(EHR) ?
  It is a digital record of patient health
 information generated by one or more
 encounters in any health care setting.

Content of EHR
Your electronic health record contains the following
information:

•   Identity Sheet- Biographic data ( age, sex, address, name)
•   Progress Notes- Notes made by doctors.
•   Immunization Records- Dates of immunization
•   Allergies
•   Radiology Images/Reports
•    Lab & Test Results
•   Medication
•   Past Medical History- Past Surgeries, previous hospitalization
•   Vital Signs- blood pressure readings, glucose readings
•   Diagnosis
• Reduces paperwork/fewer shorter forms to complete
• Gives medical providers more insight into your personal history with the
    use of your personal health record.
• Reduces unnecessary tests and procedures
• Gives you direct access to your health care records
• Helps doctors coordinate your care and safety
