Right ventricular injury
The incidence of penetrating cardiac injuries appears to be rising, presumably because of an increase in civilian violence.Herein, a case of right ventricular wound associated with laceration of left internal mammary artery- LIMA- and brachial artery transaction is reported. The management of the case is outlined with literature review. The patient with penetrating cardiac injury should be taken to the operation room as quickly as possible. Rapid transport of patient to operating room and urgent surgery were essential to save this patient.

Abstract:
Pulmonary tuberculosis (PTB) is endemic in Iraq with its incidence progressively increasing under the influence of the sanction since 1990. Nevertheless, the diagnosis of PTB should not be made easily without the appropriate investigations. Otherwise, the correct diagnosis of important thoracic disorder (sometimes a serious one like malignancy) may be missed. In this study, six patients with different benign and malignant thoracic lesions chosen among many others are presented. All of them were misdiagnosed and five were treated as tuberculosis. The study reminds the clinician in TB endemic areas that TB can be simulated by many thoracic lesions.

Abstract:
Background and Objective: There are many nurse-led clinics now in UK, like Chest-Pain, Endoscopy and Bronchiectasis Clinics. Herein, we present the project of Nurse-led Chest Drain Clinic in Guy’s and St Thomas, s Hospital/ London. The aim is to analyze this change project.
Methods: A project of change was designed to set an outpatient clinic run by specialist clinical nurses for patients discharged home with an ambulatory chest drain system in situ. The project is observed and analyzed via interviews with the responsible nurses.
Results: The clinic was established in 2005, run by two Nurse Case Managers, 60 patients were seen in 2007 and the clinic remained well attended. Patients were happy to spend less time in hospital as they could stay safely with their families.
Conclusions: A safe at home management of long-term chest drains was provided by this nurse-led clinic.

Pulmonary resection in basrah: personal experience

Abstract:
Pulmonary resection is the operation that defines the thoracic surgeon. It represents the appropriate surgical treatment for many pulmonary lesions. This is the first study on pulmonary resection in Basrah, south of Iraq. The study is conducted in the Section of Thoracic and Cardiovascular Surgery in Basrah Teaching Hospital over a 5-year period (August 1996 to July 2001). The aim of the study is to present the personal experience of the author in lung resection, analyze the indications, surgical and anaesthetic management and outcome including morbidity and mortality in view of the literature. Thirty patients (17 males and 13 females) underwent pulmonary resection for different indications were retrospectively analyzed. The results of this study indicate that despite the small number of patients and the difficulties in anaesthetic management, pulmonary resection is practiced safely in Basrah, south of Iraq.

Transsternsl transpericardial closure of postpneumonectomy bronchopleural fis...
There is no standard treatment for post-pneumonectomy bronchopleural fistula and the successful management is a challenge to the thoracic surgeon. Most of the treatment options are staged procedures.Transsternal transpericardial closure (TSTP) is attractive as it is a one stage operation, that avoids the infected pneumonectomy space and does not result in patients disfigurement. The single disadvantage of TSTP closure is that it does not address the problem of the pneumonectomy space.Herein, we report a case of chronic BPF after pneumonectomy successfully closed via the transsternal transpericardial approach.The relevant literature is reviewed to throw light on the indications and the results of this operation.
Mediastinoscopy & mediastinotomy indications & techniques
The mediastinum is the central compartment of the chest. Its boundaries and compartments are well known.Although, it contains the most vital organs of the body; it is often a forgotten compartment. Involvement of mediastinal nodes has a dramatic prognostic and therapeutic impact in patients with non-small cell lung cancer. Cervical mediastinoscopy remains the most important technique for staging of the mediastinum.
The technique of extended mediastinoscopy and redo mediastinoscopy are described as well. Indications, technique and complications are discussed.
Abdominal vascular injuries
A lecture about the management approaches for abdominal vascular injuries. Injury to the major arteries and veins in the abdomen are technical challenge to the surgeon and are often fatal. All vessels are susceptible to injury with penetrating trauma. Vascular injuries in blunt trauma are far less common and usually involve the renal arteries and veins, though all other vessels, including the aorta, can be injured. Blunt trauma results from deceleration, AP compression or pelvic fractures.
Venous Disorders
It is estimated that 20% of American women and 7% of American men suffer from venous disease. Venous disease results in symptoms such as aching, fatigue, swelling, and pain in the legs which can interfere with daily living.Cosmetic issues may affect quality of life.
At least 20% of patients with venous disease will develop leg ulcers. This presentation outlines the normal anatomy and physiology of venous drainage of the extremities as well as the common venous disorders such as varicose veins and deep vein thrombosis.
Pulmonary hydatid cysts
The goal of surgical therapy in pulmonary hydatid disease is to remove the cyst while preserving as much lung tissue as possible. The surgical method may be different in the intact (simple) and ruptured (complicated) cysts. The operation has two steps: a) removal of the germinative layer, b) management of the residual pulmonary cavity. Simple cysts are generally removed after needle aspiration or enucleation without needle aspiration.
General principles of the operation The aim of surgery in pulmonary hydatid cyst is to remove the cyst completely while preserving the lung tissue as much as possible . Lung resection is performed only if there is an irreversible and disseminated pulmonary destruction. Careful manipulation of the cyst and adherence to the precaution to avoid the contamination of the operative field with the cyst content is the imperative part of the operation.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

Hematology/oncology fellows, attending faculty, and other health care professionals involved in the treatment of patients with mantle cell lymphoma (MCL).
