SOCW 8205/SOCW 6205 Case Study: Biopsychosocial Assessment Karen is a 40-year-old African American female. She is divorced and the mother of a 20-year-old daughter, Sarah. Sarah has a daughter who is 1½ years old. Karen lives in Queens, New York, with her mother, daughter, and granddaughter. Karen has a brother who is married with a child. She is very close to her family. Karen works full-time as an emergency room nurse. She has had this job for more than 15 years. Her ex-husband was an alcoholic, and he left Karen and Sarah when Sarah was born. Karen has no history of drug or alcohol abuse. Karen raised Sarah as a single parent, and Sarah has no contact with her father. Karen and Sarah regularly attend the same local Christian church as Karen’s mother. The family considers itself religious. The church community is very important in the family’s life. Six years ago, Karen was working a shift in the emergency room. Although she followed standard universal precautions, Karen was stuck by a needle through her glove. She followed appropriate protocol for a needle stick according to hospital policy. Ultimately, Karen grew ill and she was diagnosed with HIV/AIDS. Karen has taken antiretroviral therapy for several years. She has had a good response to treatment. She is fully adherent to the HIV/AIDS treatment regimen. Karen’s family is aware of her HIV status but has not disclosed her status to friends or members of the church community. Karen was recently admitted to the hospital with a new diagnosis of leukemia. She was told that her condition was aggressive and she needed a series of treatments including chemotherapy. While on chemotherapy, Karen had an extremely difficult time. She had numerous painful and deteriorating side effects. Throughout all of this, Karen kept a bright sense of humor and the hope that she would recover. Her daughter and granddaughter were always in her thoughts. Karen’s mother spent many days and nights in the hospital. Karen allowed her daughter and granddaughter to visit on occasion, but she never told her daughter how bad her condition was. Karen was in the hospital for nearly a month. On some days, she was bright and cheerful. On others, she was so sedated on pain medication that she was not coherent. After multiple rounds of chemotherapy, Karen did not respond. Her doctors ultimately told her that there was nothing else they could offer her. Karen and her mother were devastated. At this point, Karen was weak, gaunt, and had little ability to care for herself. Her wish was to go home to die. Karen’s family tried to take her home, but they could not care for her alone. Karen fell and developed a high fever that they could not control. Karen was readmitted to the hospital for a short stay and was ultimately transferred to a hospice facility where she died. Sara and her daughter have lived together for the past 10 years, since Stephanie returned home after a failed relationship and was unable to live independently. Stephanie has a diagnosis of bipolar disorder, and her overall physical health is good. Stephanie has no history of treatment for alcohol or substance abuse; during her teens she drank and smoked marijuana but no longer uses these substances. When she was 16 years old, Stephanie was hospitalized after her first bipolar episode. She had attempted suicide by swallowing a handful of Tylenol® and drinking half a bottle of vodka after her first boyfriend broke up with her. Abstract
As providers, it is vital to be open-minded, if you are not aware or do not understand a person’s
culture, show your patients respect. Be engaged, respectful, and ask questions. Learn from your
patients, this helps develop a rapport. As providers we are obligated to provide optimal care, if
we feel a patient's needs are out of our scope, the client should be referred to a provider that can
give them the help they need and deserve (Nichols & Davis, 2020).
The purpose of this paper is to provide documentation and a psychiatric comprehensive
assessment of a patient and family during a family therapy interview that highlights differences
in a multicultural family, respecting cultural norms and differences, and develop an
individualized treatment plan.

CC (chief complaint): “I like to learn to live on my own and not depend on the kids.”
HPI: A 40-year-old female (Patti) and her five children (Sheela age 24, Sharleen aged 23
and present today, Shireen age 21 recently reunited with family, Armin Jr. age 18, and Sam age
15), of Iranian descent, has been in individual and family therapy for the last 18 months. Patti
came to the clinic related to chaos in the home after her daughter was finally reunited with the
family after ten years. The family initially had rejoiced and celebrated, and after a few weeks,
Shireen began to tell her mother and siblings the emotional, physical, and sexual abuse at the
hands of her father. She blamed and felt abandoned by her mother. Patti needed necessary
surgery to both her feet after an injury, that has now left her disabled and with chronic pain. The
additional burden of surgery and disability has increased tension and stress in the home. Patti
lives with her two sons; her daughters live on their own. Shireen recently moved out, marrying
someone the family does not know, with little contact. Mother speaks and sees Sheela and
Sharleen daily. Patti has increasingly felt alone, depressed, hopeless, and helpless wanting her
daughters to stay and help her at her home. A psychiatric provider has been referred for a
medication evaluation.
Past Psychiatric History:
General Statement: Patti and her family began therapy 18 months ago after daughter
Shireen reunited with the family after 12 years of separation. Shireen shared significant abuse
she experienced, learning the trauma she went through brought on many emotions of the family
including blame, guilt, shame increasing stress on the family.

Karen Langlois-Foster was recently awarded a trip to Disney World by the Dream Foundation after being diagnosed with Stage IV breast cancer in 2010 at age 50. She initially found a lump through self-examination and subsequent tests confirmed the cancer diagnosis, despite no family history of breast cancer. Karen received radiation therapy and chemotherapy treatment and is now enjoying time with her family. She advises others facing similar situations to talk with family and not go through the experience alone.
Maternal death case studies
A 28-year-old woman delivered at home with the help of an untrained birth attendant and experienced heavy bleeding after delivery. She died on the way to the hospital after difficulties finding transportation. A 21-year-old woman had an abortion performed by an untrained provider and developed a fever and infection after. She grew increasingly ill but did not seek medical care due to lack of money and died at home. A 25-year-old woman experienced recurring vaginal bleeding during her pregnancy and sought care from a local health worker. When the bleeding became heavy in her eighth month, she struggled to get money for transportation and died after arriving at the district hospital unconscious.

· One of the helpful things the nurse did to assist this family was to establish a consistent group of nurses to work with Mrs. Walsh, so that family members could establish trust and feel more confident about the care their mother was receiving. This eventually enabled family members to leave the hospital for intervals to get some rest. The nurse related that this was a family whose members were affluent, educated, and well informed, and that they came in prepared with lists of questions. A consistent group of nurses who were familiar with Mrs. Walsh’s particular situation helped both family members and nurses to be more satisfied and less anxious. The family developed a close relationship with the three nurses who consistently cared for Mrs. Walsh and shared with them details about Mrs. Walsh and her life.
· The nurse related that there was a tradition in this particular critical care unit not to involve family members in care. She broke that tradition when she responded to the son’s and the daughter’s helpless feelings by teaching them some simple things that they could do for their mother. They learned to give some basic care, such as bathing her. The nurse acknowledged that involving family members in direct patient care with a critically ill patient is complex and requires knowledge and sensitivity. The Case of Carol and Joseph
Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrangement
with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed
the genetics of her family, and she said that she realized that she
had deteriorated rapidly since beginning to drink and knew that
she simply could not drink alcohol.
Joseph’s mother is deceased, and his father travels extensively
in his job and is not available as a support. Joseph was
very devoted to his mother and was devastated by her premature
death. We discussed the strengths that he and Carol demonstrated
in staying together and working out their p.

The second paragraph is far too generic and gives us no idea of the strategy used to analyse the issue. We are simply promised some “critical analysis.” The one positive point is that this second paragraph at least makes clear that it is the risk phrases that are the topic of the report, not the whole article.

“Mrs. Walsh, a woman in her 70s, was in critical condition after repeat coronary artery bypass graft (CABG) surgery. Her family lived nearby when Mrs. Walsh had her first CABG surgery. They had moved out of town but returned to our institution, where the first surgery had been performed successfully. Mrs. Walsh remained critically ill and unstable for several weeks before her death. Her family was very anxious because of Mrs. Walsh’s unstable and deteriorating condition, and a family member was always with her 24 hours a day for the first few weeks.
The nurse became involved with this family while Mrs. Walsh was still in surgery, because family members were very anxious that the procedure was taking longer than it had the first time and made repeated calls to the critical care unit to ask about the patient. The nurse met with the family and offered to go into the operating room to talk with the cardiac surgeon to better inform the family of their mother’s status.
· One of the helpful things the nurse did to assist this family was to establish a consistent group of nurses to work with Mrs. Walsh, so that family members could establish trust and feel more confident about the care their mother was receiving. This eventually enabled family members to leave the hospital for intervals to get some rest. The nurse related that this was a family whose members were affluent, educated, and well informed, and that they came in prepared with lists of questions. A consistent group of nurses who were familiar with Mrs. Walsh’s particular situation helped both family members and nurses to be more satisfied and less anxious. The family developed a close relationship with the three nurses who consistently cared for Mrs. Walsh and shared with them details about Mrs. Walsh and her life.
· The nurse related that there was a tradition in this particular critical care unit not to involve family members in care. She broke that tradition when she responded to the son’s and the daughter’s helpless feelings by teaching them some simple things that they could do for their mother. They learned to give some basic care, such as bathing her. The nurse acknowledged that involving family members in direct patient care with a critically ill patient is complex and requires knowledge and sensitivity. She believes that a developmental process is involved when nurses learn to work with families.
· She noted that after a nurse has lots of experience and feels very comfortable with highly technical skills, it becomes okay for family members to be in the room when care is provided. She pointed out that direct observation by anxious family members can be disconcerting to those who are insecure with their skills when family members ask things like, “Why are you doing this? Nurse ‘So and So’ does it differently.” She commented that nurses learn to be flexible and to reset priorities. They should be able to let some things wait that do not need to be done right away to give the famil.

Working With Families:
The Case of Carol and Joseph
Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrangement
with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed
the genetics of her family, and she said that she realized that she
had deteriorated rapidly since beginning to drink and knew that
she simply could not drink alcohol.
Joseph’s mother is deceased, and his father travels extensively
in his job and is not available as a support. Joseph was
very devoted to his mother and was devastated by her premature
death. We discussed the strengths that he and Carol demonstrated
in staying together and working out their p.

Racial and ethnic identity (REI) measures are in danger of becoming conceptually meaningless because
of evaluators’ insistence that they conform to measurement models intended to assess unidimensional
constructs, rather than the multidimensional constructs necessary to capture the complexity of internal-
ized racial or cultural socialization. Some aspects of the intersection of REI theoretical constructs with
research design and psychometric practices are discussed, and recommendations for more informed use
of each are provided. A table that summarizes some psychometric techniques for analyzing multidimen-
sional measures is provided.

In counseling psychology, the measurement of racial identity
constructs is a relatively new phenomenon. Arguably, the practice
began when Jackson and Kirschner (1973) attempted to introduce
complexity into the measurement of Black students’ racial identity
by using a single categorical item with multiple options (e.g.,
“Black,” “Negro”) that the students could use to describe them-
selves. Helms and Parham (used in Parham & Helms, 1981) and
Helms and Carter (1990) built on the idea that assessment of
individual differences in racial identity is important, and they
added complexity to the measurement process by (a) developing
measures that were based on racial identity theoretical frame-
works, (b) using multiple items to assess the constructs inherent to
the theories, and (c) asking participants to use continua (i.e.,
5-point Likert scales) rather than categories to self-describe. These
principles underlie the Black Racial Identity Attitudes Scale
(BRIAS; formerly RIAS–B) and White Racial Identity Attitudes
Scale (WRIAS).
In response to perceived conceptual, methodological, or content
concerns with Helms and associates’ racial identity measures,
many rebuttal measures followed. Rebuttal measures are scales
that the new scale originator(s) specifically described as correc-
tions for one or more such deficiencies in preexisting identity
measures (e.g., Phinney, 1992, p. 157). Subsequent measures have
tended to rely on the previously listed basic measurement princi-
ples introduced by Parham and Helms (1981), although the theo-
retical rationales for the measures have varied. Phinney’s Multi-
group Ethnic Identity Measure (MEIM), the most frequently used
of the rebuttal measures to date, added the principle of measuring
“ethnic” rather than “racial identity,” which she seemingly viewed
as interchangeable constructs. The MEIM also introduced the
principle of measuring the same identity constructs across racial or
ethnic groups rather than group-specific constructs within them.

It wasn’t until just now, taking this class and through this DB assignment that I stopped to think – Just what is a Web Page?
Well, after many aimless episodes of surfing, I have now been informed – A Web Page is a page that can accessed through the internet, via the web.
It is a page that may contain 2 –dimensional or 3-dimensional graphics, audio bytes, video clips, an email address for replies, or other various outlets for user feedback.
The underlying language for web pages are written in HTML, which allows for formatting and other ActiveX controls (Web Page, 1999).

I have written many of documents which contained portions of Word and Excel, but I never knew that there was a definition or label for them, but they are considered “Compound Documents”.
I just always took for granted that Microsoft had given its users the ability to produce such documents without much thought.
But, there is much to be learned from this everyday use of complimenting applications.
The ability to combine applications from other programs, such as, inserting an audio or other media clip from one application with another, such as, a word processing application requires activating or loading its associated user interface.
The technology (DDE, OLE) that allows the user to manipulate complimenting applications, and serve them up in a single context is called COM, structured storage, and uniform data transfer (Compound Documents, n.d., para. 1).
Now, producing compound documents can be done by either linking or embedding.
The difference between the 2 methods is in how and where the objects source data is stored.
The residence of this data determines the portability and how the updates perform, and the size and structure of its containers (Microsoft, n.d. para. 1).
Linked objects inside a compound document still reside in its origin, and this is just a reference (Linked Objects, n.d. para.1).
Whereas, embedded objects are stored in the document, along with information used to manage it (Embedded Objects, n.d. para.1).
My conclusion from the above information, is this, - Web Pages and Compound Documents, such as External Excel spreadsheets linked to a Word document share many of the same characteristics, in that, they combine data from outside sources, but in a compound document, there is 2-way communication, the user not only can view the information, but also manipulate it, unlike a web page.

How do emotions affect critical thinking?
“Each human emotion mobilizes the mind and body to meet one of the challenges of living and reproducing in the cognitive niche. Some challenges are posed by physical thins, and the emotions that deal with them, like disgust, fear, and appreciation of natural beauty work in straightforward ways. Others are posed by people. The problem in dealing with people is that people can deal back. The emotions that evolved in response to other people’s emotions, like anger, gratitude, shame, and romantic love, are played on a complicated chessboard, and they spawn the passion and intrigue that misleads the Romantic” (Pink 1977).
Emotions play a big role in critical thinking; emotions have a tendency to guide us in everything we do. From crossing the street, to going a particular way to work or school, you know that if you cross without looking the possibility would be to get hit by a car, or going to work or school you travel the same route every day, but lately you go a different route because you know that the road is under construction.
When you thinking of these things before you leave, you are, planning a new way to work and leaving earlier to be there on time. And when crossing the street you look both ways before you cross, this is learned at a very early age, so it becomes a natural instinct to do any time you are crossing a street.
But there are times perhaps, you are not thinking, because you are talking on your phone or with a friend and forgetting what you are doing.
I am pretty sure we have all done this at one point or another in our lives and that is you are driving down the road, a road you have traveled many times, the weather is nice and the radio is going and you are just cruising before you know it you are going over the speed limit. You look up and see blue lights in your rear view mirror, now the thinking comes in, how do you explain this one. Yes, it happened to me, was going into town and knew the cops sit on that road, but was enjoying the weather and the music playing and not paying attention to my foot on the pedal.
When the cop says, “Do you know how fast you were going?” I honestly could not answer him, I was only going about 10 miles over (but still over). He asked me why I was going so fast. My answer was honestly I was just enjoying the weather and the music and not paying attention. I was lucky because he said since I was actually honest about it; he let me off with a warning. Saved me money!
This is where emotions affected my critical thinking, because I was not thinking, I was in a happy, energetic, and general feel good mood, enjoying the weather and the music, I traveled faster then I should have, had I been paying attention to the speed limit signs I would not .

Healing is the body’s response to injury in an attempt to restore normal structure and functions.
Healing can occur in two ways: Regeneration and Repair
There are 4 phases of wound healing: hemostasis, inflammation, proliferation, and remodeling. This document also describes the mechanism of wound healing. Factors that affect healing include infection, uncontrolled diabetes, poor nutrition, age, anemia, the presence of foreign bodies, etc.

- 1. SOCW 8205/SOCW 6205 Case Study: Biopsychosocial Assessment Karen is a 40-year-old African American female. She is divorced and the mother of a 20-year-old daughter, Sarah. Sarah has a daughter who is 1½ years old. Karen lives in Queens, New York, with her mother, daughter, and granddaughter. Karen has a brother who is married with a child. She is very close to her family. Karen works full-time as an emergency room nurse. She has had this job for more than 15 years. Her ex-husband was an alcoholic, and he left Karen and Sarah when Sarah was born. Karen has no history of drug or alcohol abuse. Karen raised Sarah as a single parent, and Sarah has no contact with her father. Karen and Sarah regularly attend the same local Christian church as Karen’s mother. The family considers itself religious. The church community is very important in the family’s life. Six years ago, Karen was working a shift in the emergency room. Although she followed standard universal precautions, Karen was stuck by a needle
- 2. through her glove. She followed appropriate protocol for a needle stick according to hospital policy. Ultimately, Karen grew ill and she was diagnosed with HIV/AIDS. Karen has taken antiretroviral therapy for several years. She has had a good response to treatment. She is fully adherent to the HIV/AIDS treatment regimen. Karen’s family is aware of her HIV status but has not disclosed her status to friends or members of the church community. Karen was recently admitted to the hospital with a new diagnosis of leukemia. She was told that her condition was aggressive and she needed a series of treatments including chemotherapy. While on chemotherapy, Karen had an extremely difficult time. She had numerous painful and deteriorating side effects. Throughout all of this, Karen kept a bright sense of humor and the hope that she would recover. Her daughter and granddaughter were always in her thoughts. Karen’s mother spent many days and nights in the hospital. Karen allowed her daughter and granddaughter to visit on occasion, but she never told her daughter how bad her condition was. Karen was in the hospital for nearly a month. On some days, she was bright and cheerful. On others, she was so sedated on pain medication that she was not coherent. After multiple rounds of chemotherapy, Karen did not respond.
- 3. Her doctors ultimately told her that there was nothing else they could offer her. Karen and her mother were devastated. At this point, Karen was weak, gaunt, and had little ability to care for herself. Her wish was to go home to die. Karen’s family tried to take her home, but they could not care for her alone. Karen fell and developed a high fever that they could not control. Karen was readmitted to the hospital for a short stay and was ultimately transferred to a hospice facility where she died.
