This document provides instructions for an essay on chronic disease management. Students are asked to choose one of four chronic diseases: hypertension, chronic obstructive pulmonary disease, diabetes mellitus type 2, or childhood asthma. For the chosen disease, students should detail the affected population including characteristics, contributing factors, past medical history, and genetic components. They should then evaluate the population size, seriousness of the disease, and special needs. Finally, students should assess the need for formal case management and argue the potential benefits of implementing a case management model in terms of economics, quality of life, social impact, and more. They are also asked to analyze why nursing should be involved and identify other team members and their roles.
