1. Risk Analysis: Carrying out quantitative risk analysis study as per IS: 15656 including identification,
screeningandrankingof riskscenarios,Quantitative analysis of risk assessment, consequence analysis
of the variousriskscenarios,studyof Domino effects,probabilisticassessmentof risks,recommendation
and preparation of reports and relevant drawing showing damage and risk contours.
It includesidentificationof systemdefinition:defining the installation or the activity whose risks are to
be analyzed.
a) Identify Major Accident Event (MAE) include release of flammable liquid/vapor.
b) IdentifyexistingEngineeringand/orOperational controlsandmeasures that are included in the
design for prevention or mitigation.
c) Identify those controls that are safety critical to the identified MAE.
QRA to be carriedout byidentifyingthe hazards at Gurgaon BP estimating the likelihood of occurrence
of the hazardous events and mathematical modeling for calculating the consequences of each event.
The QRA to be carried out using the following steps:-
i) Identificationof Hazards(Fire/Explosion/UncontrolledRelease of LPG/BLEVE/Unconfined vapor
cloud explosion etc.)
ii) Identification of maximum credible accident scenario using inputs from Fault Tree Analysis,
Event Tree Analysis etc.
iii) FrequencyAnalysis.Evaluatethe likelihoodsof occurrence of possible events. Select worst case
scenario.
iv) Consequence modeling and Analysis for the identified hazard covering impact on people and
potential escalation.
v) Vapor Cloud Explosion scenario and unconfined vapor cloud explosion scenario due to
uncontrolled leakage of LPG shall also be worked out.
vi) Assessment of risk arising from the hazards and consideration of its tolerability to personnel,
facilityandenvironment.Assessmentof risk to individual and/or societal and neighboring area
and contour mapping.
vii) Recommended additional mitigation method using ALARP principle.
Software to be used are Leak 3.2, Phast v 6.54, PhastRisk v 6.6, PHA Pro v 8.0 or any internationally
accepted equivalent software.
