9 Ajankohtaista nikotiinittomuustyössä – kuinka savuton ja nikotiiniton Suomi 2030 -tavoite saavutetaan?

Savuton ja nikotiiniton Suomi 2030 -tavoitteeseen on alle kahdeksan vuotta aikaa. Keskeisimmät haasteet tavoitteen saavuttamiseksi liittyvät nikotiinituotteiden lopettamiseen ja aloittamisen ehkäisyyn eri kohderyhmillä sekä tupakkateollisuuden, erityisesti nuorille markkinoimat, uudet nikotiinituotteet.

Tällä hetkellä kroonisten hengitystiesairauksien diagnoosi
perustuu keuhkojen toimintakokeisiin. Nämä puhalluskokeet ovat aikaa
vieviä ja vaativat ammattitaitoa. Lisäksi tulokset
korreloivat huonosti potilaan kliinisiin oireisiin,
ennusteeseen, hoitovasteeseen, pahenemisvaiheisiin
ja kuolleisuuteen. SalWen Älykäs monitorointi -ohjelmassa
etsittiin uudenlaisia biomarkkereita
sekä veri- että yskösnäytteistä.
Elämäntapamuutosten tulisi olla elinikäinen oppimisprosessi, eikä tiettyyn ajanjaksoon kohdistuva ponnistelu. Uusien oppimisteorioiden avulla interventioiden tehokkuutta voidaan edistää ja siten saada pysyvämpiä tuloksia.

Researchers in Finland have been studying the use of auditory event-related potentials (ERPs) to diagnose Alzheimer's disease in early stages. The study found sensory gating, the brain's ability to filter out repetitive sensory inputs, is impaired in patients with mild cognitive impairment and Alzheimer's disease. ERPs provide an objective, non-invasive way to test cognitive processes and sensory gating. The research was conducted on patients with mild cognitive impairment, early Alzheimer's, and a control group. Arvioiden mukaan noin 46 prosenttia Suomen terveys-
keskusten resursseista suuntautuu pitkäaikaissairai-
den hoitoon. Terveyskeskusten selviäminen kasvavien
potilasmäärien ja henkilöstövajeen aiheuttamassa
ristiaallokossa, edellyttää potilaiden oman roolin
vahvistamista kroonisten sairauksien hoidossa. Pitkäai-
kaissairauksien terveyshyötymalli (CCM) on yksi niistä
malleista, joka korostaa potilaan omaa aktiivisuutta
hoitonsa ja elämäntapamuutostensa suunnittelussa
ja toteutuksessa. Mallin avulla toivotaan päästävän
parempiin hoitotuloksiin ja hoitokustannusten pie-
nentämiseen.
”Jotta terveyshyötymallista saataisiin tehokas työkalu
kliiniseen käyttöön, tarvitaan perusteellista arviointia
sen tarjoamista hyödyistä perusterveydenhuollossa.
Aikaisemmissa tutkimuksissa sen on todettu paranta-
van hoitotuloksia ja tuovan kustannussäästöjä, mutta
kustannustehokkuuden osoittaminen vaatii lisänäyt-

den käyttöön ja hoidon kustannuksiin espoolaisessa
terveyskeskuksessa.
Terveyshyötymallin mukaista hoitoa saaneita potilaita
tutkimukseen osallistui 6 672. Tavanomaista hoitoa
saaneita verrokkiryhmän potilaita oli 2 606.

potilaan sitouttaminen oman sairautensa hoitoon.
Potilaan omahoidon tuki, hoitoa tukevat palveluket-
jut, ammattilaisen päätöksenteon tuki ja soveltuvat
kliiniset tietojärjestelmät ovat olennaisia, kun mallia
sovelletaan terveyspalveluissa”, kuvailee Iiris Riippa.
Arvot paranivat ilman lisäresursseja
CCM-ryhmän keskimääräinen verensokeriarvo aleni
1,4 mmol/mol ensimmäisen seurantavuoden aikana.
Seuraavien kolmen vuoden aikana alenema pysyi jok-
seenkin samana. CCM-ryhmä tapasi sairaanhoitajan
keskimäärin 1,8 ja lääkärin 0,7 kertaa useammin kuin
verrokkiryhmä ensimmäisen seurantavuoden aikana.
Seuraavien vuosien aikana sairaanhoitajatapaamiset
vähenivät niin, että neljäntenä vuonna CCM-ryhmäs-
sä tapaamisia oli 0,3 kertaa verrokkiryhmää enem-
män. Lääkärin tapaamiskerroissa ryhmien välillä ei
ollut merkittävää eroa ensimmäisen seurantavuoden
jälkeen.
Alenema kakkostyypin diabetes-potilaiden veren-
sokeriarvoissa säilyi koko seurantajakson ajan, vaikka
lääkärikäynnit lisääntyivät ainoastaan ensimmäisen
vuoden aikana. Tapaamiset sairaanhoitajan kanssa li-
sääntyivät koko seurantajakson aikana, joskin loppua
kohti tapaamisten määrä ei kasvanut niin paljon kuin
alussa.
”Voimme todeta, että verensokeriarvojen paranemi-
nen saavutettiin ilman hoitoresurssien merkittävää
lisäämistä, joskin hyvään tulokseen saattoi vaikuttaa
myös kontrolliryhmän tietty valikoituminen”, kertoo
Iiris Riippa.
Tutkimus antaa positiivisia viitteitä siihen, että suh-
teellisen pienillä lisäpanostuksilla terveydenhuol-
lon budjetteihin, voitaisiin saada entistä parempia
väestötason hoitotuloksia, jos mallia hyödynnettäi-
siin laajamittaisesti. Espoossa mallin soveltaminen
kaikkien kakkostyypin diabeetikoiden hoidossa oli-
si kokonaiskustannuksiltaan arvioiden mukaan vain
noin 0,5 prosenttia terveydenhuollon vuosittaisesta
budjetista.
