3. เจ็บข้อเท้าขวา 2 ชั่วโมงก่อนมารพ.

4. 2 ชั่วโมงก่อนมารพ. ระหว่างเดินทางานก่อสร้าง สะดุดก้อนอิฐ
ข้อเท้าขวาพลิก ผิดรูป เดินไม่ได้จึงเรียก EMS มาส่งโรงพยาบาล

5. A : Can speak , Full EOM of neck
B : Equal breath sounds , CCT Neg.
C : BP 120/72 mmHg PR 86 /min
D : E4V5M6 , Pupil 3 mm. RTLBE , Motor gr.V all
E : Right ankle deformity, limit ROM due to pain , No external
wound

6. A : No drug or food allergy
M : No current medication
P : No underlying disease , No history of surgery
L : Last meal 9.00 น.
E : ระหว่างเดินทางานก่อสร้าง สะดุดกองอิฐข้อเท้าพลิกเข้า ผิดรูป
เดินไม่ได้

7. Head and Maxillary
No wound, no facial deformity, no ecchymosis
C-spine and neck
No wound, Not tender along C-spine, Full active ROM of neck
Chest
Trachea in midline, Normal breath sound both lung, CCT negative
Abdomen and pelvis
Not distend, No wound, soft, not tender, normoactive bowel sound, PCT negative

8. Musculoskeletal
Deformity inversion position, Marked tender at Rt ankle, Limit ROM due to pain
DPA 2+, PTA 2+, Capillary refill < 2 s., Pinprick sensation intact
Neurologic exam
Alert, well co-operative, Pupil 3 mm. RTLBE
Motor grade V all
Perineum and rectum
No ecchymosis

correction for optimal outcomes
