This information is being provided at the request of Misti D. Colon as a professional letter of
reference, having been a contracted employee of the Patterson Army Health Clinic, Army Substance
Abuse Program, under my supervision as the Director of the Army Substance Abuse Program,
respectively. Due to the fact that Fort Monmouth is closing in the near future (BRAC), her position as
a contractor cannot be renewed.
Misti D. Colon has been employed by the above-referenced organization since 10 June 2008. She has
held the following position(s): Office Automation Assistant and Medical Records Technician. In
her most recent positions, she has had many responsibilities: IE: Answering multiple phone lines,
preparing, maintaining and organizingmedical records, data, care and disposal of personally
identifiable and protected medical/mental health information, computer security, coding and indexing
routine records, maintaining metrics and statistics for quarterly reports, scheduling appointments into
the Army medical database called CHCS/DEERS, performing background checks and entering patient
intake records through the Pentagon's website DAMIS for military patient's, successfully training for
HIPAA compliance, Core Competency Assessments, other military-required and clinical training
under the requirements of the Patterson Command, including but not limited to the above mentioned
skills and her CPR Certification as well.
Misti D. Colon interacted well with co-employees, was reliable and punctual, and showed very good
staff support skills as a valued team member of our office. If you would like to discuss this further,
