Introduction: artificial insemination
• Artificial insemination with homologous (AIH) or donor semen (AID) is
nowadays a very popular
• A procedure used for many sub fertile women worldwide
• The rationale is to increase gamete density at the site of fertilization
• Traces back to scientific studies and experimentation many centuries
ago

The renewed interest in artificial insemination is
associated with the refinement of techniques for
the preparation of washed motile spermatozoa in
the early years of IVF
Introduction: artificial insemination
• Originally developed to help couples to conceive in case of severe male factor
subfertility of a physical or psychological nature
• Nowadays artificial insemination with homologous semen is most commonly used for
unexplained and mild male factor subfertility
• In the previous century donor insemination was mainly used for male infertility due
to
• Azoospermia
• Very low sperm count
• For inherited genetic diseases linked to the Y-chromosome
• Nowadays donor insemination is more commonly used in women with no male partner

Despite the extensive literature on the
subject, controversy remains about the
effectiveness of this procedure, particularly
in relation to IVF and ICSI

The real start of a new era in assisted
reproduction was with first reports on human
artificial insemination originating from
Guttmacher (1943), Stoughton (1948) and
Kohlberg (1953)
Other important research discoveries in animal
studies
• Phillips and Lardy (1939) were the first to use egg yolk to protect bull sperm
cells from temperature shock upon cooling
• Salisbury et al. (1941) improved the media by using egg yolk with sodium
citrate, permitting the use of semen at 5° C for up to three days
• Polge and co-workers (1949) were the first to freeze fowl and bull
spermatozoa by using glycerol in the extender media
• In 1953 Dr. Jerome K. Sherman, introduced a simple method of preserving
human sperm using glycerol. He combined this with a slow cooling of sperm,
and storage with solid carbon dioxide as a refrigerant
The IVF revolution
Renewed interest in artificial insemination in human
• The main reason was the introduction of IVF in 1978 by Steptoe
and Edwards
• In the early days the ejaculate of the husband was inseminated
intrauterine without preparation resulting in uterine cramps and
increasing the probability of tubal infections
• With the arrival of IVF, semen preparation techniques were
developed and IUI regained its popularity, being more safe and
painless

IVF: Sperm washing
• Necessary to remove prostaglandins, infectious agents and
antigenic proteins.
• Another advantage is the removal of nonmotile spermatozoa,
either leucocytes or immature germ cells
• This may be an important factor in enhancing sperm quality
by a decreased release of lymphokines and/or cytokines and
a reduction in the formation of free oxygen radicals after
sperm preparation

• The swim-up procedure

• The mini-Percoll (small volume) gradient technique
• The use of Sephadex columns
Infertility care: the present value of AIH
• Artificial insemination became very popular as a first line treatment
procedure in case of unexplained and mild male factor infertility
• At this moment AIH is probably one of the most applied assisted
reproductive techniques worldwide
• Nevertheless, there is still an on-going debate whether or not AIH is
an effective treatment option for various indications
• Controversy remains about its effectiveness, particularly in relation to
IVF and ICSI

You may be offered IUI if:
• you and your partner are unable (or find it very difficult) to have sexual intercourse, for
example because of a physical disability
• you have a condition (such as a viral infection that can be sexually transmitted) that
means you need specific help to conceive – for example, if you are having sperm washing
before conception can take place
• you are in a same-sex relationship and have not become pregnant after 6 cycles of
artificial insemination.
IUI: Clinical evidence
Recent studies, including a large prospective randomized multicentre
study, have shown that AIH remains a useful and cost-effective first-
line treatment in case of mild male infertility and unexplained infertility
with an unfavourable prognosis as compared to the more aggressive
techniques such as IVF and ICSI (Van Rumste et al., 2014; Bensdorp et
al., 2015; Moolenaar et al., 2015)
• The ongoing pregnancy rates were comparable: 24% with IVF-eSET versus
21% with IUI-ovarian stimulation. There were two multiple pregnancies
with IVF-eSET and three multiple pregnancies with IUI-ovarian
stimulation.
• We conclude that IUI-ovarian stimulation is the preferred treatment to
start with. When IVF-eSET results in a higher ongoing pregnancy rate
(>38%), IVF would be the preferred treatment.
The choice of IVF over IUI with ovarian stimulation and ICSI over
IVF depends on the willingness to pay for an extra live birth. If
only cost per live birth is considered for each treatment, above a
pre-wash TMSC of 3 million, IUI is less costly than IVF and, below
a pre-wash, TMSC of 3 million ICSI is less costly. Effectiveness
needs to be confirmed in a large randomized controlled trial.
Moral and social implications of artificial insemination
• Debated in both the medical and popular press in the United
States since 1909
• The Catholic Church objected to all forms of artificial
insemination
• The main criticism was that artificial insemination with donor
semen was a form of adultery promoting the vice of masturbation
• Other critics were concerned that AID could encourage eugenic
government policies

Demand for donor sperm increased tremendously
• After the first successful pregnancy from frozen sperm, a
thriving sperm-bank industry developed
• The commercialization of AID became unavoidable
• The growing number of AID‘s raised new concerns leading to new
regulations
• Because of the possible transmission of sexually transmitted
diseases, including HIV, when using fresh sperm screening for
infections of donors became mandatory

Sociocultural and ethical concerns
• Biological paternity and the maintenance of the
heterosexual, married couple as the basis of the family
remain important in many countries
• A lot of countries all over the world have not approved the
use of AI with donor semen for single women and lesbian
couples yet
• Another point of debate is whether the donor has to be
anonymous or non-anonymous
Sociocultural and ethical concerns
• When to inform and what to tell AID children about their
biological parentage, if non-anonymous donors are used
• Is it possible and/or advisable to use sperm of relatives,
such as brothers or the father?
• Whether or not to pay the donors and sexing of sperm by
DNA quantification using flow cytometry instrumentation
became a point of discussion.
The historical story of artificial
insemination is a successful one
The use of frozen/thawed donor samples and
the renewed interest in sperm washing were
the most important milestones in the history
of human artificial insemination.
Intrauterine insemination with husband’s sperm
turned out to be a valuable first choice
treatment before starting more invasive and
more expensive techniques of assisted
reproduction for many sub fertile patients.
The increasing demand of single women for AI with
donor semen is another challenge in many countries
worldwide. Many debates, socio-cultural and
ethical, are to be expected in the near future.
