the escalating challenge of unhealthy aging. As our
population ages at an unprecedented rate, societies around
the globe find themselves at a tipping point. The
implications of this growing problem encompass not only
social and economic aspects but also profound healthcare,
marketing, financial, and workforce considerations.
Having immersed himself in these dynamic issues for
nearly fifty years, Ken Dychtwald, Ph.D., has emerged as
North America's foremost visionary and original thinker
regarding the social, economic, healthcare, marketing,
financial, and workforce implications of the "age wave"—a
term he coined in the 1980s.
In this exclusive article for Insights Care Magazine, Ken
Dychtwald shares his provocative views about how to heal

Birth of a Visionary
Ken is a multidimensional personality. In addition to being
the Founder and CEO of Age Wave, he is a psychologist,
gerontologist, documentary filmmaker, and best-selling
author of 19 books. Some of his noteworthy works include
'What Retirees Want: A Holistic View of Life's Third Age,'
'Radical Curiosity: My Life on the Age Wave' and bestseller,
'Age Wave: The Challenges and Opportunities of an Aging
America.' With an impressive client list that encompasses
more than half the Fortune 500, Ken has established himself
as one of the most sought-after experts in the fields of aging
and longevity and their related domains.
Journey of Revolutionizing Healthcare
In the late 1960s, as mass media and mind-expanding
education began, Ken's journey in transforming healthcare
also began with the anti-authoritarian attitudes of his
generation—the Baby Boomers who were being taught to
question authority. What emerged was an anti-authoritarian
mood; a rebellion against militarism, materialism, gas-
guzzling cars, the sexual mores of our parents' and
grandparents' generations, and even medicine. Women (led
in the late 1960s by the Boston Women's Health Collective)
and then men, began to believe that people should have
knowledge about and control of their own bodies.
Hoping to make a positive impact on this, a group of
people—including Gay Luce Ph.D., Eugenia Gerrard M.A.,
Ken Pelletier Ph.D., M.D., Len Duhl M.D., and Ken—came
together in the early 1970s in Berkeley, California and
formed a study council to create a new mashup of
mind/body medicine. They called this holistic health. Ken
was 24, completing his doctorate on the 'Psychology of the
Body' and writing his first book 'Bodymind'. Around the
same time, a young California physician named John
Travis, who trained at Tufts and Johns Hopkins, found
himself inspired by psychologist Abraham Maslow's
innovative suggestion that one didn't have to be mentally ill
to focus on being more mentally well. Travis applied that
notion to medicine and popularized the word wellness.
In subsequent years, a flow of inventive and high-minded
people worked to pursue a new health paradigm. Dr. Dean
Ornish followed up on Pritikin's diet with a more
comprehensive program of nutrition, yoga, meditation and
supportive group co-counseling sessions. Dr. Andy Weil
conjured integrative medicine, Dr. Deepak Chopra
attempted to blend an east/west approach and Mark Hyman
was crafting functional medicine.
Now, in the far more entrepreneurial, capitalistic
era—driven by the aging of the Boomers—there is a new
spectrum of skin preparations, hormone therapies, vitamins,
bio-hacks, prepared meals, pillows, mantras, brain hacks,
self-help apps, and exercise technology, along with books
and seminars.
Ken says, "Over the past 50 years, I have heard and seen a
thousand different experts make the case that they've got the
secret sauce or treatment that can cure almost everything."
The "age wave" has emerged as a powerful force shaping
society, the economy, healthcare, and various other sectors
around the globe. As the Founder and CEO of Age Wave,
Ken has dedicated his career to understanding and
addressing the challenges and opportunities presented by
the aging population.
Coined by Ken in the 1980s, the term, "age wave", refers to
the unprecedented demographic shift caused by the aging of
the Baby Boomer generation and its subsequent impact on
different aspects of life. In this context, Ken stands as a
visionary and original thinker, offering profound insights
into the social, economic, healthcare, marketing, financial,
and workforce implications of the "age wave".

Health, Science and Sociology: An Evolving
Relationship
Over the years, healthcare has made significant strides,
leading to a decline to the acute infectious diseases and a
rise in life expectancy. Until relatively recently, most people
died swiftly and relatively young of infectious diseases,
accidents, or childbirth. Twentieth-century breakthroughs in
healthcare aided in eliminating many of those threats. The
death rate from tuberculosis—one of the leading causes of
premature death a century ago—has been reduced by more
than 99 percent. Similarly, once-fatal illnesses such as
measles, streptococcal infections, pneumonia, influenza,
whooping cough, syphilis, typhoid, and diphtheria, are no
longer major epidemic diseases.
When the first U.S. Census was taken in 1790, half the
population was under the age of 16 and less than two
percent were 65 and older. As a result, society was not
concerned with the health needs of its aging citizens.
During the past century, advances in public health and
sanitation, medical diagnostics, pharmaceuticals, surgical
techniques, and nutrition have eliminated many acute
problems or infectious diseases that once caused premature
death.
However, the focus has been on prolonging lifespan rather
than healthspan. Ken emphasizes the need to align health
span with lifespan, as the later years should not be
th
dominated by illness and suffering. He shares, "In the 20
century, average life expectancy increased by 29 years, but
for many older adults, the later years are a time of illness,
pain, disability and suffering. Today, the average American
will spend 12.4 years (16 percent of their lifespan) living
with a disability or disease."
According to the World Health Organization, 58 countries
can boast a higher life expectancy than the U.S., ranging
from Switzerland to Japan, New Zealand to Peru and
Hungary to China.
The relatively mediocre rating is not because of inadequate
funding. Each year, the U.S. devotes an increasing
percentage of its resources to health care. In 1960,
healthcare expenditures were five percent of the gross
domestic product (GDP). By 2020, healthcare spending in
the U.S. was estimated at 20 percent of the GDP.
As the Baby Boomers age and the demographic landscape
changes, the intersection of health, science and sociology
becomes increasingly crucial in addressing the challenges
posed by unhealthy aging.
At the Tipping Point: Addressing Unhealthy Aging
Ken firmly believes that the world has reached a tipping
point, necessitating bold action to address unhealthy aging.
In his book 'Radical Curiosity: My Life on the Age Wave',
he presents a five-part solution to create healthy longevity
for everyone, ensuring a better quality of life in the later
years.
Ken says, "We need to create a new healthy longevity vision
and infrastructure. Below is my recommended five-part
solution for creating a scientific grand concerto to create
healthy longevity for everyone."
Solution 1: Gerontology and Geriatrics Expertise
To cater to the aging population's unique needs, Ken
advocates for recruiting and training healthcare
professionals with expertise in gerontology and geriatrics.
Despite the demographic shift, the number of geriatric
specialists remains significantly low. By prioritizing and
incentivizing these fields, society can better care for its
older adults, reducing costs and improving overall health
outcomes.
Ken shares, "Medicare is the largest funder of medical
residency training in the U.S. Yet only 315 medical
residents (out of around 148,000 medical residencies that
Medicare helped to support) specialized in geriatrics
during the academic year 2021-2022. This statistic is
insane – almost criminal."
He also emphasizes improvement in care, "If we required
physicians, nurses and other health professionals to attain
basic geriatric competencies in order to be eligible for
reimbursement, geriatric skills would swiftly improve,
mistakes and do-overs would shrink; and we'd have better-
cared-for elders at a far lower cost."

Solution 2: Harnessing the Power of Medicine,
Technology and AI
Ken says, "Since there might be thousands of biomarkers to
measure and thousands of ways to improve one's health,
how can every individual doctor know which approach or
combination of approaches will get the best results? It's not
humanly impossible."
Exponential advances in artificial intelligence and machine
learning offer a promising path to precision medicine. By
leveraging AI and extensive data on individual biomarkers
and outcomes research, a personalized approach to health
management can be developed, improving the overall
effectiveness of medical treatments and interventions.
He shares, "Utilizing AI and possibly ChatGPT, it will
become possible to make sense of thousands—even
millions—of individual biomarkers and then, like a kind of
Health Waze, provide direction to every individual so that
they can live better longer."
Solution 3: Prioritizing Prevention and Wellness
One seldom-discussed problem with our healthcare system
is that many men and women take poor care of themselves
on the way to maturity and old age. Unhealthy behaviors
are not unique to the elderly, but the older we get, the more
problematic they are. Lack of proper self-care hurts our
elders, their families and the economy.
Talking about the facts, Ken shares, "According to the
CDC, only 15 percent of men and 11 percent of women age
65 and over meet the physical activity guidelines for
aerobic and muscle-strengthening activities, approximately
two-thirds of those age 65 and older are overweight or
obese and more than eight percent of the elderly still
smoke."
Creating age-friendly living
environments and communities is
essential to support healthy aging.

housing, transportation and urban
planning to cater to the needs of
older adults.
Prevention and wellness play a pivotal role in promoting
healthy longevity. Ken emphasizes the need to shift the
focus from reactive healthcare to proactive measures that
prevent diseases and promote well-being. This includes
implementing policies and programs that prioritize
preventive screenings, lifestyle modifications and health
education from an early age. Regular exercise, proper
nutrition, maintaining a healthy body weight, not smoking
and limiting alcohol intake can improve lifespan by an
estimated 12-14 years.
Maintaining a healthy lifestyle can reduce heart disease,
hypertension, non-insulin-dependent diabetes mellitus,
colon cancer and osteoporotic fractures—the most common
diseases of aging. Healthy behaviors have also been shown
to increase bone mass, mineral content and lean muscle
mass; improve metabolic rate, balance, coordination,
strength, elimination efficiency, and heart/stroke volume
fostering a sense of well-being.
By investing in prevention and wellness, society can reduce
the burden of chronic diseases and enhance the overall
health of individuals as they age.
Solution 4: Redesigning Living Environments and
Communities
Creating age-friendly living environments and communities
is essential to support healthy aging. Ken advocates for
reimagining housing, transportation and urban planning to
cater to the needs of older adults. This includes designing
accessible and inclusive housing options, promoting
walkability, ensuring transportation options for older adults
and fostering intergenerational social connections and
community engagement. By creating environments that
enable independence, social interaction and access to
necessary services, older adults can maintain their well-
being and lead fulfilling lives.
Solution 5: Develop a Humane Approach to the End of
Life

Until the 20 century, death was no more strongly
associated with old age than with any other time of life. In
fact, one of the riskiest periods was infancy and early
childhood. Dying was an integral part of everyday
experience. Adults and children alike observed it regularly-
animals killed for food and family members dying not only
of old age but also of disease and accidents. Death, like
birth, mostly occurred at home. The deathbed scene was a
communal experience shared by family and friends.
When the end of life is near, older adults should be helped
to whatever extent possible to have a "good" death. This
includes enacting advance directives to ensure their final
wishes are respected, having access to spiritual and
emotional support, taking steps to minimize discomfort or
suffering and dying with dignity. Hospitals, health systems
and home care programs can help with this by offering
education for older adults and their families on advance
directives, palliative and hospice care programs and
education for caregivers on end-of-life options.
Palliative care needs to be the emphasis for the dying
patient—relieving symptoms, controlling pain and
providing emotional and spiritual support for the patient
and their family. Such treatment requires relatively little
apparatus and technology and is far less costly than the
procedures currently in place in most hospitals. Ken says,
"If we allowed elderly patients the dignity of dying a 'good'
and natural death at home supported by family and capable
hospice and palliative professionals, we could save both
human suffering and tens of billions of dollars each year."
Embracing a Brighter Future
Ken's five-part solution presents a comprehensive
framework for addressing the challenges of unhealthy aging
and creating a future of healthy longevity. By incorporating
gerontology expertise, harnessing the power of medicine
and technology, prioritizing prevention and wellness,
redesigning living environments, rethinking work and
retirement, society can embrace the "age wave" as an
opportunity for growth, innovation and improved well-
being for people of all ages. With concerted efforts and
visionary thinking, we can build a future where individuals
can age healthily, maintain independence and contribute to
the betterment of society.

A combination of strategic thinking and collaboration, with
a strong commitment to improving healthcare, Shai
Policker has worked within the medical startup ecosystem
in the US, EU, and Israel for more than 20 years.
Over the years, Shai has worked with various hospitals and
medtech manufacturers around the world, providing
solutions to an ever-growing stream of clinical problems.
With such a diverse background, we at Insights Care
interviewed Shai to know about his journey so far.
Below are the highlights of the interview:
Brief us about yourself and shed some light on your
journey so far. What inspires you to serve the healthcare
industry
My background is in electrical engineering (Technion,
Israel) and business (Columbia University, NY) and I have
been working within the medtech startup ecosystem in the
US, EU and Israel for more than 20 years. From a young
age, I was always intrigued by the complexity and fine
balance of the human body and as early as my initial
engineering studies, I was excited to create new solutions to
solve therapeutic and diagnostic problems.
Can you elaborate upon the core values, vision and
mission based on which your services are aligned with
MEDX Xelerator?
MEDX Xelerator advances excellent teams who grow
companies that solve a clinical unmet need and this fits very
much with what I like to do. One of the fun elements of this
industry is that you get to work with a very diverse group of
people from many backgrounds, ages and experiences, so
the ability to make an impact on the lives of different types
of patients and entrepreneurs plays an important part in
driving us at the incubator.
What areas in your opinion, does the health system
struggle with, when it comes to caring for/treating
patients?
The two main problems we are facing today are lack of
capacity and cost. Both are manifested by limited access to
some populations. The challenge is to add innovation that
will enable better care while reducing the burden on
healthcare personnel as well as lower costs for payers and
providers.

Please highlight your roles and responsibilities at
MEDX Xelerator
I am the CEO of the incubator, and, as other members of the
incubator team, I wear two hats: one is the hat of an
investor looking for the best solutions for clinical unmet
needs and the other is operational – supporting our portfolio
companies and making sure they reach their milestones. It's
a diverse role in a very dynamic environment and is very
rewarding.
What endeavors are you currently pursuing to improve
the care/treatment measures for your patients?
We are working on various ways to improve care through
our portfolio. Some examples:
Ÿ A device that can perform a rapid physical exam of
Dialysis patients without touching them. This saves time
at the clinic and improves outcomes. (PatenSee)
Ÿ A home-use wearable device to help COPD patients
clear their lungs providing patient-tailored, chest
physiotherapy. (Synchrony Medical)
Ÿ A system for fighting infection following total knee
replacement. (Dimoveo)
What are some of the challenges you face when
conducting projects and how do you turn them into
opportunities for growth?
One of the main challenges is to grow the initial core team
of founders into a fully-staffed company, without losing the
fighting spirit and short-range communication. This is a
challenge to which we pay special attention.
Another important challenge is of course funding, which is
always a major issue for any early stage medtech project.
That said, I am happy that the efforts along the years have
created a nice network which we are able to regularly reach
out to and has helped generate some creative funding
models that we are using until today.
Share with us some of the strategies that have helped
you in your position as a leader. How do you keep up
with specific responsibilities at MEDX Xelerator?
What helped me the most is the amazing group of people
that I work with. My two partners at the leadership of
MEDX Xelerator alleviate much of the potential loneliness
of facing the occasional stressful situation. My advice to
any leader out there is not to compromise on the people you
work with. It pays off big time. Another relevant strategy is
diversity in talents, backgrounds and experience. This is a
known subject but in medtech this is especially important –
the problems are so multi-disciplinary that the only way to
solve them effectively is with a team that comes from
diverse backgrounds.
What advice would you like to give to budding
entrepreneurs and enthusiasts who desire to venture
into the healthcare sector?
As a "recovering engineer" I used to think that technology
is what counts the most, where in fact, product-market fit is
so much more important. So my advice is to always start
from the clinical unmet need and willingness to pay and
make sure you understand those very well before starting to
work on a solution.
How do you envision scaling your services and
operations in 2023 and beyond?
We are currently raising our next fund which will allow us
to expand our model and continue to invest in later stage
companies while keeping our incubation "engine" running.
This is our main goal for the coming year.
What also helps us scale is the unique collaborations we
have created over the years with hospitals and medtech
manufacturers around the world that provide an ever
growing stream of clinical problems for us to address.

Having a system in place with all the necessary information
and analyzed data can help doctors come up with improved
solutions. Having chunks of data is not helpful until it is
analyzed properly. It is important that data is analyzed so
that redundant and irrelevant can be removed or deleted
from the system.

the life of people with spinal cord injuries.
This cutting-edge technology requires expertise in several
specialities and Stéphane certainly helps with his role as a
President and Chief Medical Officer at NeuroBioMat.
Appreciating his contribution, we at Insights Care
interviewed Stéphane to know the workings of this
groundbreaking technology.
Below are the highlights of the interview:
Brief us about yourself and shed some light on your
journey so far. What inspires you to serve the healthcare
industry?
My career that spans more than 30 years has focused
primarily on the design and evaluation of biomaterials to
repair lesions of the nervous system. After a doctorate in
medicine from the Louis Pasteur University of Strasbourg
(France), I pursued a research career via a PhD in
Neuroscience at Laval University (Canada). In order to gain
expertise in biomaterials, particularly polymer-based
hydrogels, I went to the Institute of Macromolecular
Chemistry in Prague for a year. I had the chance to be at the
forefront of macromolecular chemistry, because it was in
this institute that contact lenses were invented in 1961.
At that time it was a huge challenge to bring together the
field of advanced polymeric biomaterials with the biology
of spinal cord repair. My pioneering work based on a
conceptual breakthrough, has engaged a new technology in
the field of tissue neuroengineering using hydrogel-based
scaffolds.
I have held a variety of roles in both academics as a
professor of biomaterials at the Quebec Biomaterial
Institute and in private settings as a manager when I
founded Organogel, a biotechnology company in Quebec in
Canada. Later, I co-founded NeuroBioMat which is
developing a polymer-based medical device to treat post-
traumatic spinal cord injuries. I wanted to translate
advances in research and development into medical devices
for the healthcare industry.
My inspiration was driven by the science of biomaterials.
As early as 1986, I saw the science of biomaterials as
having the potential to make a significant contribution to
the development of innovative treatments in organ repair.
This was the starting point of my motivation to serve the
healthcare industry specially in the field of neurotrauma. I
could say that my research has been a pioneering work that
has greatly contributed to a novel neurorestorative approach
to treat lesions of the spinal cord using polymer scaffolds.
Can you elaborate upon the core values, vision and
mission based on which your services are aligned with
NeuroBioMat?
Spinal cord injury (SCI) is a devastating neurological
disorder that affects 2.5 million people worldwide. The
disruption of motor and sensory nerve tracts results in
significant morbidity including loss of motor and sensory
function as well as respiratory deficiency, bladder and
bowel dysfunction. So, SCI affects almost every aspect of
the patient's life and represents a high level of unmet
therapeutic area along with high costs to payers.

What areas in your opinion, does the health system
struggle with when caring for/treating patients?
As the healthcare industry continues to evolve, the quality
of care for patients also improves. But contrary to other
disciplines of medicine, neurological surgery is "the poor
child of medicine" because this discipline deals with the
general cure by just eliminating the problem like brain
tumors but has no solutions to truly care for patients, e.g.
restoring some neurological functions. In the case of spinal
cord injury following trauma, there is a need to develop
effective treatments that require developing innovative
solutions and what NBM is working on for the benefit of
neurological spinal cord surgery. For this discipline, the
health industry must still make progress to provide more
targeted and effective treatments to its patients.
Please highlight your roles and responsibilities at
NeuroBioMat.
As CEO of NeuroBioMat, I have multiple responsibilities,
but the most important is to drive a clear vision for NBM
with the collaborators and to create enthusiasm to carry out
the mission of NBM until the end of its course. This is
accomplished by establishing overall direction and ensuring
a logistics strategy to ensure workflow is optimized
between key partners. Another job is building relationships
with medical professionals as well as physical medicine and
rehabilitation. I have also a key role in securing adequate
funding to ensure financial solidity and scalability of the
company. My concern is also to maintain solid protection of
intellectual property.
In what way do you consider NeuroBioMat's
technological advances to be a pathway in conducting its
operations effectively?
The concept of the hydrogel device overcomes many
obstacles in managing spinal cord injury in a way that
promotes healing and regeneration. Our technology allows
to rebuild the missing nerve connection at the level segment
of the lesioned spinal cord and our device is "ready to use".
It is a safer and more effective procedure compared for
example, of injection of stem cells. This concept meets the
needs of practitioners as it is a biomaterial on demand: it
promotes cell regeneration that results to the restoration of
the spinal cord, perfectly bio-compatible, easy to handle
and easy to store which is a great advantage compared to
techniques based on engraftment of cells. Moreover, the
surgical technique is relatively straightforward. And beyond
the advance that this technology represents for neurological
surgery, it is also an interesting economic opportunity with

mission is well aligned with its vision, as the task is to
change the lives of SCI people while the vision is to
become a leader in the SCI market. NBM's values reside in

to rebuild missing spinal nerve connections to allow nerve
impulses to once again travel to and from the brain along
the spinal cord. Our solution is based on a tissue
engineering approach, which promotes the self-healing
ability of the spinal cord after the introduction of a three-
dimensional porous scaffold at the injury site that provides
structural support for tissue remodeling and healing.
Afterwards, patients will follow a rehabilitation program to
promote neuroplasticity. For this project, which requires
expertise in several specialties, including macromolecular
chemistry, neurobiology, tissue engineering and
microsurgery, NBM has developed a professional network
of first-rate strategic partnerships around its cutting-edge
technology.

with specific responsibilities at NeuroBioMat?
The only strategy is to stay ahead of cutting-edge
technologies align with a robust intellectual property
protection.
What advice would you like to give to budding
entrepreneurs and enthusiasts who desire to venture
into the healthcare sector?
First of all to have the necessary background in medicine
and then to identify key unmet clinal need for which no
cleared alternative exist. Also knowing how to surround
yourself with the key technical skills for the project. Very
important, have some knowledge of the target market,
otherwise get some market research done and explores to
establish strong intellectual property protection. Finally, be
aware that it will be a long and very laborious journey so
have a spirit of success.
How do you envision scaling your services and
operations in 2023 and beyond?
At this stage, it is difficult to envision scaling before a
clinical trial validated. However, beyond this stage, our
economic model is based on the ramp-up of our service by
gradually opening new centres where the surgery will be
performed and by increasing the number of patients who
will be operated on in each centre. So, we can expect future
gradual market expansion, surgical technology with
improvements to provide more efficient and effective care
to patients, brand awareness, customer experience and
operational efficiency. This means developing new
partnerships with healthcare providers. Additionally, we
believe that our technology has the potential to benefit a
wide variety of therapeutic areas, and we will explore new
opportunities to expand.

is confirmed eligible for the FDA's Streamlined
Breakthrough Technology Registration Process.
What endeavors are you currently pursuing to improve
the care/treatment measures for your patients?
The endeavor I am currently pursuing is to set up a First-in-
Human (FIH) study with our SPINEREP product. This FIH
is based on robust preclinical results that represent a solid
scientific basis. It is the ultimate step in the development of
medical device with the goal to assess safety and efficacy
because this ensure the product/service to reach its full
value for the patient. My current effort now is to organize
the strategy to set up the logistics to properly conduct this
trial which includes many different aspects including data
monitoring plans and committees.
What are some of the challenges you face when
conducting projects and how do you turn them into
opportunities for growth
A significant challenge was to identify the right ISO 13485
certified contract manufacturing organization to
manufacture our polymer hydrogel device to deliver safe
and high-quality products. But the biggest challenge faced
is regulatory compliance in Europe and the US, as the
medical device regulatory landscape is complex, with
regulations specific from various regulatory bodies

Around the same time, a young California physician named John Travis, who trained at Tufts and Johns Hopkins, found himself inspired by psychologist Abraham Maslow's innovative suggestion that one didn't have to be mentally ill to focus on being more mentally well. Health, Science and Sociology: An Evolving Relationship Over the years, healthcare has made significant strides, leading to a decline to the acute infectious diseases and a rise in life expectancy. Until relatively recently, most people died swiftly and relatively young of infectious diseases, accidents, or childbirth. Twentieth-century breakthroughs in healthcare aided in eliminating many of those threats. The death rate from tuberculosis—one of the leading causes of premature death a century ago—has been reduced by more than 99 percent. Similarly, once-fatal illnesses such as measles, streptococcal infections, pneumonia, influenza, whooping cough, syphilis, typhoid, and diphtheria, are no longer major epidemic diseases. When the first U.S. Census was taken in 1790, half the population was under the age of 16 and less than two percent were 65 and older. As a result, society was not concerned with the health needs of its aging citizens. During the past century, advances in public health and sanitation, medical diagnostics, pharmaceuticals, surgical techniques, and nutrition have eliminated many acute problems or infectious diseases that once caused premature death. However, the focus has been on prolonging lifespan rather than healthspan. Ken emphasizes the need to align health span with lifespan, as the later years should not be th dominated by illness and suffering. He shares, "In the 20 century, average life expectancy increased by 29 years, but for many older adults, the later years are a time of illness, pain, disability and suffering. Today, the average American will spend 12.4 years (16 percent of their lifespan) living with a disability or disease." According to the World Health Organization, 58 countries can boast a higher life expectancy than the U.S., ranging from Switzerland to Japan, New Zealand to Peru and Hungary to China.
  • 14. The relatively mediocre rating is not because of inadequate funding. Each year, the U.S. devotes an increasing percentage of its resources to health care. In 1960, healthcare expenditures were five percent of the gross domestic product (GDP). By 2020, healthcare spending in the U.S. was estimated at 20 percent of the GDP. As the Baby Boomers age and the demographic landscape changes, the intersection of health, science and sociology becomes increasingly crucial in addressing the challenges posed by unhealthy aging. At the Tipping Point: Addressing Unhealthy Aging Ken firmly believes that the world has reached a tipping point, necessitating bold action to address unhealthy aging. In his book 'Radical Curiosity: My Life on the Age Wave', he presents a five-part solution to create healthy longevity for everyone, ensuring a better quality of life in the later years. Ken says, "We need to create a new healthy longevity vision and infrastructure. Below is my recommended five-part solution for creating a scientific grand concerto to create healthy longevity for everyone." Solution 1: Gerontology and Geriatrics Expertise To cater to the aging population's unique needs, Ken advocates for recruiting and training healthcare professionals with expertise in gerontology and geriatrics. Despite the demographic shift, the number of geriatric specialists remains significantly low. By prioritizing and incentivizing these fields, society can better care for its older adults, reducing costs and improving overall health outcomes. Ken shares, "Medicare is the largest funder of medical residency training in the U.S. Yet only 315 medical residents (out of around 148,000 medical residencies that Medicare helped to support) specialized in geriatrics during the academic year 2021-2022. This statistic is insane – almost criminal." He also emphasizes improvement in care, "If we required physicians, nurses and other health professionals to attain basic geriatric competencies in order to be eligible for reimbursement, geriatric skills would swiftly improve, mistakes and do-overs would shrink; and we'd have better- cared-for elders at a far lower cost."

Solution 2: Harnessing the Power of Medicine, Technology and AI Ken says, "Since there might be thousands of biomarkers to measure and thousands of ways to improve one's health, how can every individual doctor know which approach or combination of approaches will get the best results? It's not humanly impossible." Exponential advances in artificial intelligence and machine learning offer a promising path to precision medicine. By leveraging AI and extensive data on individual biomarkers and outcomes research, a personalized approach to health management can be developed, improving the overall effectiveness of medical treatments and interventions. He shares, "Utilizing AI and possibly ChatGPT, it will become possible to make sense of thousands—even millions—of individual biomarkers and then, like a kind of Health Waze, provide direction to every individual so that they can live better longer." Solution 3: Prioritizing Prevention and Wellness One seldom-discussed problem with our healthcare system is that many men and women take poor care of themselves on the way to maturity and old age. Unhealthy behaviors are not unique to the elderly, but the older we get, the more problematic they are. Lack of proper self-care hurts our elders, their families and the economy. Talking about the facts, Ken shares, "According to the CDC, only 15 percent of men and 11 percent of women age 65 and over meet the physical activity guidelines for aerobic and muscle-strengthening activities, approximately two-thirds of those age 65 and older are overweight or obese and more than eight percent of the elderly still smoke."

Ken advocates for reimagining housing, transportation and urban planning to cater to the needs of older adults. Prevention and wellness play a pivotal role in promoting healthy longevity. Ken emphasizes the need to shift the focus from reactive healthcare to proactive measures that prevent diseases and promote well-being. This includes implementing policies and programs that prioritize preventive screenings, lifestyle modifications and health education from an early age. Regular exercise, proper nutrition, maintaining a healthy body weight, not smoking and limiting alcohol intake can improve lifespan by an estimated 12-14 years. Maintaining a healthy lifestyle can reduce heart disease, hypertension, non-insulin-dependent diabetes mellitus, colon cancer and osteoporotic fractures—the most common diseases of aging. Healthy behaviors have also been shown to increase bone mass, mineral content and lean muscle mass; improve metabolic rate, balance, coordination, strength, elimination efficiency, and heart/stroke volume fostering a sense of well-being. By investing in prevention and wellness, society can reduce the burden of chronic diseases and enhance the overall health of individuals as they age. Solution 4: Redesigning Living Environments and Communities Creating age-friendly living environments and communities is essential to support healthy aging. Ken advocates for reimagining housing, transportation and urban planning to cater to the needs of older adults. This includes designing accessible and inclusive housing options, promoting walkability, ensuring transportation options for older adults and fostering intergenerational social connections and community engagement. By creating environments that enable independence, social interaction and access to necessary services, older adults can maintain their well- being and lead fulfilling lives.
  • 17. Solution 5: Develop a Humane Approach to the End of Life th Until the 20 century, death was no more strongly associated with old age than with any other time of life. In fact, one of the riskiest periods was infancy and early childhood. Dying was an integral part of everyday experience. Adults and children alike observed it regularly- animals killed for food and family members dying not only of old age but also of disease and accidents. Death, like birth, mostly occurred at home. The deathbed scene was a communal experience shared by family and friends. When the end of life is near, older adults should be helped to whatever extent possible to have a "good" death. This includes enacting advance directives to ensure their final wishes are respected, having access to spiritual and emotional support, taking steps to minimize discomfort or suffering and dying with dignity. Hospitals, health systems and home care programs can help with this by offering education for older adults and their families on advance directives, palliative and hospice care programs and education for caregivers on end-of-life options. Palliative care needs to be the emphasis for the dying patient—relieving symptoms, controlling pain and providing emotional and spiritual support for the patient and their family. Such treatment requires relatively little apparatus and technology and is far less costly than the procedures currently in place in most hospitals. Ken says, "If we allowed elderly patients the dignity of dying a 'good' and natural death at home supported by family and capable hospice and palliative professionals, we could save both human suffering and tens of billions of dollars each year." Embracing a Brighter Future Ken's five-part solution presents a comprehensive framework for addressing the challenges of unhealthy aging and creating a future of healthy longevity. Below are the highlights of the interview: Brief us about yourself and shed some light on your journey so far. What inspires you to serve the healthcare industry My background is in electrical engineering (Technion, Israel) and business (Columbia University, NY) and I have been working within the medtech startup ecosystem in the US, EU and Israel for more than 20 years. From a young age, I was always intrigued by the complexity and fine balance of the human body and as early as my initial engineering studies, I was excited to create new solutions to solve therapeutic and diagnostic problems. Can you elaborate upon the core values, vision and mission based on which your services are aligned with MEDX Xelerator? One of the fun elements of this industry is that you get to work with a very diverse group of people from many backgrounds, ages and experiences, so the ability to make an impact on the lives of different types of patients and entrepreneurs plays an important part in driving us at the incubator. What areas in your opinion, does the health system struggle with, when it comes to caring for/treating patients? The two main problems we are facing today are lack of capacity and cost. Both are manifested by limited access to some populations. The challenge is to add innovation that will enable better care while reducing the burden on healthcare personnel as well as lower costs for payers and providers. It's a diverse role in a very dynamic environment and is very rewarding. What endeavors are you currently pursuing to improve the care/treatment measures for your patients? We are working on various ways to improve care through our portfolio. Some examples: Ÿ A device that can perform a rapid physical exam of Dialysis patients without touching them. This saves time at the clinic and improves outcomes. (PatenSee) Ÿ A home-use wearable device to help COPD patients clear their lungs providing patient-tailored, chest physiotherapy. (Synchrony Medical) Ÿ A system for fighting infection following total knee replacement. (Dimoveo) What are some of the challenges you face when conducting projects and how do you turn them into opportunities for growth? One of the main challenges is to grow the initial core team of founders into a fully-staffed company, without losing the fighting spirit and short-range communication. This is a challenge to which we pay special attention. Another important challenge is of course funding, which is always a major issue for any early stage medtech project. That said, I am happy that the efforts along the years have created a nice network which we are able to regularly reach out to and has helped generate some creative funding models that we are using until today. Share with us some of the strategies that have helped you in your position as a leader. How do you keep up with specific responsibilities at MEDX Xelerator? What helped me the most is the amazing group of people that I work with. My two partners at the leadership of MEDX Xelerator alleviate much of the potential loneliness of facing the occasional stressful situation. My advice to any leader out there is not to compromise on the people you work with. It pays off big time. Another relevant strategy is diversity in talents, backgrounds and experience. This is a known subject but in medtech this is especially important – the problems are so multi-disciplinary that the only way to solve them effectively is with a team that comes from diverse backgrounds. What advice would you like to give to budding entrepreneurs and enthusiasts who desire to venture into the healthcare sector? As a "recovering engineer" I used to think that technology is what counts the most, where in fact, product-market fit is so much more important. So my advice is to always start from the clinical unmet need and willingness to pay and make sure you understand those very well before starting to work on a solution. Appreciating his contribution, we at Insights Care interviewed Stéphane to know the workings of this groundbreaking technology. Below are the highlights of the interview: Brief us about yourself and shed some light on your journey so far. What inspires you to serve the healthcare industry? My career that spans more than 30 years has focused primarily on the design and evaluation of biomaterials to repair lesions of the nervous system. After a doctorate in medicine from the Louis Pasteur University of Strasbourg (France), I pursued a research career via a PhD in Neuroscience at Laval University (Canada). In order to gain expertise in biomaterials, particularly polymer-based hydrogels, I went to the Institute of Macromolecular Chemistry in Prague for a year. I had the chance to be at the forefront of macromolecular chemistry, because it was in this institute that contact lenses were invented in 1961. At that time it was a huge challenge to bring together the field of advanced polymeric biomaterials with the biology of spinal cord repair. My pioneering work based on a conceptual breakthrough, has engaged a new technology in the field of tissue neuroengineering using hydrogel-based scaffolds. I have held a variety of roles in both academics as a professor of biomaterials at the Quebec Biomaterial Institute and in private settings as a manager when I founded Organogel, a biotechnology company in Quebec in Canada. Later, I co-founded NeuroBioMat which is developing a polymer-based medical device to treat post- traumatic spinal cord injuries. I wanted to translate advances in research and development into medical devices for the healthcare industry. My inspiration was driven by the science of biomaterials. As early as 1986, I saw the science of biomaterials as having the potential to make a significant contribution to the development of innovative treatments in organ repair. This was the starting point of my motivation to serve the healthcare industry specially in the field of neurotrauma. I could say that my research has been a pioneering work that has greatly contributed to a novel neurorestorative approach to treat lesions of the spinal cord using polymer scaffolds. Can you elaborate upon the core values, vision and mission based on which your services are aligned with NeuroBioMat? Spinal cord injury (SCI) is a devastating neurological disorder that affects 2.5 million people worldwide. The disruption of motor and sensory nerve tracts results in significant morbidity including loss of motor and sensory function as well as respiratory deficiency, bladder and bowel dysfunction. As CEO of NeuroBioMat, I have multiple responsibilities, but the most important is to drive a clear vision for NBM with the collaborators and to create enthusiasm to carry out the mission of NBM until the end of its course. This is accomplished by establishing overall direction and ensuring a logistics strategy to ensure workflow is optimized between key partners. Another job is building relationships with medical professionals as well as physical medicine and rehabilitation. I have also a key role in securing adequate funding to ensure financial solidity and scalability of the company. My concern is also to maintain solid protection of intellectual property. In what way do you consider NeuroBioMat's technological advances to be a pathway in conducting its operations effectively? The concept of the hydrogel device overcomes many obstacles in managing spinal cord injury in a way that promotes healing and regeneration. Our technology allows to rebuild the missing nerve connection at the level segment of the lesioned spinal cord and our device is "ready to use". It is a safer and more effective procedure compared for example, of injection of stem cells. This concept meets the needs of practitioners as it is a biomaterial on demand: it promotes cell regeneration that results to the restoration of the spinal cord, perfectly bio-compatible, easy to handle and easy to store which is a great advantage compared to techniques based on engraftment of cells. Moreover, the surgical technique is relatively straightforward. At this stage, it is difficult to envision scaling before a clinical trial validated. However, beyond this stage, our economic model is based on the ramp-up of our service by gradually opening new centres where the surgery will be performed and by increasing the number of patients who will be operated on in each centre. So, we can expect future gradual market expansion, surgical technology with improvements to provide more efficient and effective care to patients, brand awareness, customer experience and operational efficiency. This means developing new partnerships with healthcare providers. Additionally, we believe that our technology has the potential to benefit a wide variety of therapeutic areas, and we will explore new opportunities to expand. a faster time-to-market than other medical devices: in fact, given its specificity which targets patients, our technology is confirmed eligible for the FDA's Streamlined Breakthrough Technology Registration Process. What endeavors are you currently pursuing to improve the care/treatment measures for your patients? The endeavor I am currently pursuing is to set up a First-in- Human (FIH) study with our SPINEREP product. This FIH is based on robust preclinical results that represent a solid scientific basis. It is the ultimate step in the development of medical device with the goal to assess safety and efficacy because this ensure the product/service to reach its full value for the patient. My current effort now is to organize the strategy to set up the logistics to properly conduct this trial which includes many different aspects including data monitoring plans and committees. What are some of the challenges you face when conducting projects and how do you turn them into opportunities for growth A significant challenge was to identify the right ISO 13485 certified contract manufacturing organization to manufacture our polymer hydrogel device to deliver safe and high-quality products.
