- Wisdom teeth typically erupt between ages 18-25 and often cause problems requiring extraction. There are many complications associated with third molar extraction due to root variants.
- The study examined variants in root numbers and morphology of wisdom teeth using panoramic x-rays and CT scans. It found that most patients had 2 roots, with fused roots being the most common variant.

As you know Wisdom teeth, or third molars, are the latest teeth in the development of permanent dentition. They generally erupt between the ages of 18 and 25 and often cause many problems such as Adjacent second molar caries, pulpitis, cellulitis, malocclusion, therefore need to be extracted.
There’re a lot of complications before, during and after 3rd third molar extraction…..
In the world and in VN there were a lot of study about the wisdom teeth, morphology of wisdom teeth and about the role of Panoramic Xray in diagnosis and prognosis in impacted mandibular third molars.
In the book dental anatomy of Hoang Tu Hung, he said that, Normally, the root external morphology of wisdom tooth was similar to this one of the 2nd molar: had 2 root: misial root and distal root. The root were curved distally.
But.. The majority of their roots was variant. You can see this wisdom tooth had fused roots, and this one had 4 roots, and this one had an accessory root..

You can see in this panaroma, the root shape of 38 was similar to the root shape of 37
And here in this Panoramic X-ray You can see the Panoramic image was conformity with the real tooth. (38)
And in this case. You can see clearly that the panorama was not conformity with the real root. we had to use CT scanner to determine exactly the root shape. In this case the root curved bucally.
Let focus on some review of the recent related studies. All most of these studies showed that the majority of wisdom teeth had 2 roots. And about the role of Panorama le duc lanh said:

The next part is my results and discussion. Let’s take a look at the table 1: as you see The ratio of patients in the age group 21-30 and 31-40 had higher percentage (54.4% and 28.1%) more than in the remaining groups. This results was consistent with the form...
Now take a look at| the F1 we talk about the ratio of variants in mandibular wisdom teeth. In total of 57 lower impacted wisdom teeth were extracted the percentage of lower wisdom teeth which similar to the 2nd molar root was 14%. The ratio of variants was 86%. This results consistent with
Now I would like to move on to the F2 : as we can see the Proportion of wisdom teeth having 2 roots (normal quantity) was high as 77.2%.
Variant root having 1 root had the highest proportion being of 12,3% (accounting to 53.7% of total variations in quantity)
Variant root having 3-4 roots had a low rate (8.8% and 1.8%)
Next, we focus on the F3 : you can see that the Proportions of normal roots made up to 19,3 %, the others had proportion was 8,8%
Fused roots had the highest proportion of 31.6%, followed by mesio - angular roots with percentage of 15.8%. Equal length roots, straight roots contributed to 10.5% while apical fused roots made up only 3.5%.
The majority of root morphology were fused.
similar to previous studies, like the study of Hoang Tu Hung, showed that The majority of wisdom tooth had fused roots.
Continue with the figure 4. You can see clearly that the ratio of variations by morphology was very high, more than the variations by quantity. And the combination were 23 %. The difference was statistically significant.
We find out that when the variations by quantity appear, the variations by morphology appear too.
This result was consistent with the review of Hoang Tu Hung.
The restriction of this study was the number of studied teeth still small, so have not reflected fully about the variations of impacted lower wisdom teeth, and about the relationship between the variations.
Finally, look at the F5: you can see Most of Panoramic X- ray assessed correctly the image of impacted wisdom teeth but still had a percentage that nonconformity that because the roots or the root apex were obscured, because of limited of Panoramic X- ray, just reflects two-dimensional space.
