been sexually active with 1 male partners in the past 3 months. She has never used any
form of protection or contraceptive. She reports she took a home pregnancy test 1 week
ago and that it was positive. She states her last menstrual period was almost 3months
ago. She Lives with parents and one older female sibling in a single-dwelling home. She is a
college student in local private Christian college. She describes herself as an “A-student”
and involved in several extracurricular school activities or athletics. She reports
experimenting with alcohol and cigarettes when she was in junior high school but that “both
made her so sick” that she did not continue to use. Denies illicit drug use in the past or
present. She reports feeling safe in her home. She’s had a 20-year old boyfriend for the past
two months. She denies history of emotional, sexual, and physical abuse. She reports she
feels safe in her home and when she is with her boyfriend. FICA assessment reveals that is
of Christian faith and belief and that her faith is very important to her. She attends church
on Sunday and is an active member of her college discipleship group. She holds her faith as
essential to her life and desires faith integration into her health care. The Nurse
Practitioner informs the patient that her urine pregnancy test is positive. The patient upon
hearing of her pregnancy began weeping. She told the NP that her parents and her
boyfriend would be devastated that she had become pregnant out of wedlock and that she
was terrified to share this information with them. The NP listened compassionately and
asked the patient to consider sharing the information with a trusted Christian mentor. The
patient said she knew just who she would seek guidance. The NP also referred the patient
to gynecology for prenatal care.The patient returned to the clinic two days later with her
boyfriend to discuss pregnancy options. Her appt is scheduled with gynecology in 10

The Case of Carol and Joseph
Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrangement
with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed
the genetics of her family, and she said that she realized that she
had deteriorated rapidly since beginning to drink and knew that
she simply could not drink alcohol.
Joseph’s mother is deceased, and his father travels extensively
in his job and is not available as a support. Joseph was
very devoted to his mother and was devastated by her premature

December 21, 2013, is the anniversary of a day that Paige can’t remember. In fact, she
can’t remember a full two weeks after this day, as she was in a medically induced slumber.
Concerned family watched as she lay in isolation, hallucinating and tossing and turning, her
blood count plummeting. Today, she enthusiastically announces this day as her “re-birthday.”
Paige, 52, her husband, and her three adult daughters live in Hemet, California. She
addresses her family’s illnesses with a holistic attitude, viewing diet, exercise, and spirituality as
having as much healing potential as pharmaceutical drugs. She values doctors who share this
perspective, yet who are also aggressive and candid. Her approach to medicine reflects a union
between the Western biomedical model and the growing trend of favoring natural and organic
remedies seen among the upper/middle-class public. Until the summer of 2008, she was
unaware of just how much these professional qualities would mean to her.
Summer was vacation time when Paige worked at a local public school. Yet, as a self-
described “Type A personality,” she liked to keep herself busy with other activities. However,
Paige soon became aware that she was unusually tired and requested a blood test from her
doctor. The results came back that she was intensely anemic. She was given a prescription and
changed her diet to include iron-rich foods.
By July, she had developed a painful lump on the back of her right knee, which was
diagnosed as a torn muscle. But the lump was still there when school started in August. While
supervising a detention class, Paige stood up from her seat, heard a popping noise, and
collapsed to the ground. By the time a student had gotten help, her knee had swollen to three
times its original size. Managing the pain long enough to wait for a friend to drive her to the
hospital, Paige received an MRI and a leg brace on the assumption that she had torn her ACL.
Two days later, she received a phone call from her doctor requesting an immediate
appointment. He would even keep the office open late until Paige and her husband were

1. Demographic information: The patient is a 17-year-old black girl. She is escorted by her parents to the clinic after a recommendation from the school counsellor.
2. Presenting problem: According to the father, "our daughter is not feeling well. She has not been attending classes and she tried to commit suicide. We think she is depressed".
3. History of present illness.: On today visit, patient present awake, alert and oriented, calm, cooperative, organized, with good hygiene.According to the young girl who was ready to share her feelings, she started feeling pressure from schoolwork when she was in her junior year. Her grades were not so good, and she felt useless and that is why she started being a truant. One year later, after starting her senior year, she feels the pressure is too much and she cannot manage to go through the year. One day before being referred to the clinic by the school counsellor, she was found bleeding in the school washroom after she slit her wrist.
4. Past psychiatric history: Since her frustration with school started a year ago, she has never been diagnosed with any psychiatric disorder. The parents though that she was simply going through a phase as a teenager and they were trying to help her cope with school and be a better person. No past psychiatric history. No history of Abuse.
5. Medical history: Despite her recent issue and the fact that she lost her weight suddenly, in the past few months, she is of perfect health. she had asthma and chicken pox when she was young, and she is allergic to penicillin.
6. Substance use history: She has no history of substance abuse. Her parents describe her as a very good girl.
7. Developmental history: Her mother had a successful pregnancy and gave birth to her through normal delivery. She grew up with her older siblings and she achieved all developmental milestones. The client is a 34-year-old Pakistani female who moved to the United States in her late teens/early 20s. She presents following a 21-day hospitalization for what was diagnosed as “brief psychotic disorder.” She was given this diagnosis as her symptoms have persisted for less than 1 month.
Prior to admission, she was reporting visions of Allah, and over the course of a week, she believed that she was the prophet Mohammad. She believed that she would deliver the world from sin. Her husband became concerned about her behavior to the point that he was afraid of leaving their 4 children with her. One evening, she was “out of control,” which resulted in his calling the police and her subsequent admission to an inpatient psych unit.
During today’s assessment, she appears quite calm and insists that the entire incident was “blown out of proportion.” She denies that she believed herself to be the prophet Mohammad and states that her husband was just out to get her because he never loved her and wanted an “American wife” instead of her. She says she knows this because the television is telling her so.
She currently weighs 140 lbs., and she is 5’ 5.
SUBJECTIVE
Client reports that her mood is “good.” She denies auditory/visual hallucinations but believes that the television talks to her. She believes that Allah sends her messages through the TV. At times throughout the clinical interview, she becomes hostile towards you but then calms down.
A review of her hospital records shows that she received a medical workup from a physician, who reported her to be in overall good health. Lab studies were all within normal limits.

The client is alert and oriented to person, place, time, and event. She is dressed appropriately for the weather and time of year. She demonstrates no noteworthy mannerisms, gestures, or tics. Her speech is slow and, at times, interrupted by periods of silence. Self-reported mood is euthymic. Affect is constricted. Although the client denies visual or auditory hallucinations, she appears to be “listening” to something. Delusional and paranoid thought processes as described above. Insight and judgment are impaired. She is currently denying suicidal or homicidal ideation.

Ms. Torrez is a 17-year-old Caucasian woman without prior psychiatric history who is brought to the Emergency room by ambulance after her parents called 911 when they found her having a seizure in their living room. She was admitted to the medical intensive care units in status epilepticus and was quickly stabilized with intramuscular lorazepam and fosphenytoin loading. Her heigh is 5 feet 6 inches, she is of medium build, and her weight is 101 lbs. (BMI16.3kg/m2). She does not suffer any medical conditions, and this is her first seizure. Laboratory workup shows an electrolyte imbalance as the most likely cause of the seizures. Although initially reluctant, she admits to purging with the use of ipecac several times this week. She reports that although she normally restricts her daily caloric intake to 500 calories, she regularly induces vomiting if her weight is above 100 lbs. Her last menstrual cycle was 1 year ago. The Case of Carol and Joseph
Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrangement
with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed
the genetics of her family, and she said that she realized that she
had deteriorated rapidly since beginning to drink and knew that
she simply could not drink alcohol.
Joseph’s mother is deceased, and his father travels extensively
in his job and is not available as a support. Joseph was
very devoted to his mother and was devastated by her premature
death. We discussed the strengths that he and Carol demonstrated
in staying together and working out their p.

He and his wife left Vietnam for the United States 7 years ago, and they own and operate a small nail salon and spa. Van was raised in a poor family and has little formal education. Aside from routine business conflicts, Van and his wife have gotten along quite well in their marriage and have always been supportive of each other. This changed abruptly last year when his oldest son, age 9, accused him of physical and sexual abuse. Because he described ongoing abuse while at school, Van’s son’s claim was reported to the state by one of his teachers. Since then, Van has had to make several court appearances, and this matter has put tremendous strain on the entire family. Although he denies the claims, Van has started to abuse alcohol since the accusation was made, and he will not talk to his oldest son or his wife. He does not use alcohol daily but is a binge drinker, consuming a large amount of whiskey two to three times per week and becoming very intoxicated. Van has never seen a therapist but is now court-ordered to seek psychological evaluation and treatment. He feels wrongly accused of these acts, although he admits that he has used physical punishment on both boys in the past. He believes that physical punishment is accepted in his native culture but is not understood in the United States. At this point, his marriage has suffered, as has his relationship with both sons. December 21, 2013, is the anniversary of a day that Paige can’t remember. In fact, she
can’t remember a full two weeks after this day, as she was in a medically induced slumber.
Concerned family watched as she lay in isolation, hallucinating and tossing and turning, her
blood count plummeting. Today, she enthusiastically announces this day as her “re-birthday.”
Paige, 52, her husband, and her three adult daughters live in Hemet, California. She
addresses her family’s illnesses with a holistic attitude, viewing diet, exercise, and spirituality as
having as much healing potential as pharmaceutical drugs. She values doctors who share this
perspective, yet who are also aggressive and candid. Her approach to medicine reflects a union
between the Western biomedical model and the growing trend of favoring natural and organic
remedies seen among the upper/middle-class public. Until the summer of 2008, she was
unaware of just how much these professional qualities would mean to her.
Summer was vacation time when Paige worked at a local public school. Yet, as a self-
described “Type A personality,” she liked to keep herself busy with other activities. However,
Paige soon became aware that she was unusually tired and requested a blood test from her
doctor. The results came back that she was intensely anemic. She was given a prescription and
changed her diet to include iron-rich foods.
By July, she had developed a painful lump on the back of her right knee, which was
diagnosed as a torn muscle. But the lump was still there when school started in August. While
supervising a detention class, Paige stood up from her seat, heard a popping noise, and
collapsed to the ground. By the time a student had gotten help, her knee had swollen to three
times its original size. Managing the pain long enough to wait for a friend to drive her to the
hospital, Paige received an MRI and a leg brace on the assumption that she had torn her ACL.
Two days later, she received a phone call from her doctor requesting an immediate
appointment. He would even keep the office open late until Paige and her husband were

1. Demographic information: The patient is a 17-year-old black girl. She is escorted by her parents to the clinic after a recommendation from the school counsellor.
2. Presenting problem: According to the father, "our daughter is not feeling well. She has not been attending classes and she tried to commit suicide. We think she is depressed".
3. History of present illness.: On today visit, patient present awake, alert and oriented, calm, cooperative, organized, with good hygiene.According to the young girl who was ready to share her feelings, she started feeling pressure from schoolwork when she was in her junior year. Her grades were not so good, and she felt useless and that is why she started being a truant. One year later, after starting her senior year, she feels the pressure is too much and she cannot manage to go through the year. One day before being referred to the clinic by the school counsellor, she was found bleeding in the school washroom after she slit her wrist.
4. Past psychiatric history: Since her frustration with school started a year ago, she has never been diagnosed with any psychiatric disorder. The parents though that she was simply going through a phase as a teenager and they were trying to help her cope with school and be a better person. No past psychiatric history. No history of Abuse.

boni Logan is a 16-year-old biracial African American/Caucasian female in 11th grade. She is an honors student, has been taking Advanced Placement courses, and runs track. Eboni plans to go to college and major in nursing. She is also active in choir and is a member of the National Honor Society and the student council. For the last 6 months, Eboni has been working 10 hours a week at a fast food restaurant. She recently passed her driver’s test and has received her license.
Eboni states that she believes in God, but she and her mother do not belong to any organized religion. Her father attends a Catholic church regularly and takes Eboni with him on the weekends that she visits him.

BACKGROUND
The client is a 34-year-old Pakistani female who moved to the United States in her late teens/early 20s. She is currently in an “arranged” marriage (her husband was selected for her when she was 9 years old). She presents following a 21-day hospitalization for what was diagnosed as “brief psychotic disorder.” She was given this diagnosis as her symptoms have persisted for less than 1 month.
Prior to admission, she was reporting visions of Allah, and over the course of a week, she believed that she was the prophet Mohammad. She believed that she would deliver the world from sin. Her husband became concerned about her behavior to the point that he was afraid of leaving their 4 children with her. One evening, she was “out of control,” which resulted in his calling the police and her subsequent admission to an inpatient psych unit.
During today’s assessment, she appears quite calm and insists that the entire incident was “blown out of proportion.” She denies that she believed herself to be the prophet Mohammad and states that her husband was just out to get her because he never loved her and wanted an “American wife” instead of her. She says she knows this because the television is telling her so.
She currently weighs 140 lbs., and she is 5’ 5.
SUBJECTIVE
Client reports that her mood is “good.” She denies auditory/visual hallucinations but believes that the television talks to her. She believes that Allah sends her messages through the TV. At times throughout the clinical interview, she becomes hostile towards you but then calms down.
A review of her hospital records shows that she received a medical workup from a physician, who reported her to be in overall good health. Lab studies were all within normal limits.
Client admits that she was tolerating her Risperdal well but stopped taking about a week after she got out of the hospital because she thinks her husband is going to poison her so that he can marry an American woman.
MENTAL STATUS EXAM
The client is alert and oriented to person, place, time, and event. She is dressed appropriately for the weather and time of year. She demonstrates no noteworthy mannerisms, gestures, or tics. Her speech is slow and, at times, interrupted by periods of silence. Self-reported mood is euthymic. Affect is constricted. Although the client denies visual or auditory hallucinations, she appears to be “listening” to something. Delusional and paranoid thought processes as described above. Insight and judgment are impaired. She is currently denying suicidal or homicidal ideation.

- 1. [From 10$/Pg] Home Pregnancy Test 1 [From 10$/Pg] Home Pregnancy Test 1 A 19-year old female presents to your clinic with complaints of nausea and vomiting every morning for the past week. She admits she has been sexually active with 1 male partners in the past 3 months. She has never used any form of protection or contraceptive. She reports she took a home pregnancy test 1 week ago and that it was positive. She states her last menstrual period was almost 3months ago. She Lives with parents and one older female sibling in a single-dwelling home. She is a college student in local private Christian college. She describes herself as an “A-student” and involved in several extracurricular school activities or athletics. She reports experimenting with alcohol and cigarettes when she was in junior high school but that “both made her so sick” that she did not continue to use. Denies illicit drug use in the past or present. She reports feeling safe in her home. She’s had a 20-year old boyfriend for the past two months. She denies history of emotional, sexual, and physical abuse. She reports she feels safe in her home and when she is with her boyfriend. FICA assessment reveals that is of Christian faith and belief and that her faith is very important to her. She attends church on Sunday and is an active member of her college discipleship group. She holds her faith as essential to her life and desires faith integration into her health care. The Nurse Practitioner informs the patient that her urine pregnancy test is positive. The patient upon hearing of her pregnancy began weeping. She told the NP that her parents and her boyfriend would be devastated that she had become pregnant out of wedlock and that she was terrified to share this information with them. The NP listened compassionately and asked the patient to consider sharing the information with a trusted Christian mentor. The patient said she knew just who she would seek guidance. The NP also referred the patient to gynecology for prenatal care.The patient returned to the clinic two days later with her boyfriend to discuss pregnancy options. Her appt is scheduled with gynecology in 10 days. While her boyfriend is in the waiting room, the patient tells the NP that she has not discussed her pregnancy with her Christian mentor but did tell her boyfriend. Her boyfriend is not a Christian and he wants her to have an abortion. The patient is tearful, saying that she doesn’t know what to do. She requests that the NP help them understand their options and the boyfriend joins the meeting with the patient and the NP. The boyfriend states he is in his fourth year of premed, that his father and grandfather are physicians and his career trajectory cannot be compromised by a child. He wants the NP to counsel them regarding options for pregnancy termination. He states he’s heard she can just take a pill and that technically would not be abortion. The NP is also a Christian, facing a
- 2. spiritual dilemma, as she believes that life begins at conception and that abortion is morally and ethically wrong. QUESTION:Identify and discuss a spiritual/moral/ethical dilemma that this patient is experiencing.
