1. JOB SUMMARY/RESPONSIBILITIES:
• Implements, maintains and provides clinical pharmacist services including: clinical, educational, informational and distributional functions for
newborn, infant, pediatric, adolescent, adult and geriatric patients of The Queen’s Medical Center (QMC).
MINIMUM QUALIFICATIONS:
Education/Certification and Licensure:
• Current Hawaii State license as a Pharmacist.
• Doctor of Pharmacy degree preferred.
• Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists preferred.
Experience:
• Clinical hospital pharmacist experience preferred. Preferably with experience in rounding with a medical team, monitoring of patient drug
therapy, pharmacokinetic dosing and patient counseling and education.
• Experience in unit dose and IV services preferred.
• Ability to communicate professional expertise to all levels of the health care team.
• Experience operating blood gas analyzing machine and anti-coagulation instruments preferred.

• Fills routine orders for unit doses, prepacked pharmaceuticals, intravenous medications, floor stock and outpatient prescriptions and
provides technical and clerical assistance to pharmacists.
• May provide orientation and training in the technical functions and facilitate the technical work in the department. Serves as a resource
to staff technicians regarding procedures and other information pertaining to their technical functions and responsibilities.
MINIMUM QUALIFICATIONS:
