Forklifts are an essential piece of equipment in many workplaces, especially in warehousing and manufacturing. Although these vehicles make work more efficient, they can pose serious hazards to those operating or working near them.
Accidents caused by powered industrial trucks (PIT), commonly called forklifts or lift trucks, can arise due to a misuse of the machinery itself, as well as poor or unsafe working conditions. As an employer, the biggest opportunity for reducing these incidents is to prepare your team and monitor the state of your workplace.
Forklifts are extremely useful workplace vehicles, as long as they are used safely and properly by operators who have been trained and are deemed competent to use them. Forklifts are dangerous: they account for 25% of injuries at work. Many workplace accidents involve people being hit or run over by forklift trucks (typically when the forklift is reversing). Forklift accidents can cause serious injury, death, property and product damage. Accidents can be prevented if risk assessment are conducted, safe work procedures are followed, regular Tool Box Talks are conducted and HSE continuous surveillance observations of work tasks are performed periodically.
Common Forklift Hazards:
• Overloading
• Unsafe Stacking
• Speeding
• Unauthorized operation of forklift
• Untrained forklift operators.
• Pedestrians and forklifts moving in the same place.
• Obstruction in the path e.g. overhead, uneven ground surfaces, blind corners etc.
• Reversing
Common Safe Practices:
• Prior to use, forklifts must be inspected by a competent person, & inspection sticker should be posted on it.
• Only trained and authorized operators can operate a forklift.
• Carry out daily forklift pre-operation checks using checklist.
• Report to supervisor any forklift damage or problem.
• Don’t use a forklift to lift a worker.
• Check the load loads carefully before moving them for stability and damage.
• Slow down and sound the horn at crossings, and locations where view is blocked.
• Do not authorize anyone to stand or walk under the load or forklift machinery.
• Be aware of the height of the load, mast and overhead guard of the forklift when entering or existing buildings.
• Don’t handle loads that are above the weight capacity of the forklift.
• Follow the speed limit.
• Flagmen must be available while forklift is in operation.
• Focus on the travelling path and keep a clear view of it.
• When leaving a forklift, pull the parking brake, lower the forks and neutralize the controls.
• Don’t drive and raise load at the same time
• Don’t make sudden sharp turns
• Don’t brake suddenly when the forklift is loaded.
• Watch out for overhead structures (e.g. pipes & beams)
• Follow traffic management.
• Use rear view mirrors and camera while reversing.

Dear Dr. Kornbluth and Mr. Gorenberg,
The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. The Committee on Oversight and Accountability is the principal oversight committee of the US House of Representatives and has broad authority to investigate “any matter” at “any time” under House Rule X.

On 2 Apr 2016, a worker was checking a forklift on the 4th storey of a building under construction
when the forklift suddenly surged backwards and fell through a floor opening. He fell together with it
and landed on the floor below. He was conveyed to the hospital where he later succumbed to his
injuries.

Recommendations*
Occupiers and employers with similar activities in their workplaces are advised to consider the
following risk control measures to prevent similar accidents:
De-energisation of forklift
Ensure that when a forklift is parked, the forks are fully lowered, controls put in neutral mode,
power shut off and parking brakes applied to prevent any unwanted movement.
In the event that an unmanned vehicle (including a forklift) moved inadvertently, the driver or
operator should not attempt to re-enter the moving vehicle. Instead the driver or operator should
immediately disperse any personnel near or in the travel path of the vehicle.
Operator competency
Ensure that all authorised forklift operators have minimally passed the Workforce Skills
Qualification Operate Forklift Course and obtained a Statement of Attainment.
Ensure that all authorised forklift operators have attended refresher training on the operation of
forklift every 3 years.

Forklift Maintenance
Ensure that all forklifts have undergone preventive maintenance in accordance with manufacturers’
recommendations.
Ensure that all forklift maintenance works are carried out by authorised, competent and trained
personnel.
Designated areas for forklift maintenance
Provide an area designated for forklift maintenance. Ensure that all forklift maintenance works are
carried out only at these designated areas.
Ensure that the area designated for forklift maintenance is not near any hazardous zones such as
an edge of building or any openings.
Daily Pre-operational check
Ensure that the authorised forklift operator carry out a daily pre-operational check before operating
any forklift. Any damage or defects spotted must be reported to the supervisor immediately.
Develop and provide a daily pre-operational checklist to facilitate the checking process.
Risk Assessment
Conduct a thorough RA for all work activities to manage any foreseeable risk that may arise in
connection with management and operation of forklifts. The RA should cover all foreseeable risks
and include the following:
- Possibility of any forklift movement while carrying out maintenance works; and
- Forklift operation near hazardous zones such as floor openings.
