High risk or targeted population groups usually consist of individuals that are at a dis-advantage in some way, shape or form. These populations are at an increased risk for poor health due to lack of support, resources and/or appropriate health care facilities.
The first step to identifying a high-risk population regardless to the situation is to perform a risk assessment. This process enables you to examine information and data that can help to identify health disparities. The data collected via surveys, interviews, and observations can be used to not only identify high-risk populations, but it can also help to determine what interventions or resources are needed.
High-risk or targeted populations may not have access to the things they need to support their health. Some examples of those resources include: affordable health care, safe and adequate housing, mental health support, family support, and financial resources and unfortunately, this population sometimes falls through the cracks when it comes health resources and other needed resources. With the right resources in place it can help the community for many generations. Sometimes poor health practices are passed down and can continue to plague many generations of a family. East Harlem, an upper Manhattan neighborhood, is home to many immigrant
communities including one of the largest Latino communities in New York City with an overall
population of nearly 120,000. This neighborhood has the largest population of Latinos and
African-Americans with low incomes in New York City. Because of their low socioeconomic
status and low quality of living, the residents of this neighborhood face several health concerns.
Diabetes is a major health concern in the United States, but the residents of East Harlem are
disproportionately affected by it (Fox, Mann, Ramos, Kleinman, & Horowitz, 2012; National
Alliance for Hispanic Health [NAHH], n.d). East Harlem has the highest diabetes mortality rate
(13%) and the highest incidence of obesity (33%) in New York City (New York City Department
of Health and Mental Hygiene [NYCDOHMH], 2015). This disparity exists as Hispanics lack
access to proper nutrition.
The Frequency and Causes of Diabetes among the Vulnerable Population
The high rate of diabetes among the residents of East Harlem can be better understood by
analyzing the factors behind it. There are a number of factors that contribute to higher rates of
diabetes among certain racial or ethnic groups. Lack of access to health care because of language
and financial barriers directly impact the rates of diabetes mortality. According to the Agency for
Healthcare Research and Quality (AHCRQ), the diabetes patients among Hispanics are more
likely than whites to get readmitted to hospitals within a span of six months since the beginning
of the treatment (NAHH, 2010).
A primary reason for the increased occurrence of diabetes among the Hispanic residents
of East Harlem is the lack of resources to cope with their external environment, stress-related
problems, and poor nutrition. A lack of exercise also contributes to the onset of diabetes among

Hispanics. The external environment also plays a large role in the onset of diabetes by creating
unequal opportunities for people from different backgrounds to participate in recreational
physical activities. A poor understanding of the disease contributes to higher instances of

environmental factors, Hispanics are more likely to have a family history of diabetes (NAHH,
2010).

The reason many immigrants or those without citizenship have a lower frequency to facilities and physicians would be for the fear? Health education? A vulnerable population is a group or groups that are more likely to develop health related problems, have more difficulty accessing healthcare to address those health problems, and are more likely to experience a poor outcome or shorter lifespan because of those healthconditions. There are a number of characteristics, traits, or circumstances that enhance the potential for poor health. Certain groups are more vulnerable to health risk. At risk groups or persons includes the poor, the homeless, disabled, the severely mentally ill, very young and the very old. For exampleolder adults are vulnerable to being victimized. Because of increased dependency and the potential for chronic health impairment, older adults may be placed in situations in which they are less able to advocate for their own needs. Older adults may be abused or neglected by caregivers or be victims of criminal practices (Medicine and Health Articles, 2018). Immigrants are also at greater risk for these related health issues as well, due lack of resources and barriers with communication.
Nurses can advocate on behalf of vulnerable groups by writing and calling government representatives and speaking to professional and community organizations about the problems and needs for high risk groups. The community health nurse may work with a coalition of providers and citizen groups to advocate for the needs of the older adult population or may participate in hearings of area agencies to advocate for greater community-based long-term care services (Medicine and Health Articles, 2018).

East Harlem, an upper Manhattan neighborhood, is home to many immigrant
communities including one of the largest Latino communities in New York City with an overall
population of nearly 120,000. This neighborhood has the largest population of Latinos and
African-Americans with low incomes in New York City. Because of their low socioeconomic
status and low quality of living, the residents of this neighborhood face several health concerns.
Diabetes is a major health concern in the United States, but the residents of East Harlem are
disproportionately affected by it (Fox, Mann, Ramos, Kleinman, & Horowitz, 2012; National
Alliance for Hispanic Health [NAHH], n.d). East Harlem has the highest diabetes mortality rate
(13%) and the highest incidence of obesity (33%) in New York City (New York City Department
of Health and Mental Hygiene [NYCDOHMH], 2015). This disparity exists as Hispanics lack
access to proper nutrition.
The Frequency and Causes of Diabetes among the Vulnerable Population
The high rate of diabetes among the residents of East Harlem can be better understood by
analyzing the factors behind it. There are a number of factors that contribute to higher rates of
diabetes among certain racial or ethnic groups. Lack of access to health care because of language
and financial barriers directly impact the rates of diabetes mortality. According to the Agency for
Healthcare Research and Quality (AHCRQ), the diabetes patients among Hispanics are more
likely than whites to get readmitted to hospitals within a span of six months since the beginning
of the treatment (NAHH, 2010).
A primary reason for the increased occurrence of diabetes among the Hispanic residents
of East Harlem is the lack of resources to cope with their external environment, stress-related
problems, and poor nutrition. A lack of exercise also contributes to the onset of diabetes among

Hispanics. The external environment also plays a large role in the onset of diabetes by creating
unequal opportunities for people from different backgrounds to participate in recreational
physical activities. A poor understanding of the disease contributes to higher instances of

environmental factors, Hispanics are more likely to have a family history of diabetes (NAHH,
2010).

The reason many immigrants or those without citizenship have a lower frequency to facilities and physicians would be for the fear? Health education? What causes the immigrants to not seek citizenship or work visas? The whole political issue has not changed many in that they are still not seeking status in the US. Do you see this as a problem as a health care professional and as a community member?

A vulnerable population is a group or groups that are more likely to develop health related problems, have more difficulty accessing healthcare to address those health problems, and are more likely to experience a poor outcome or shorter lifespan because of those healthconditions. There are a number of characteristics, traits, or circumstances that enhance the potential for poor health. Certain groups are more vulnerable to health risk. At risk groups or persons includes the poor, the homeless, disabled, the severely mentally ill, very young and the very old. For exampleolder adults are vulnerable to being victimized. Because of increased dependency and the potential for chronic health impairment, older adults may be placed in situations in which they are less able to advocate for their own needs. Older adults may be abused or neglected by caregivers or be victims of criminal practices (Medicine and Health Articles, 2018). Immigrants are also at greater risk for these related health issues as well, due lack of resources and barriers with communication.
Nurses can advocate on behalf of vulnerable groups by writing and calling government representatives and speaking to professional and community organizations about the problems and needs for high risk groups. The community health nurse may work with a coalition of providers and citizen groups to advocate for the needs of the older adult population or may participate in hearings of area agencies to advocate for greater community-based long-term care services (Medicine and Health Articles, 2018).

Artificial Intelligence (AI) technologies such as Generative AI, Image Generators and Large Language Models have had a dramatic impact on teaching, learning and assessment over the past 18 months. The most immediate threat AI posed was to Academic Integrity with Higher Education Institutes (HEIs) focusing their efforts on combating the use of GenAI in assessment. Guidelines were developed for staff and students, policies put in place too. Innovative educators have forged paths in the use of Generative AI for teaching, learning and assessments leading to pockets of transformation springing up across HEIs, often with little or no top-down guidance, support or direction.
This Gasta posits a strategic approach to integrating AI into HEIs to prepare staff, students and the curriculum for an evolving world and workplace. We will highlight the advantages of working with these technologies beyond the realm of teaching, learning and assessment by considering prompt engineering skills, industry impact, curriculum changes, and the need for staff upskilling. In contrast, not engaging strategically with Generative AI poses risks, including falling behind peers, missed opportunities and failing to ensure our graduates remain employable. The rapid evolution of AI technologies necessitates a proactive and strategic approach if we are to remain relevant.

Tam//// There are several vulnerable populations that have a chronic illness (older; homeless; and lesbian, gay, bisexual, and transgender populations) that face challenges when it comes to care. Choose one vulnerable population and discuss what can be done to help alleviate these challenges “More than 80% of homeless people have at least one chronic health condition & more than 50% have a mental health disease” (Chronic Disease Management in the Homeless, 2014). Of the populations there are about 40-60% that suffer from substance abuse. Some of the risk factors of those that are homeless are tobacco abuse, alcohol abuse, malnutrition, exposure to the environment, IV drug use, trauma, stress and suppressed immunity. Some of the barriers to treatment are no access to medications, medication storage, no transportation, no financial resources, no housing, and poor nutritional access. Treatment approaches for the homeless that have chronic disease are to discuss in detail the patient’s limitations to follow the treatment plan. You should also try to empower the patient by helping them set goals and set priorities that they will be able to obtain. There are also homeless assistance programs that would benefit those that were willing to receive it like HUD and the National Coalition for the Homeless.
