MID-YEAR REPORT 
Memorial is required to provide 
progress reports on the programs we 
fund periodically during the year, from 
health system reporting to President 
reports and the IRS. 
Providing us with a mid-term project 
report allows us to communicate 
program status, keeps organizations 
accountable for the funds they were 
granted and helps to prep 
documentation for year-end reporting. 
As a bonus, it gives our partners a 
opportunity to showcase their success 
and ask for help with any problems or 
concerns they may have.

PROPOSAL 
MID-YEAR REPORT 
ANNUAL REPORT 
is an opportunity to present outcomes 
at the successful completion of the 
program. Analyzing success and 
shortfall helps to adjust program and 
project trajectory, focusing on not just 
the needs of the community, but also its 
capacity for change. 
Not every project is systemic; not all are 
designed to be. However, with 
measurement and outcome analysis 
every program can inform the greater 
goal of a healthier community. Positive 
or negative results are an important 
indicator of current community 
support, opportunity and limitations. 
These are vital pieces of the puzzle and 
this cumulative report will detail that.

Activities, Outputs and Outcomes 
delivered by community projects can 
have a wider impact. 
Community Health's Innovation Training 
will define and demonstrate the link 
between objectives, inputs; activities, 
outputs and outcomes within established 
and developing projects. And how these 
initial and projected steps can lead to 
true Impact. 
This Logic Chain template , developed 
by Memorial staff, explores - in detail – all 
the various aspects of the program’s 
stages, outlining a plan of measurement 
and setting up success from the initial 
stages of implementation.

BUDGET 
Obviously, the budget is a vitally 
important part of any organization’s 
proposal. Requesting funds is a great 
deal more difficult if the required 
amount is unknown. 
The budget should provide a clear 
overview of what everything is 
expected to cost and who/how that 
cost will be covered. 
Memorial has clear regulations on what 
we can provide funding for, as the 
template clearly indicates. However, 
we need to know that the costs we 
cannot cover will be provided by 
someone, or the program’s chance of 
success is sabotaged from the start.

Memorial understands that our 
partners’ time is limited. In an effort to 
alleviate some additional stress, we 
developed a standard invoice which 
can be used to request awarded 
funding at the agreed upon intervals. 
We require an invoice from the 
receiving organization to facilitate 
check release. This template includes 
all the information we require to 
process and release funds. 
Organizations are always welcome to 
utilize invoices of their own design 
provided it includes all relevant 
information.

Memorial provides this packet of

partners to help streamline processes 
and meet requirements we need to 
provide to both our board and 
executives not only to support the 
programs we fund, but also to provide 
accountability to the hospital and 
health system, and the federal 
government from which the mandates 
come.

  • 1. Creating community health is at the core of Memorial Hospital and Beacon Health System’s mission We believe promoting community health is the right thing to do, and is key to long-term cost-effectiveness. We believe improving the community’s health status is as much social, economic and environmental as it is a medical issue.
  • 2. Mission: To enhance the physical, mental and emotional well-being of the communities we serve as the community's provider of outstanding quality, superior value and comprehensive health care services. Vision: To be recognized as a model for the delivery of health and wellness by achieving the Top 10% in clinical quality, Top 10% in patient satisfaction and top tier in value per patient in the United States. Beacon Intent Statement: Building on the Health Systems' strengths of being community oriented, building deep ties in the community, and being recognized as a core hub by the community, our future promise is to: Reduce by 40% the number of preventable chronic disease cases in our community by 2021 and reduce treatment expenditures by 50%.

PROPOSAL Once a need is identified and a project or program is decided on, the proposal will help organizations organize their plan and supporting data into a format that allows easy reference for our Council and Executives when determining funding priorities. Based on the strength areas and helping to support any weaker aspects of the plan, feasibility and sustainability can be assessed and tithing awards determined.   • 6. PROPOSAL MID-YEAR REPORT Memorial is required to provide progress reports on the programs we fund periodically during the year, from health system reporting to President reports and the IRS. Providing us with a mid-term project report allows us to communicate program status, keeps organizations accountable for the funds they were granted and helps to prep documentation for year-end reporting. As a bonus, it gives our partners a opportunity to showcase their success and ask for help with any problems or concerns they may have. -- TEMPLATE --
  • 7. PROPOSAL MID-YEAR REPORT ANNUAL REPORT is an opportunity to present outcomes at the successful completion of the program. Analyzing success and shortfall helps to adjust program and project trajectory, focusing on not just the needs of the community, but also its capacity for change. Not every project is systemic; not all are designed to be. However, with measurement and outcome analysis every program can inform the greater goal of a healthier community. Positive or negative results are an important indicator of current community support, opportunity and limitations. These are vital pieces of the puzzle and this cumulative report will detail that. -- TEMPLATE --
  • 8. is a way to demonstrate how the Activities, Outputs and Outcomes delivered by community projects can have a wider impact. Community Health's Innovation Training will define and demonstrate the link between objectives, inputs; activities, outputs and outcomes within established and developing projects. And how these initial and projected steps can lead to true Impact. This Logic Chain template , developed by Memorial staff, explores - in detail – all the various aspects of the program’s stages, outlining a plan of measurement and setting up success from the initial stages of implementation.   • 9. BUDGET Obviously, the budget is a vitally important part of any organization’s proposal. Requesting funds is a great deal more difficult if the required amount is unknown. The budget should provide a clear overview of what everything is expected to cost and who/how that cost will be covered. Memorial has clear regulations on what we can provide funding for, as the template clearly indicates. However, we need to know that the costs we cannot cover will be provided by someone, or the program’s chance of success is sabotaged from the start. Budget template
  • 10. BUDGET INVOICE Memorial understands that our partners’ time is limited. In an effort to alleviate some additional stress, we developed a standard invoice which can be used to request awarded funding at the agreed upon intervals. We require an invoice from the receiving organization to facilitate check release. This template includes all the information we require to process and release funds. Organizations are always welcome to utilize invoices of their own design provided it includes all relevant information. Invoice template

IN CONCLUSION Memorial provides this packet of templates to our potential community partners to help streamline processes and meet requirements we need to provide to both our board and executives not only to support the programs we fund, but also to provide accountability to the hospital and health system, and the federal government from which the mandates come. Our status as a non-profit hospital is very important to our continuing mission with and in the community.
