Lorry Delivery Checklist
Step 1

Level Ground
Ang lorry ba ay nasa patag at “designated” na lugar at malayo
sa ano mang puwedeng pagmulan ng apoy? (siguruhin ito nang
sa ganoon ay maisagawa ng maayos at ligtas ang inspeksiyon)
Step 2

Lorry Safety
Ang lorry ba ay naka-parkbrake at mayroong kalso ang gulong
para maiwasan ang hindi inaasahang pag-galaw nito?
Step 3

Proper Unloading Position
Matapos ma-inspeksiyon, ang lorry ba ay nakaparada sa
ligtas na lugar, malapit sa pagdidiskargahan at nakaharap sa
Exit o labasan?
Step 4

Driver ID at PPE

Ang lorry driver ba ay may valid ID na galing sa Pilipinas Shell at nakasuot ng tamang uniform – cotton long sleeves at long pants, hard hat,
safety shoes, nitrille gloves?
Step 5

Authorized Receiver
Ang tatanggap ba ng delivery ay may valid I.D., “authorized”
ayon sa Competent Person Register o CDSS Card at naka-suot
ng tamang uniform – reflectorized vest, safety shoes, nitrile
gloves?
Step 6

Delivery
Note

Kumpleto ba ang mga dokumentong
dala ng lorry driver at nakasaad sa
Delivery note ang tamang pangalan
ng customer, numero ng customer,
address, produkto at quantity?
Step 7

Calibration
Certificate

Valid pa rin ba ang Calibration
Certificate ng lorry (hindi pa lampas ng
isang taon)?
Step 8

Valve Seals

Nasa ayos ba ang kundisyon ng mga selyo at magkakasunod ba
ang numero ng mga ito sa pang-itaas na takip (tank hatch cover) at
barbula ng tangke (discharge valves) sa numero na nakalagay sa
dokumentong dala ng lorry driver ?
Step 9

Lorry Fire Extinguisher

Nakababa na ba ang fire extinguisher at madaling makuha sa oras ng
emergency at naka-cordon ba ang discharge area gamit ang “reflectorized
cones/signs” at “cordon tapes”?
Step 10

Sufficient Ullage

Susukatin ni authorized receiver ang U.G.T.
kasama ang driver, upang masiguro ang
eksaktong laman ng UGT
Step 11

Smart Box

Kukunin ng driver ang susi sa smartbox para buksan ang
padlock sa discharge valve ng compartment na ididiskarga.
Step 12

Unloading Sequence, Tank Ullage, Hose Connection
Malinaw bang napagkasunduan ang planong pagkasunod-sunod na
pagdidiskarga ng mga produktong ide-”deliver”? (punan ang mga detalye
at “sequence” ng pag-diskarga sa lorry delivery checklist)

Gamit ang adaptor, padaluyin ang produkto para macheck ito ayon sa kulay

isang dulo ng product tag at ikakabit
nya ito kung saang fill-pipe dapat
ikabit ang lorry discharge hose upang
maka-siguro sa tamang tangke (UGT)

Hose Connection
Connect to fill-pipe first

Ikakabit

ng driver ang discharge hose sa fill-pipe na pinag
kabitan ng MISSION Z cable ng authorized receiver

Hose Connection

Connect to discharge valve next

Ikabit ang kabilang dulo ng discharge hose sa lorry,
buksan ang E.S.V. at gate valve

Hose Integrity


Wala bang maaaring pag-mulan ng leak ng
produkto ang discharge hoses at fittings?

Commence Product Unloading

Product Sampling from
Compartment 1 and 2 and so on…

sample gamit ang adaptor at tamang sampling bottle
na ayon sa compartment number

Sampling

Ang produkto ba sa bawat compartment ng lorry ay nakunan ng
sample at tama sa nakasulat sa delivery note?

Sample Retention

Ilagay ang mga bote sa sample box at lagyan ng tag

Observe Proper

Tank Emptiness

Wala na bang laman ang lahat ng lorry
compartments, lorry pipelines at discharge hose
pagkatapos magdiskarga?

DOUBLE CHECK
-Check MISSION Z Cable connections
-Perform “Finger Follow” Check
-Confirm by signing off proper entry at the LDC
Step 29

Level Ground

Siguruhin na ang lorry ay nasa patag na lugar
bago mag-drain ng produktong naiwan sa
compartment.
Step 30

Proper Grounding

Isa-ayos ang grounding cable sa pamamagitan ng pag-clip ng
kabilang dulo sa discharge valve at ihanda ang draining bucket bago
mag-umpisa.
Step 31

Open and Close Valves

Umpisahan ang pag-drain ng produktong naiwan sa compartment
(naka-masid ang customer’s receiver habang isinasagawa ito).
Siguruhing nai-sara ang discharge valve pagkatapos mag-drain.
Step 32

Remove Grounding

Alisin ang grounding cable mula sa pagkaka-kabit sa discharge
valve (siguruhing wala ng produktong naiwan bago isagawa ito).

Residual Product Turnover

I-turn over ang na-drain na produkto sa customer’s receiver
para ito ay mai-salin sa underground tank muli.

Do not take shortcuts when completing the LDC.
Do the Finger Follow!
MHD shall only be done in Retail sites with Remote Fill
Ensure to have different colored Mission Z cables and hoses.
Before proceeding to hose connection of the 2nd product,
ensure that there are no leaks or problems on the 1st product
being discharged.
• Only drivers TRAINED IN MULTI-HOSE DISCHARGING
shall be allowed to perform simultaneous discharging. Driver
ID will be updated after training (1 classroom session and 1
actual product discharging witnessed by Mentor Driver)

  • 6. Step 2 Lorry Safety Ang lorry ba ay naka-parkbrake at mayroong kalso ang gulong para maiwasan ang hindi inaasahang pag-galaw nito?
  • 7. Step 3 Proper Unloading Position Matapos ma-inspeksiyon, ang lorry ba ay nakaparada sa ligtas na lugar, malapit sa pagdidiskargahan at nakaharap sa Exit o labasan?
  • 8. Step 4 Driver ID at PPE Ang lorry driver ba ay may valid ID na galing sa Pilipinas Shell at nakasuot ng tamang uniform – cotton long sleeves at long pants, hard hat, safety shoes, nitrille gloves?
  • 9. Step 5 Authorized Receiver Ang tatanggap ba ng delivery ay may valid I.D., “authorized” ayon sa Competent Person Register o CDSS Card at naka-suot ng tamang uniform – reflectorized vest, safety shoes, nitrile gloves?
  • 10. Step 6 Delivery Note Kumpleto ba ang mga dokumentong dala ng lorry driver at nakasaad sa Delivery note ang tamang pangalan ng customer, numero ng customer, address, produkto at quantity?
  • 11. Step 7 Calibration Certificate Valid pa rin ba ang Calibration Certificate ng lorry (hindi pa lampas ng isang taon)?
  • 12. Step 8 Valve Seals Nasa ayos ba ang kundisyon ng mga selyo at magkakasunod ba ang numero ng mga ito sa pang-itaas na takip (tank hatch cover) at barbula ng tangke (discharge valves) sa numero na nakalagay sa dokumentong dala ng lorry driver ?
  • 13. Step 9 Lorry Fire Extinguisher Nakababa na ba ang fire extinguisher at madaling makuha sa oras ng emergency at naka-cordon ba ang discharge area gamit ang “reflectorized cones/signs” at “cordon tapes”?
  • 14. Step 10 Sufficient Ullage Susukatin ni authorized receiver ang U.G.T. kasama ang driver, upang masiguro ang eksaktong laman ng UGT

  • 49. Big Rules • • • • • Do not take shortcuts when completing the LDC. Do the Finger Follow! MHD shall only be done in Retail sites with Remote Fill Ensure to have different colored Mission Z cables and hoses. Before proceeding to hose connection of the 2nd product, ensure that there are no leaks or problems on the 1st product being discharged. • Only drivers TRAINED IN MULTI-HOSE DISCHARGING shall be allowed to perform simultaneous discharging. Driver ID will be updated after training (1 classroom session and 1 actual product discharging witnessed by Mentor Driver)

  4. Attach the hose adaptor to lorry discharge valve and ensure that there is a dust cap.
  5. Use the adaptor in order to check the color of the product.
  6. Authorized receiver should ensure/identify correct fill-point before placing each lorry discharge hose
  7. The driver will connect the discharge hose to fill-pipe where Mission Z cable was placed by the authorized receiver.
  8. Connect the end part of the discharge hose to lorry then open the E.S.V and gate valve.
  9. Start unloading the product.
  10. While discharging, get a retaining sample using adaptor and place it to a proper sampling bottle in accordance with compartment number/label.
  11. Properly tagged the bottles and place it in sample box.
  12. After attaining sample from each compartment, labeled bottled should be sealed in the sample box with the sealing wire provided with the invoice.
