  • #3 溝通效率不好,有些資料不方便作記錄,或是資料不易攜帶,造成溝通時常會有資訊不對等的狀況,回想一下是不是有去過大醫院但是不同醫師有不同說詞的經驗?當你有疑問,醫師會跟你說不同的醫師有不同的想法,但是事實上在醫院內部的工作人員在病人離開之後會趕快跑去跟別的醫師討論治療計劃,往往一個治療的療程需要許多醫療團隊的合作,但是中間常常會有溝通不良的狀況,有時候是因為資料不易攜帶,也許做過的紀錄是在一張紙條上,或是只是口頭上討論沒有去記錄,此時對於病患的治療就是一種拖延,治療的黃金期可能就這樣過了
  • #4 醫生在會診時需要印出大量紙本,即使現在有電子化病例,仍然會把電子病歷印出來貼在傳統病歷上,因為電子病歷必須在電腦上才能看,但醫護人員又必須有很高的機動性
