A failed IVF cycle can be because of poor egg quality, sperm quality or uterine lining. It is assumed that all the stimulation egg pick up, laboratory procedures and embryo transfers have been done meticulously in previous attempts. We offer certain modifications in an IVF cycles for optimizing outcome in couples suffering from failed IVF attempts-
Optimized stimulation protocol: The short antagonist protocol offers the best results in terms of selection of the best oocytes (eggs) in most cases.
Selection of Sperm: In many cases, Intra Cytoplasmic Sperm Injection (ICSI) is offered as it has been suggested that it may improve fertilization rates and hence, overall pregnancy outcome. Our embryologist takes special care to select the best sperms for doing ICSI.
Hysteroscopy: The hysteroscope aids us in picking up uterine abnormalities which are sometimes missed at routine ultrasound e.g., small polyps. It is also useful in washing and cleaning the uterus which sometimes may help in improving the outcomes. Endometrial scratching is also done at the same setting to improve the uterine receptivity.
Intravenous Immunoglobulin (IYIg): IVIg seems to directly affect NK cell level and activity, by reducing their absolute numbers and increasing the expression of inhibitory receptors CD94 which potentially can improve pregnancy outcome.
Vitamins and Antioxidants: DHEA, L Arginine, Zinc, selenium etc. are given to women and men as indicated to improve the egg and sperm quality.
Atosiban: This is a uterine relaxant which is given during the embryo transfer. It helps in relaxing the uterus and therefore, improving the endometrial receptivity.
Laser Hatching: Laser hatching of the embryos is performed on the day of embryo transfer to ensure that the shell of the embryo hatches easily. This allows the embryo to implant better. This is mainly suitable for embryos with thick shell, advanced age group and frozen embryos.
ERA: ERA presumably detects the phase of the endometrium in which the embryo best implant. However, there is controversy regarding the actual benefit of this in improving the live birth rate.
PGS: PGS is a way of detecting abnormal embryos thus may help in improving the pregnancy rates. However, each case must be individualized.
Day of Transfer: Not all women will be benefitted by Blastocyst (Day 5 ) transfer as many seem to believe by studying the internet. The day of transfer should be individualized for each patient.

Threatened abortion is associated with bleeding and/or uterine cramping while the cervix is closed. This stage of abortion may progress to spontaneous incomplete or complete abortion. While this event may be considered a part of the quality control process in human reproduction, it is important to know the possible etiologies and when therapy might prevent pregnancy loss. The World Health Organization estimated that 15% of all clinically recognizable pregnancies and in spontaneous abortion, 50-60% of which are due to chromosomal abnormalities. Apart from the fetal factors, several maternal and probably paternal factors contribute to the causes of spontaneous abortion. The maternal factors that may be responsible for abortion include both local and systemic conditions such as infections, maternal disease states, genital tract abnormalities, endocrine factors and other miscellaneous causes (antiphospholipid antibodies, maternal-fetal histocompatibility, excessive smoking and other environmental toxicants, etc.). This review focuses on the management of threatened abortion, but it should be emphasized that the management to maintain pregnancy is reasonable only in those cases, in which the fetus is not seriously affected. It would not be beneficial to provide treatment that would permit chromosomally and anatomically abnormal embryos to survive to term. Treatment is feasible first of all in cases with maternal factors. Surgical procedures may precede pregnancy (correction of septate uterus, removal of a submucous leiomyomata) or may be performed usually in the second trimester (cervical cerclage). Maternal general diseases (diabetes, hypothyroidism) and infections should be treated accordingly. The most common entity to be treated in this category is luteal phase deficiency. Progesterone is the most important hormone for the maintenance of an early human pregnancy. Besides progesterone administration, human chorionic gonadotropin (hCG) also is the logical endocrine treatment of choice. • Routine surgical evacuation of the uterus is not
required. It should only be undertaken if there is clinical
evidence that the abortion is incomplete (El-Refaey and
Templeton, 1995).
• Only 8–11% of women needed surgical evacuation
following T2 medical abortion (Ashok et al., 2004).
• Surgical evacuation was as low as 2.5% (Hamoda et
al., 2005a).
8. Making choices
• Although D&E is a very safe and effective procedure,
its safety profile derives from the surgeon’s knowledge,
experience, and skills.
• In the past, institutions that have lacked skilled D&E
providers have had to refer patients to other facilities or
choose medical induction regimens characterized by
long induction-abortion intervals and relatively high
rates of complications.
• Newer medical regimens using PG E1 analogue with
or without mifepristone offer sufficiently effective, well
tolerated abortion-induction intervals so that clinicians
have far greater latitude choosing a medical vs surgical
approach.
9. HOW MUCH DOES THE PROCEDURE
COST?
The non-surgical procedure is $470. This includes the initial
consultation, the preliminary ultrasound to determine the exact
weeks and location of pregnancy, and the cost of the
medications used in the office such as the RU-486 and the
misoprostol. The $470 also includes all follow-up visits in the
Physician to ensure a completion of non-surgical abortion. The
$470 also includes any surgical procedure performed in the
Physician as a follow-up of the non-surgical abortion.
10. WHO DOES NOT QUALIFY FOR A MEDICAL
ABORTION?
A medical abortion is not recommended if you:
• re more than 56 days pregnant
• take anti-clotting medication or have blood-clotting
disorders.
• have severe heart, liver, or active inflammatory
bowel disease.
• have certain severe kidney problem.
• have a serious immune system problem, such as
HIV.
• have seizures more than once a week.
• take any medicine that should not be combined with
methotrexate, mifepristone, or misoprostol.
• cannot return for follow-up visits.
• have a tubal pregnancy.
