EMPLOYMENT HISTORY
Sandalwood Healthcare & Rehabilitation – October 20, 2015– March 26, 2015
Social Services Director – Covered all aspects of Social Service needs, resident advocate on
concerns and needs, worked closely with families on their concerns and planned care plan meetings
to resolve issues rather it be in nursing, dietary, maintenance and kept record of all complaints and
oversaw that complaint was resolved. Conducted the MDS Social sections for MDS which includes
mood, behavior, cognitive skills and delirium, attend Medicare meetings and care plan meetings.
Planned care plan meetings with families that were having problems to prevent from calling OLTC.
Worked closely with families of the facility and consulted as needed. Conducted psychosocial and
mental assessments as needed, met with families regarding behavior, current status changes and
mood behaviors. As needed, made referrals to mental health facilities and psych units upon
behavioral changes as needed. Networking with mental health facilities for over 6 years through
referrals and consulting. Worked with APS on residents under our care to meet residents’ needs as
required. Keep documentation to date on all residents. Discharged rehab patients back to community
though housing resources and durable equipment needs. Developed strong resources in housing and

discharge planners in hospital or Geri-psych units. Developed a social history, social evaluation, and

Sandalwood Healthcare & Rehabilitation March 2011 – November 2011
Marketing/Social Director
Marketing targeted to the healthcare industry, hospitals, physician offices to increase census 2011 at
the facility for both Medicare and Medicaid referrals. Increased census by 25% since starting in
March, 2011. Planned in-services for staff and other agencies to promote the facility. Worked
closely with all hospital social workers and discharge planners. Exhibited at various venues to
promote awareness of facility. Over 29 years networking with the medical community and
healthcare organizations. Performed Social Services and admissions for new admits and conducted
discharge plans for those in Rehabilitation to outside organizations. Worked with Hospice
organizations for orders on of Hospice Care for residents. Planned Family Council Meetings.
Worked closely with APS on all APS residents, appeared in court for APS residents and took to
community mental health for their pysch evaluations if necessary. Resident group counseling.

admissions to the facility. Marketing tools were brochures, pens, pads and setting up
luncheons with physician offices. Conducted work with the Media and print for advertising of
facility. Planned and implemented all special events such as Open House and Physician visits
to facility. Facility is a 90 bed, Medicare and Medicaid with 20 of those being Medicare
Rehab. Through Marketing, increase census to 80 while in this position. Performed Social
Services and Admissions to new admits, face sheets for charts, Medicare letters to those
discharged from Rehabilitation, Conducted Initial Social Services assessments and provided
necessary chart information which dealt with the Social Services aspect and completed MDS

complaints. Served on family care plan meetings both with long term care and Medicare families.

EMPLOYMENT HISTORY Sandalwood Healthcare & Rehabilitation – October 20, 2015– March 26, 2015 Social Services Director – Covered all aspects of Social Service needs, resident advocate on concerns and needs, worked closely with families on their concerns and planned care plan meetings to resolve issues rather it be in nursing, dietary, maintenance and kept record of all complaints and oversaw that complaint was resolved. Conducted the MDS Social sections for MDS which includes mood, behavior, cognitive skills and delirium, attend Medicare meetings and care plan meetings. Planned care plan meetings with families that were having problems to prevent from calling OLTC. Worked closely with families of the facility and consulted as needed. Increased census by 25% since starting in March, 2011. Planned in-services for staff and other agencies to promote the facility. Worked closely with all hospital social workers and discharge planners. Exhibited at various venues to promote awareness of facility. Over 29 years networking with the medical community and healthcare organizations. Planned and implemented all special events such as Open House and Physician visits to facility. Facility is a 90 bed, Medicare and Medicaid with 20 of those being Medicare Rehab. Through Marketing, increase census to 80 while in this position.
