Upon joining the executive, she realized the governance and management of the RCDSO did not meet best practices in several areas such as having no treasurer, disorganized meetings, and lack of transparency. Despite proposals to improve these issues, her recommendations were disagreed with. Due to being prevented from fulfilling her fiduciary obligations and an uncivil board culture, she felt she had no choice but to resign.

Connecting the Dots: Empowering Young Adults in Cedar RiversideJulia Eagles
This final capstone project was part of the Cedar-Humphrey Action for Neighborhood Collaborative Engagement (CHANCE) community-based research class, looking at educational and workforce development opportunities for young adults (ages 18-24) in the Cedar Riverside neighborhood. Research included a literature review, semi-structured interviews & focus groups with community members, and demographic & geospatial analysis. This paper includes key findings and recommendations for our clients.
1. Matthew Hamilton is the Youth Services Manager at Kraft Youth Services, which provides comprehensive services to approximately 325 at-risk youth and 100 families per day through partnerships with local schools and mix of public and private funding.
2. The organization uses a risk-based model to deliver residential, education, and community services to youth, and evaluates youth needs and progress through a self-sufficiency continuum.
3. By developing youth entrepreneurship and leadership skills, the organization aims to empower youth to take control over the services and eventually transition the youth into driving the services themselves.
The document discusses perspectives on the SEN reforms from various viewpoints including a local authority, a provider of services for those with autism, and service users. The Nashville Area MPO has several projects related to health and transportation. These include but are not limited to: prioritizing bicycle and pedestrian projects according to areas with populations with highest amounts of health disparities, conducting a Health Impact Assessment of a Transit Oriented Development Project, providing Safe Routes to School bicycle and pedestrian education training to P.E. teachers throughout the MPO area, and conducting a study with Vanderbilt University on energy expenditure by mode of transportation.
The Tennessee Department of Transportation will discuss state level efforts to link transportation and health including adopting language addressing the safety of bicyclists and pedestrians in the State Strategic Highway Safety Plan, adding language and a test question on non-motorized modes to the state Drivers’ License Manual and exam, adopting a rumble stripe policy that addresses the comfort and safety of bicyclists, and training law officers across the state on bicycle and pedestrian laws.
The MPO and TDOT will also discuss efforts to include transportation into the statewide plan to address obesity in Tennessee.
Activity Agreement EOYR 2015 - Final versionJohn Cairns
Curtis has made excellent progress through his Activity Agreement in increasing his self-confidence and physical ability. He found that regular horse riding and care developed his skills and confidence. He completed the British Horse Society Horse Ownership course and Saltire Awards up to 100 hours. Curtis was able to re-engage with the Duke of Edinburgh Award at Silver level by utilizing his horse care experiences. He has become a confident public speaker and is considering careers in the police or armed forces.
Lisa Amidon's action plan aims to assist RTD in developing lifelong public transit riders by getting youth familiar with using public transportation. Her tactics include presenting her plan to RTD and Denver Public Schools to establish partnerships and sponsorships. She will set up initial meetings with RTD Director Larry Hoy and Pauletta Tonilas within 3 months to obtain buy-in. If successful, her plan would educate young riders, hold scavenger hunts to promote transit, and benefit riders, RTD, and Denver through developing future customers and promoting the city.

Senior Practitioner – Operational leads.Qualified Practitioner – Supports clients on their transition into adulthood through the SDS process. The number of qualified practitioners has increased to 12 from 2011 when there were only 4.Support Broker – Facilitates support planning where there might be particularly high support needs or there are communication difficulties.Carers Worker – Specific responsibility for supporting and advocating for carers.OT – Will be assessing equipment needs and supporting with behaviour and sensory needs, and independent living skills.
Transition Protocol will also explain the roles and responsibilities of the Transition Team and other professionals during this time.
Financial Assessment (Paying for adult community care services):If we provide you with community care services you might need to contribute towards the cost of your care package. The amount you will be asked to pay will depend on your income, savings and expenditure. It will also depend on the type of service you are receiving. Community Care includes home based care, Self-directed Support, major adaptations, and day care.You or your representative will be asked to complete a financial assessment form, so that we can work out how much, if any, you will have to pay. We will help you to complete the form and answer any questions you might have.The rules on charging for community care services can be complicated, so we have produced a leaflet which is called: Paying for Care - home and community services, to help you understand how we work out if you will pay.Our policy on charging for community services is based on guidance issued by the Department of Health. This is called: Fairer Charging Policy 2010.
Supported Self Assessment:This Supported Self Assessment will help us to understand and identify what needs the young person has so that we can work out how much support they need. Most of the questions help us do this, and will calculate the personal budget. Some questions will act as triggers to other more detailed assessments, as appropriate, for example OT assessments or Continuing Health Care Assessments.Supported Self Assessment is in two parts – The Young Person & The CarerFirst part is for the young person to tell us about their needs around personal care, daily living activities such as cooking, cleaning, shopping and managing money, leisure activities, keeping safe, making decisions etc. Second part looks at the impact on the Carer, and takes into account their lifestyle, health and well-being.Supported Self Assessment is about choice and control for young people and their families. It is person centred and needs led.
Resource Allocation & Indicative Budget:After completing Supported Self Assessment the young person will be informed of their Personal Indicative Budget Allocation.Once the young person knows how much their personal budget is, they can begin developing their support plan to meet their needs with people they choose for example; their family, a broker or transition worker.
Support Plan:An individual can use their Personal Budget in a way that best meets their needs, for example:To access community activitiesTo provide support at home or in the communityTo provide short breaksSupported LivingThe council wants the young person to be in control where possible. This depends on what they feel able to do, and what they want to do, their level of understanding and their wishes.
