Children's Mental Health Services - Amerigroup Presentation
Amerigroup Georgia provides Medicaid services to over 373,000 members across 159 counties in Georgia, most of whom are children. They have over 300 employees across 4 offices serving 6 Medicaid regions. Their programs include Temporary Assistance for Needy Families, Children's Health Insurance Program, and Foster Care. Amerigroup has innovative behavioral health programs including a behavioral health workgroup, psychotropic medication coaches, and complex case management. They are working to expand telehealth and community partnerships. As of October 2015, they were managing behavioral health for over 23,000 members through their Georgia Families 360 program.
Sick and Tired
1) Social assistance recipients in Ontario have significantly poorer health outcomes than both the working poor and non-poor across a wide range of health measures, even after controlling for other socioeconomic factors.
2) Both social assistance recipients and the working poor in Ontario are less likely to access preventative health care services and have higher rates of unmet health care needs than the non-poor, with cost and transportation being common barriers.
3) The report recommends expanding Ontario's poverty reduction strategy, increasing social assistance and minimum wage rates, improving access to health care including coverage of dental and vision services, and addressing other socioeconomic determinants of health.

1) The Sudbury District Nurse Practitioner Clinics (SDNPC) are a group of NP-led primary care clinics in Sudbury, Ontario that aim to provide access to care for underserved populations.
2) The SDNPC consists of 3 clinics with a multidisciplinary team including 5.5 NPs, 2 part-time physicians, and other health professionals. They provide comprehensive primary care.
3) The SDNPC was established in 2007 in response to thousands of residents lacking access to primary care. It has shown successes in improving access to care and detecting previously undiagnosed health issues, with a 7% reduction in ER visits attributed to increased access.

Chad Morris, PhD, is an Associate Professor at the University of Colorado Denver, Department of Psychiatry, and Director of the Behavioral Health & Wellness Program and interdisciplinary Wellness Leadership Institute. At UCD, he is the principal investigator of multiple studies exploring the effectiveness of organizational, psychosocial, and pharmacologic tobacco control and wellness strategies across the age range. As the Vice President of Spark Inspiration, Dr. Morris also offers corporate wellness solutions. The Garden River Wellness Centre delivers a variety of health and social services to members of the Garden River First Nation community. It implemented an electronic wellness record system to address problems with its previous paper-based system, including information not being easily shared between providers and inaccurate reporting. The electronic system required extensive customization to meet the community's holistic view of client care and incorporate both clinical and non-clinical services. Outcomes of the new system include improved communication and care coordination, centralized client registration, and more accurate reporting. Future plans include sharing client data electronically with off-reserve providers while ensuring community consent.

There were 9 community health worker (CHW) programs represented in Kent County, Michigan that employ a total of 51 CHWs on average. The majority of CHW programs address issues like diabetes, hypertension, maternal/child health, health literacy, asthma, nutrition, and obesity. CHWs provide services like outreach, health promotion, systems navigation, and case management in various locations like clients' homes, community events, and agencies. Most CHW programs receive funding from local grants, state grants, foundations, and federal grants. Barriers to sustainability include funding uncertainty and lack of management support for CHWs in some agencies.

Community HealthCorps National Evaluation
Community HealthCorps members work in federally qualified health centers and other organizations to help underserved communities access healthcare services. A study of over 2,800 Community HealthCorps members between 1995-2008 found that:
1) Members helped over 72,000 people each year gain access to health insurance and services and over 120,000 people improve their use of care.
2) Members provided health education to over 110,000 people annually through workshops and other activities.
3) Health center administrators saw Community HealthCorps members as having a very or somewhat significant positive impact on patient access, care continuity, and service delivery.

Capstone Project - final
Jacquelyn Favours conducted a capstone project assessing the primary care needs of women in Middle Tennessee and making recommendations for Planned Parenthood of Middle & East Tennessee (PPMET) to expand its services. She analyzed 450 patient records from PPMET clinics, finding high rates of chronic conditions. She recommended that PPMET expand primary care services, seek partnerships with medical schools, and launch a "Primary Care Safety Net" program to provide initial primary care to uninsured women. The project provided evidence of need to support PPMET expanding its role in improving women's healthcare access in the region.
EOA2016: Connecting Community to the Delivery System Public
During the last breakout session of the day, at Edge of Amazing 2016, a panel came together to discuss the interdependencies that are not the responsibility of any single organization, but are required if we want to achieve population health. They featured the many ways community is linking to the delivery system, including an overiew of the Plan for Improving Population Health and the Practice Transformation Support Hub.

Evaluation data found that most patients are Mexican immigrants and the top reasons for visits included diabetes, hypertension, and obesity. The goals of Puentes de Salud are to provide medical services, develop long-term health strategies, and create a research partnership to advance Latino health outcomes.

Emerging Models- Reaching the Hard to Reach and Underserved
This panel discussion explored emerging models for reaching underserved populations in healthcare. Panelists presented on models for African American males, immigrants, rural residents, and the elderly. Community health centers were shown to effectively serve populations with high rates of poverty. A community health worker model improved access and outcomes. A home-based program reduced hospitalizations and improved management of diabetes and heart failure in rural areas. The transition to value-based care emphasizes primary care and care coordination through models like integrated behavioral health teams.

The purpose of this Health Policy Study is to better understand adolescents’ views on what are considered core components of the medical home and identify barriers to promoting adolescent health in relation to the medical home.
In addition, this study sought to better understand the needs and challenges in providing adolescents with access to medical homes—from the perspective of both adolescents and experts in adolescent health and medical home policy. To accomplish these goals, researchers conducted focus groups with adolescents, presented these findings to experts, and gathered experts’ reactions to the adolescents’ perspectives.
