Carolinas Hospital System Florence, SC
Responsible for running report of previous day’s inpatient discharges, retrieving, prepping and scanning
records into Kofax.
Travel daily to Cedar Campus inpatient rehab facility to scan wound care pictures and interdisciplinary
plans of care for discharges.
Print records for Carolinas Outpatient Rehab Facility from shared drive, prep, scan, quality control (QC)
and validate for previous days patients.
Scan, QC and validate Emergency Room records.
Help director with H&P list, researching to assure that H&P’s were done in a timely manner.
Release of information as requested in accordance with HIPAA regulations.
Assembly and Analysis Technician & File Clerk

Responsible for obtaining the previous day's Emergency Room records, checking that each record is
present, assembling and analyzing each for accuracy and prepping them ready for the on sight coder by
printing and attaching any missing information, such as labs or x-rays.
Maintained and kept file room, pulled, check in/out and filed charts as needed.
Release of patient information in a timely and appropriate manner according to HIPAA regulations.
Assisted patients daily via phone or in person on how to legally obtain their records.
Back up for outpatient services assembly and analysis (Radiology and Labs). Duties included obtaining
the previous day's records, checking that each record is present and prepping for the on sight coder.
Back up for birth certificate clerk. Duties include obtaining information on and from mothers for birth
certificates for their newborn, which required interacting with patients and at times their families.
Entering birth certificate information into the system and reported to Columbia, SC.
Other duties include but are not limited to answering phones, filling requests for records from doctors,
patients and other departments within the hospital. Scanning charts to off sight corporate coders.
Assisting with audits for the monthly DRG disposition reports as well as documenting said report which
required the use of Excel.

Responsible for general patient information services, which included loose document filing, pulling and filing
patient records. ER analysis, including previous day retrieval, assembly and accuracy check. Assisted in
purging of medical records and reorganization of file room. Filling requests for records whether by phone,
fax or face to face according to HIPAA regulations, as well as other duties requested of me by my
supervisor.
