every American by the year
2014…by computerizing health
records, we can avoid dangerous

and improve care” (Bush, 2004). This was the first
time a president formally recognized the value of HIT
and set a deadline to do something about it! President
Bush went on to establish the Office of the National
Coordinator for HIT (ONC), and Dr. David Brailer was
appointed as the first coordinator by Tommy
Thompson, then Secretary of the Department of
Health and Human Services (HHS).

HHS was earmarked to establish organizations for
standards harmonization (HIT Standards Panel) and
for certification of electronic health record (EHR) sys-
tems (Certification Commission for HIT). In 2006, the
Agency for Healthcare Research and Quality (AHRQ)
launched its National Resource Center for HIT.
Government attention persisted in 2007 with the
funding of National Health Information Network pro-
totypes. Momentum was building and there was
much attention on HIT from the federal government.
Fast forward to 2009. President-Elect Barack
Obama says he wants the federal government to
invest in EHRs so all medical records are digitized
within 5 years and vows to continue to push for the
2014 deadline established by Bush. “This will cut
waste, eliminate red tape, and reduce the need to
repeat expensive medical tests,” he said, adding that
the switch also will save lives by reducing the num-
ber of errors in medicine (Obama, 2009).
President Obama then does more than talk about
HIT. He works with Congress to pass the American
Recovery and Reinvestment Act (ARRA), providing
unprecedented funding to promote health care reform
through the use of HIT. Incentives totaling $19 billion
are allocated for “meaningful use” of EHRs in hospi-
tals and ambulatory settings beginning in 2011. This
sets the stage for today’s focus on the use of HIT, and
the proliferation of EHR implementation projects in
our clinical settings. Let’s explore the legislative back-
ground and details surrounding the federal incen-
tives.
Legislative Background
On March 23, 2010, President Obama signed into
law the landmark Patient Protection and Affordable
Care Act (PPACA), a federal statute that represents the
most recent legislation in a sweeping health care
reform agenda driven into law by the Democratic
111th Congress and the Obama Administration. The
new law is dedicated to replacing a broken system
with one that ensures all Americans have access to
health care that is both affordable and driven by qual-

The First Medical Records
The earliest forms of medical records were narratives written by ancient Greeks to document
successful cures, share observations about symptoms and outcomes, and teach others who
provided medical advice through these case studies. While written reports describing patients’
complaints and diagnoses predate the records of Simon Forman and Richard Napier – astrologers
who documented clients’ medical questions and treatment – their records from 1596 to 1634
form the earliest complete collection of medical records in existence.
The 1920s
As healthcare advanced, physicians realized that the best way to continue improving diagnosing
and treating illnesses was to carefully document observations and actions while treating patients
– and share this information as a way to teach other health professionals.
As early as 1600, physicians offered advice on how to present information in a medical record,
but it wasn’t until 1928 that the American College of Surgeons (ACOS) took steps to standardize
the growing number of medical records by establishing the American Association of Record
Librarians (AARL) – known today as the American Health Information Management
Association (AHIMA). “Record librarians” was the term used because early medical records
were documented on paper.
Standardization of medical records and growth of complete record-keeping continued from the
1920s through the 1960s, but records were paper-based.
The 1960s
The development of computers presented the opportunity to maintain records electronically, but
the expense of purchasing and maintaining a mainframe, and the expense associated with storage
of data, meant that only the largest organizations could use technology to handle medical
records.
The field of health informatics, as it is known today, emerged when computer technology
became sophisticated enough to manage large amounts of data. One of the earliest efforts took
place under the jurisdiction of the American Society for Testing and Materials (ASTM). These

An E.H.R. means different things to different organizations and the people within them. Smaller physician practices think of it as a key system for documentation and coding support. For some, it is merely a document imaging system that provides access to paper-based records. For lager hospitals and clinics, it consists of a massive and costly undertaking or merging data from source systems redesigning processes, building decision support and gaining options (Amatayakul, 2004).
The Institute of Medicine offered a concise definition on the computer based patient record and had a vision for the E.H.R. system that consists of collecting data from multiple sources and it is used by clinicians as the primary source of information at the point of patient care in order to provide a decision support for evidence-based health care (Amatayakul, 2004).
The achievement of this vision has had its challenges since no standards define what an E.H.R should do and this left the system open for many interpretations. However, the Institute of Medicine managed to identify eight core functions for the EHRs that have been woven into the EHR-System Functional Model Draft Standard that include health information and data, results management, order entry management, decision support, electronic communication and connectivity, patient support, administrative processes and reporting and population health management (Amatayakul, 2004)

Introduction and Definitions
The term “Electronic Health Records” (EHR) means
something different to each of the stakeholders in
health care, but it always seems to carry a degree of
emotional baggage. Increasingly, EHRs are advert-
ized as a nearly unmitigated good that will transform
medical care, improve safety and efficiency, allow
better patient engagement, and open the door to an
era of cheap, effective, timely, and patient-centered
care.1 Indeed, for some EHR proponents the ben-
efits of adopting them are so obvious that adoption
has become an end in itself.2 But for others — and
especially for a number of skeptical practitioners and
patients — EHR is a code word that portends the cor-
porate transformation of health care delivery, the loss
of patient privacy, the demand that patients bear more
responsibility in health care, and the unreflective take-
over of the health care system by people who do not
understand medical care or how health care relation-
ships unfold.3
For our purposes, we will consider EHRs impar-
tially, as a set of tools that can be used for a variety of
purposes. We define EHRs broadly as any electronic
means of storing and transferring health-related
information. We exclude from this definition the use
of the telephone and fax, arguably precursors to the
electronic means of data exchange now available. Like
face-to-face and paper-based interactions, the tele-
phone and fax are generally limited to two people.
Breaches of phone line security, while possible and
perhaps even frequent, are unlikely to affect thou-
sands of people at once.
In this paper, we examine the development of a new
set of EHR tools, Personal Health Records (PHRs).
PHRs may be variously defined (Table I) and have sev-
eral potential functional and payment models (Table
II), but the general aim of all PHRs is to increase
patients’ access to and sense of ownership over their
health care information. According to the Markle
Foundation, the advent of PHRs “represents a transi-
tion from a patient record that is physician-centered
to one that is patient-centered, prospective, interac-

Health Information Technology (HIT) involves the use of a digital format to store, distribute, and analyze health information. HIT is used between doctor-patient communications to improve the patient’s care. The IT professionals working in the HIT are involved in the technical side of managing health information using software and hardware to manage patients’ data. The professionals provide support to the Electronic Health Records (EHRs) management and other professionals to ascertain accurate dissemination of medical records adhering to HIPAA and HITECH rules and regulations (McInnes, 2016).

Improved health care and convenience
Access to the patients’ records is easy even in remote locations which ease the quality of care in the case of emergencies. Having all the information located at a central place makes decision making easy. Use of EHRs helps different hospitals and laboratories to access a patient’s medical history

EHRs makes it possible for health professionals to give the patients all their medical evaluations. The providers can provide follow-up information such as web resources and self-care instructions which aid in improving the quality of health care.
Improved diagnostics and patient outcomes
With an accurate medical history of a patient, the medical professionals can check for any allergies, prior treatments which help during the treatment process. The frequency of medical errors during diagnosis. Qualified EHR checks for problems when a new medication is given to the patient and alerting the doctor in advance in the case of any potential threats. In regard to national disasters where casualties forget their medical history, EHR helps quick retrieval of their history and aid in saving their lives.

Use of EHRs assists in reducing duplication of testing which saves on the cost of conducting laboratory tests. Moreover, the administrative expenses as a result of filling of numerous patients paperwork. As a consequence of the use of EHRs, there is reduced transcription, storage, and chart-pull costs. The use of electronic prescribing reduces medical errors (Goldberg, & Feng, 2012).
Cost of implementing and maintaining EHR
Setting up an EHR system up and running is quite expensive. Some of the expenses associated with establishing the system are the required hardware and software, installation, training of the IT professionals and ongoing maintenance. There are other costs such as over-time-pay for the providers. Taking a primary care facility with five physicians, the cost of setting up an EHR system is $162,000 and maintenance cost of $ 85,500 within the first year.
