Needs Assessment A community health assessment is a fundamental instrument of public health practice. Its objective is to depict the health of the
community, by presenting information on health standing, community health needs, resources, and epidemiologic and other studies of present local
health problems. It seeks to recognize target populations that may be at augmented risk of poor health results and to increase a better understanding of
their needs, as well as evaluates the larger community surroundings and how it relate to the health of people. It also identifies those areas where better
information is desired, particularly information on health differences amid different subpopulations, quality of health care, and the incidence and
severity of disabilities in the population. The Community Health Assessment is the foundation for all local public health development, giving the local
health component the instance to recognize and network with key community leaders, businesses and concerned residents about health priorities and
concerns. This information shapes the foundation of improving the health status of the community by way of a strategic plan (The Municipal Public
Health Services Plan Community Health Assessment Guidance and Format, n.d.).

When recalling completing the census for 2010; there were many questions on health and smoking. It left a question as to how this data would be
used. In more recent times there has appeared to be an increase in surveys from medical, corporations, and sales. All of the data gathered is used to
assess how they are doing and are needs being met, the same is true for health assessments.
Disadvantages and Benefits One of the big disadvantages is the potential for misuse of the information. An example of misuse is the Breach of
HIPAA policies. While working in the military health environment in Spain, there were many times that someone I knew socially was a patient in the
clinic. One such encounter netted a good friend's whose husband was treated for an STD (Sexually Transmitted Disease) and was prompted to notify
all his sexual partners. His wife did not come in for a visit; which caused a moral dilemma, questioning what to do; I spoke with the treating physician
and voiced my concerns. The physician took charge and the appropriate action was completed, thereby avoiding a HIPAA violation. In the wrong hands
medical information can be abused. The stringent punishment for HIPPA violations has curtailed the misuse of information and limited the public

complimentary walk–in sexual health service which provides medical consultation/advice, testing and treatment for sexually transmitted infections
(STIs) (SA Health 2016a). Ultimately, this paper will illustrate how an ongoing health assessment, history taking, provision of client education and
care options of certain STIs are fundamental to guiding the planning, implementation and evaluation of care for specific people. All information

guidelines of the Government of South Australia (2015, p.7; Nursing and Midwifery Board of Australia 2016). On 16/9/16 R presented to the clinic
and upon arrival, completed a form to guide his care plan (Gulanick & Myers 2014, p. 852–7; SA Health 2016a). This form revealed he was

Unfortunately no assessment tools or guidelines were received in response to these approaches. We were referred to four different assessment tools
/processes which are currently in use in England: Derby City Health Needs Assessment Tool, Tynedale Health Needs Assessment Tool, Kirklees
Universal Needs Assessment and the Common Assessment Framework. The Derby, Tyndale and Kirklees assessments are locally developed and used
by Health Visitors and school nurses. The Common Assessment Framework is a central government initiative, with the Every Child Matters
framework, which is designed to be used by practitioners working in any children's services, not only health.

When conducting a needs assessment, people's needs with in the community are assessed to see where gaps lie and improvements can be made. If
carried out and reported correctly these needs are then bought to local government or relevant organisations/peoples attention, giving the opportunity
for programs to be put in place to benefit the people of the community. This is where the third step of Finifter, Jensen, Wilson & Koenig (2005)
process comes into place; the implementation of projects to assist where the gaps were found. "Implementation of recommended solutions should be

The organization I choose is Dignity Health Mercy Medical Center Redding (MMCR), located in Northern California. MMCR 2014 Community Health
Needs Assessment Report is easy to read and understand. They provided the picture of the community they are serving and what are the problems that

example of community that is defined by geography would be a neighborhood or residential area. Houses clustered together share a common space. The

but effective group of 5 individuals. The teachers and administrators that are involved with the ELs are extremely passionate and constantly working
with the numbers that they have to better service their students. The team meets frequently and is in constant contact within one another to help each
other overcome obstacles that are occurring within their classrooms with their EL students. Although the team is small, they are only servicing a small
population, and always open to change. If the EL enrollment were to increase, the team would increase as well. Their goal is to ensure that all of the
students are being serviced for all of their needs for success. Lastly, the districts community outreach programs for the amount of ELs in the district is
outstanding, with one–on–one meetings and communication, the families are constantly kept in the loop and current with their student's learning.
Along with that, the school also provides summer camp for ELs and additional supports. There are no improvements that I would suggest to this portion
of the program at this time.

Windshield Survey Within the community, a large amount of individuals are visible. Many are entering convenience stores and some are standing in
the parking lot areas talking. A variety of ages is noted and the two major ethnic groups in the community, Caucasian and Hispanic, are the two that
