Description of how I made the questionnaire.

1. To create my questionnaire I used ‘Word.’
First I wrote the title with the text tool and
rotated it vertically. I then wrote my first
question and using the shape tool, made 4
rectangles. I adjusted the sizes of the
rectangles so that the tick boxes where
smaller. Inside the smaller rectangles, I made
2 more red squares.
