This document provides a rationale and instructions for using the PAU-eForms electronic record keeping system for student grades and performance. It explains that accurate record keeping is important for teachers, students, and administrators to track student progress and evaluate teaching effectiveness. The PAU-eForms were created based on the Department of Education's guidelines and use a simplified spreadsheet format to make them easy for teachers to use. Step-by-step instructions are provided for teachers to input student information, grades, and generate reports each grading period. Maintaining accurate and up-to-date student records in PAU-eForms helps schools fulfill legal requirements for documentation and allows for internal and external audits of teaching quality and student achievement.

2. RATIONALE
Record keeping is an important key role of a teacher and
the main aspect is to assist planning and set future learning
goals to the learner. Records may contain learner’s
information as well as teacher’s observation on the learner’s
performance. They need to continually reassess to meet
changing needs, e.g. personal situations may change and
they may need to be considered to assist the learner to
complete their studies/learnings. Records can also assist the
teacher or student to evaluate the teaching-learning
process, in other words, whether the teacher needs to
improve or change any aspects of their teaching method.
Good and accurate records are important for teachers,
learners, verifiers, training providers, and school heads as
they can keep a track on the individual’s development and
progress as well as in the teachers’ performance and
professionalism in delivering the subject. Additionally, it is a
legal requirement to keep accurate and up to date records
on learners as it serves as a clear channel for external, as
well as internal, audits. In other words, providing accurate

3. and up to date records is to prove and account for the
teaching effectiveness and the progress of learners or even
special needs for other learners.
The template was so-called Master e-Card but
eventually it was called PAU-eForms (Philippine
Assessment Unified electronic forms) because it uses to
summarize the result of the grades of all students in all their
subjects.
The PAU-eForms for Elementary and Junior High
School, were patterned on the official E-Class Records for
Junior High School released by the Department of Education
(DO 8, S. 2015 Policy Guidelines on Classroom
Assessment for the K to 12 Basic Education Program.).
To ensure sustainability and to minimize technical
difficulty, the designed templates were simplified using basic
features used in a spreadsheet Using spreadsheets is the
best way to keep track of student records for every quarter.
For example, for each class that you teach, you can use a

4. spreadsheet to track student grades for every test and
assignment.
User manual is provided to guide teachers on how to use
the PAU - eForms.

5. THE PAU-eForms USER MANUAL
How to use the PAU-eForms?
1. The class adviser must secure a softcopy of SF1
(downloaded from LIS) or get a list of learners from the
ICT school coordinator.
2. After consolidating all grades and getting the copy
of SF1, in the PAU-eForms click on the “INPUT” sheet.
3. In the INPUT sheet, fill out the boxes with the
correct information.
4. Copy first the names of the male learners from the
SF1 and paste them under MALE LEARNERS’ NAME
column. Afterwards, copy the names of female learners
from the SF1 and paste them under FEMALE
LEARNERS’ NAME column. Followed by the respective
LRN and age of the student. If the SF1 is not yet
available, manually encode the names of learners.

6. 5. After completing the INPUT sheet, click on the
grading period you are working on. For example, if you
are working on FIRST quarter, simply click the FIRST
sheet as shown below:
6. Encode the grades of students in their different
subject areas. Upon successfully completing all the
data/grades needed, the FINAL GRADE for MAPEH is
automatically computed to give you the AVERAGE
GRADE and give the descriptive academic award of the
student for the First Quarter.

7. The AVERAGE GRADE is the grade to be encoded
on the Report Card (SF9) and Student’s Permanent
Record (SF10).
7. All data entered in the FIRST sheet are
automatically encoded in the MC sheet and SF9 sheet.
8. For the succeeding grading periods repeat step 6.
9. You may now print the Summary of Grades for each
grading period and the Master Card (MC) and prototype
SF9.

8. IMPORTANT REMINDERS
1. Keep learners’ name from FIRST to MC sheet in the
INPUT sheet. Do not delete the names of learners who
are transferred out/dropped out.
2. There is no need to encode the learners’ names and
subjects in each sheet because the learners’ names and
subjects will automatically appear in them right after
encoding the learners’ names in the INPUT sheet.
