic tool. ICD has many uses throughout the health industry. For the most part ... Show more content on Helpwriting.net ... The reason for the need of all these extra details is because not only does it increase accuracy in medical records but it also allows doctors to administer better patient care. Using ICD–10 for reimbursement purposes will also be a lot more efficient and cost–effective. With ICD–9's broad classification system some procedures, associated with certain codes, would be unnecessary for specific patients. However, with ICD–10 because the codes are so specific, procedures associated with those codes will all be necessary for further diagnoses and no procedure will go to waste. As necessary as ICD–10 is, smaller practices that only have a few physicians on staff will have more of a difficult time integrating the system than larger organizations. The reason being that larger healthcare organizations tend to have coders. Coders are individuals who hardly ever have contact with patients but they are important in the sense that they are the ones that input the ICD codes into patients' medical records. Without coders, physicians would have to be the ones to input the ICD codes into the patients' medical record. Luckily physicians at hospitals and other large health organizations do not need to do this. Unfortunately, physicians at smaller organizations do not have coders at their disposal and they are the ones that enter the ICD codes. How does this pose a problem for the physician or ... Infantile Autism Research Paper The word autism comes from the Greek word "autos," meaning "self;" although the term itself has been used for about 100 years, it was initially used to describe one set of symptoms of schizophrenia. Eugen Bleuler, a Swiss psychiatrist, was the first person to utilize the term around 1911. In the 1940s, researchers in the United States began to use the term to describe children with social and emotional troubles. Leo Kanner, an American child psychiatrist, published a paper in 1943 describing 11 children who were greatly intellectual but displayed "a powerful desire for aloneness" and "an obsessive insistence on persistent sameness." He ultimately classified their condition as "early infantile autism." Subsequently, Hans Asperger indentified ... Show more content on Helpwriting.net ... By this time, autism had been categorized below schizophrenia in the International Statistical Classification of Diseases and Related Health Problems, although researchers are now mindful that there is no relation between the two conditions. In 1977, research on twins found that autism is significantly influenced by genetics and biological differences in brain development. Autism was officially distinguished from childhood schizophrenia and added to the DSM in 1980, and in 1987, it was given a more expansive description and a list of diagnostic criteria. In some diseases or disorders this special function of the brain is either lost or diminished. One such disorder which diminishes the functions of the brain is the Attention Deficit Hyperactivity Disorder. This disorder was initially considered to affect only children but this is a misconception as this disorder is also noticed amongst adults. In the latter stages of 1960 the disorder became known as "Hyperkinetic Disorder of Childhood". In 1970s more symptoms were joined with this disorder which showed how an individual would behave when he had ADHD. It was in 1980s that the name Adult Deficiency Disorder evolved and this was included in the DSM III given by the American Psychiatric Association. • 39. Depression : The Causes Of Depression As A Medical Depression Depression is a medical illness that affects negatively the way an individual feels, thinks, and acts. MDD occurs when a person goes through extreme sadness for an extended period. The symptoms of MDD vary from mild to severe. A person feels sad and depressed, loses interest in activities, feels worthless, and starts having suicidal thoughts. MDD affects a person's life as it impacts a person's mood and behavior as well as sleep and appetite. People with MDD lose interest in activities, develop poor concentration and loss of energy. These problems become embedded in an individual and make it difficult for them to cope with everyday responsibilities. MDD is a common mental disorder in the United States. Some individuals do not seek treatment, but those who seek treatment get better through medications and psychotherapy. The precise cause of MDD has not been identified, but several factors increase a person's vulnerability to the condition. These factors include alcohol and drug abuse, stressful medical diagnosis such as cancer, and medications such as steroids. Different schools of thought have developed their own theories to answer why people become depressed. Psychological beliefs provide explanations for people's thinking and why they behave the way they do. Freud and Aaron Beck developed theories that help explain the causes of depression. Personality factors, history, and personal relationships are important factors that contribute to depression. Cognitive theories became prominent to integrate mental events into the behavioral framework. Cognitive behavioral theorists believe that depression is a result of irrational cognitions. According to Aaron Beck, dysfunctional beliefs create negative thoughts that cause depressive symptoms (Disner, Beevers, Haigh & Beck, 2011). He asserts that having more negative thoughts increases the depression that a person will experience. Beck stated that a person has three dysfunctional beliefs that predict the probability of occurrence of depression. They include the belief that a person is defective, the belief that a person's experiences result in failure, and the belief that the future is hopeless. If these beliefs are present in a person's mind, there is a high ... Get more on HelpWriting.net ...

Over the next few years, NIMH will be conducting a series of workshops focused on further refining the definition of the broad domains of functioning and respective dimensional constructs and subconstructs covered within each domain. Additionally, these workshop groups are tasked with organizing and disseminating gold standard measures or indicators of analyses for the previously mentioned areas of study (e.g., genes, neural circuitry, physiology). As President Obama campaigned, he strongly emphasized the importance of universal health care. With a great team and planning, the Affordable Care Act, also know as Obama Care, was passed. Dr. Joel Paris, professor and chairman of the department of psychiatry in McGill University, Montreal said there is a big problem with the new manual because it doesn't address the "diagnostic epidemic". He stated that 11% of the population of the Americans is on antidepressants. Another Psychiatrist, Dr. Suzane Renaud was hoping for DSM–5 to include things for borderline personality.
