LH Surge, hCG shot, IUI timing issues…HELP!?
I use the fertility monitor and got “peak fertility” (LH surge) on Saturday & Sunday. Yet my RE
had me take the hCG shot Sat. night and scheduled the IUI for Monday morning.
If I started surging Saturday morning on my own…wouldn’t I ovulate Sunday? And wouldn’t
Monday be too late for an IUI? Am I missing something here? I am worried we might be paying
for an ill-timed and pointless IUI….

Ovulation usually doesn’t occur until after the LH surge starts to diminish. Given that you were
still getting a positive surge on Sunday, it sounds like the timing is perfect to me. Besides, the
worst case scenario is that you ovulate on Sunday, but the egg is still viable Monday because it
will live about 24 additional hours (making Monday still good timing). It’s actually better to error
on the side of being a little later with an IUI rather than too early. I don’t think you have anything
to worry about.

Why does IUI with clomid require an HCG shot?
If there’s no infertility issues, what is the reason for an HCG trigger shot for ovulation? Why not
wait for a natural LH surge and time IUI procedure from the natural surge?

You can wait for a natural surge . . . But some doctors prefer to go with the HCG shot due to
scheduling (IUI is not a “walk-in” procedure), the shot will trigger a stronger ovulation, you will
be assured that you have ovulated in most cases (LH surge does not guarantee ovulation) and
it will support hormone levels like progesterone which will help with the uterine lining and
implantation.

TTC: LH Surge 3 hours after a HCG shot! Should I get IUI
tommorrow OR next day? Please I need your wisdom!?
I did the Femara earlier.
Monday:
2:45 pm: 2 follicles (22.00 mm & 13.50 mm) were found.
3:15 pm: Blood test revealed no LH surge.
8:00 pm: HCG Shot taken.
8:10 pm – no LH surge (I tested LH with urine test)
11:00 pm – yes a LH surge.
1:00 am – yes a LH surge (I retested to be sure)
If LH Surge began at 11:00 pm on Monday,
is it too late if IUI is scheduled for exactly 40 hours later?
Should I get IUI today instead?
Also, what is the optimal exact time for me to have IUI.
And why?

I Just talked to the Nurse, I told her everything, and
she said the HCG shot is doing it’s job,
it makes the LH surge or something.
And the IUI is timed good for Wednesday at 2:30 pm.

I was doing the LH tested through an LH kit.
I agree just “ask the doctor” isnt the only way to go.
I love asking questions here first,

I disagree that your doctor can answer these questions because, as my doctor pointed out, it’s
guess work a lot of the time.
When I was taking clomid, I would start to surge on day 11, the day of my US, take the hcg at
night, and then they wanted me to schedule the IUI for 12 hours later, not 36 hours, which is
when they like to schedule it if you take the hcg without surging prior to the shot.
Your question is a little different to mine. In your case, I would imagine you’d want to schedule
the IUI for 12- 24 hours after the LH surge (there’s also no way to know if it happened within
the 24 hours before you got tested).
I would get the IUI today. The sperm lasts about 12 hours when washed, so you would probably

5. want to have intercourse as well tomorrow, just to make sure you don’t miss your ovulation.
Are you having the LH tested through blood work or with an lh kit? An LH kit would record a
surge right after the shot because it interprets hcg as being LH, so if you are not surging on your
own at the time you take the shot, the IUI is typically scheduled at 36 hours after the shot. Sorry,
I started to understand your question as I got through my answer, that’s why I went back and
forth a bit.

Will a trigger shot override your natural LH surge if it was to be
before the 36 hour window?
TTC for 9 months. Clomid this cycle and ultrasound this morning showed a mature follicle. So,
received the trigger shot this afternoon at 4pm and IUI scheduled for 9:30 am Sunday. The
ultrasound was scheduled one day later than recommended due to my work schedule. If my
body was going to naturally surge today would the trigger shot override the natural process?
With IUI you only get one shot at things so I want to be as careful as possible with timing.

The hcg shot will override anything your body was thinking about doing naturally. You will
ovulate about 38 to 40 hours after hcg shot.

Does it make sense to get trigger shot and IUI at the same time?
I tested positive for ovulation Tuesday evening and had both the IUI followed by the trigger shot
at 10am the next day. Does that make sense? They weren’t going to do the trigger since I
tested positive on LH surge without it but then gave it anyway. Was the timing for the IUI or
trigger off? I called to say I tested positive for the ovulation the evening it happened and am
wondering why they didn’t have me do the trigger then instead of waiting for the next morning.
So when I called to report positive OPK ovulation would it have been too late to take the trigger
at that point?

I would always suggest a trigger shot and for one reason… To absolutely guarantee ovulation.
With an LH surge, it’s possible to surge and not ovulate… So there goes a wasted IUI. With a
trigger shot, you are forcing ovulation to occur in 36 hours. No if’s and’s or but’s. If you’re
going to do IUI, do the trigger.

On Gonal F, did you start surging before you took the hcg shot to
ovulate?
I’m just wondering because, on Clomid, my LH always started surging before it was time to take
the shot, so I could never time an IUI with certainty. I asked the nurse today and she said it was
less likely that I would start to surge on my own with Gonal F.
I was wondering what people’s experiences were because I would like to be confident that we
will time the IUI correctly.

I didn’t surge early on it, but by doctor used Ganirelix too. It actually prevents LH, then he gave
Luveris (LH) toward the end to mature the eggs prior to trigger.
